Fredericksburg Health Inspections: Buffalo Mo's, George's Pizza

Health inspectors recorded violations at several facilities around Fredericksburg during the most recent round of restaurant inspections.

FREDERICKSBURG, VA — Health inspectors recorded violations at several facilities around Fredericksburg during inspections between June 29 and July 5.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • George's Pizza, 5306 Plank Road Fredericksburg, VA 22407
    • Date inspected: June 29
    • Total violations: 18
      • Person in Charge does not have a Certified Food Manager Certificate. Person in Charge did not demonstrate food safety knowledge relating to bare hand contact with ready-to-eat food, proper handwashing procedures, ensuring food is held at proper temperatures, and proper storage of chemicals.
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • Facility does not have a signed Employee Health Policy available.
      • Observed Person in Charge donning gloves without washing hands prior. Person in Charge was instructed to wash hands prior to donning gloves.
      • Observed Person in Charge handle sandwich without wearing gloves to prevent contamination. Person in Charge discarded food, washed hands, then donned gloves.
      • Observed meatballs held on steam unit holding at 66F, rather than 135F or higher as required. Food was put into oven to reheat.
      • Observed cheese (64F), sausage (71F), pan pizza (66F) sitting out of refrigeration for 2 hours. Food was relocated to refrigeration to cool.
      • Observed milk dated 6/10 stored in refrigerator. Food Employee discarded milk product.
      • Observed medication stored in cabinet above microwave, stored next to plastic single-use food bags. Person in Charge relocated medicine.
      • Observed food employees in kitchen area of food facility not wearing hair restraints such as hats or hair nets.
      • Observed knife and pizza cutter stored on soiled cardboard boxes on shelf above pizza prep area. Observed ice scoop stored in ice machine.
      • Observed pizza paddles and pizza pans stored on floor near oven area of food facility.
      • Observed tape and bungee cord on potato slicer handle, not a smooth and easily cleanable surface. Shelving in 3-door refrigerator observed to be rusted and is no longer smooth and easily cleanable.
      • Observed olives, tomatoes, salami, and cheese stored on paper towels in prep unit. Observed sheet pan used to stored cooked french fries to be broken. Observed container used to store french fries to be broken. Observed porous sponge stored in crisco container in back prep area of food facility, not a smooth and easily cleanable food contact surface.
      • Ceiling above kitchen area of food facility observed with an accumulation of static dust and is in need of cleaning.
      • Observed wall behind prep table to be damaged and is in need of repair.
      • Observed personal phone stored on prep unit cutting board and personal eye glasses stored on prep table. Person in charge relocated items to prevent contamination.
      • Air intake located above 3-compartment sink and ceiling vents located above kitchen area of food facility observed with an accumulation of static dust and are in need of cleaning.
  • Salem Fields Early Learning Center, 11120 Gordon Road Fredericksburg, VA 22407

      • Observed one of the handsinks of the kitchen had no paper towels available. During the inspection paper towels were provided.
      • Observed two dented cans on a food shelf. During the inspection dented cans were removed and separated.
      • Observed a food employee working without a hair restraint.
      • The wire shelves holding dried foods and equipment in the kitchen are soiled to the touch and sight.
      • The ac/ceiling vents in the kitchen require cleaning.

      • Facility does not have an available signed Employee Health Policy. Person in Charge was provided Employee Health Policy and was reviewed and signed.

      • Observed food employee preparing food with gloved hands stop preparing food and adjust handouts on bulletin board and not wash their hands before changing gloves to continue preparing food.
      • Observed the inside of microwave soiled.
      • Observed three food employees wearing wrist watches and bracelets while preparing food.
      • Observed the following items stored without being allowed to dry: Baking sheets, reusable plastic storage containers, bunt pans.
      • Observed multiple single-use Styrofoam to-go plates stored on rack without protection from contamination. Person in Charge inverted the containers to prevent them from becoming contaminated.
      • Observed the following non-food contact surfaces soiled: Baking ovens, Two door reach-in cooler, inside fryer, outside of dishwasher, outside of ice machine.
      • Observed the floors, the drain for 2 compartment sink, and the drain for the dishwasher soiled.
      • Observed the lighting by the flat top grill at 16 foot candles where food employee was preparing food.

      • Observed small flies throughout the kitchen.

      • Observed the 4 bathrooms in the front of the house are missing handwashing signs. EHS provided person in charge with handwashing signs.
      • Observed no hand soap was available at the handwashing sink of the bar area.
      • Observed less than 25ppm sanitizer level was detected at the chemical dish washing machine. EHS advised person in charge to manually sanitize dishes in the 3 compartment sink area.
      • Observed the interior surface of the microwave needs cleaning.
      • Observed a container marinara that was reheated to 130 degrees F. Marinara had been in hot holding unit for less than 20 minutes. Food employee reheated the marinara to 187 degrees F.
      • Observed a container of alfredo sauce made the day prior cold holding at an improper temperature of 50 degrees F due to improper cooling of the alfredo sauce. During the inspection the alfredo sauce was discarded.
      • Observed boxes of food stored on the floor of the walk in cooler and in the dry storage room boxes of single service use items and canned food were stored on the floor.
      • Recommend a deep cleaning of floors and walls of the kitchen and dishwashing area.

      • Observed no hot water at the hand sink.

      • Facility was unable to provide an employee health policy. Facility operates volunteer based members mostly. EHS discussed with the person in charge and provided an employee health policy for employees/volunteers involved in the preparation of food.
      • Observed the presence of small flying insects (fruit flies) throughout the kitchen area.

      • Several food stainless steel containers were observed soiled to sight and touch. Items were removed to be washed, rinsed and sanitized.
      • Observed accumulation of debris on the non food contact surfaces of all refrigeration units and dry storage shelving units.
      • Observed the following in need of cleaning: 1) floors under equipment, under the wired shelving units in the dry storage area on the second level, in the walk in cooler and freezer, 2) ceiling vents.
      • Observed the following in need of repair: 1) hand washing sink near the walk in cooler (WIC) faucet is leaking, 2) broken window by the drive-thru area, 3) broken light shield cover in the kitchen, 4) missing coving by the hand washing sink near WIC and in the dry storage area on the second level, 5) chipped paint on the ceiling by the hamburger oven, 6) broken floor tiles in the dry storage on the second level.

    • Date inspected: July 1
    • Total violations: 6
      • Observed food employee donning gloves prior to washing hands. Food employee was directed to wash hands.
      • Paper towel dispenser at back handwashing sink was not working properly and no paper towels were available at the handwashing sink. Person in Charge places paper towels next to handwashing sink.
      • Observed raw beef stored over garlic bread and marinara sauce in walk-in refrigerator. Person in Charge relocated beef.
      • Observed an accumulation of debris and residue on the inside of the microwave. Food employee cleaned microwave.
      • Observed marinara sauce stored out of refrigeration at 57F for less than 2 hours, rather than 41F or below as required. Person in Charge relocated marinara sauce to walk-in cooler to cool.
      • Observed food items (pasta, shredded cheese, sliced cheese, & tomatoes) stored in walk-in refrigerator and held for greater than 24 hours not being date marked as required. Food employee properly date marked food items.
  • Commonwealth Senior Living at Stafford, 30 Kings Crest Drive Stafford, VA 22554
    • Date inspected: July 1
    • Total violations: 16
      • The facility does not have a certified food protection manager
      • Observed uncovered cups that belong to the employees stored on the prep table. The person in charge relocated the cups.
      • Observed the employee handle dirty dishes with gloved hands then handle clean dishes from the dish machine with the same gloves. EHS discussed this with the person in charge.
      • Observed the employee wash her hands then turn off the hand sink with her bare hands. EHS discussed this with the person in charge.
      • Observed raw bacon stored over liquid eggs in the walk in cooler. The person in charge relocated the products.
      • Observed that the can opener (corrected), coffee machine, and juice machine require cleaning.
      • Observed dispensing utensils in 3 locations in the kitchen and the ice scoop that require cleaning.
      • Observed cottage cheese at 51'F and yogurt at 45'F cold holding at improper temperatures. The person in charge discarded the products.
      • Observed ready to eat foods like cooked onions and chicken salad that were dated for more than 7 days. The person in charge instructed the employees to redate the items.
      • Observed chemical spray bottles that are not labeled.
      • Observed foods stored on the floor in the walk in cooler (corrected) and walk in freezer.
      • The doors to the dish machine require cleaning.
      • Observed that there are no test strips for the quaternay sanitizing solution.
      • Observed a leak under the 3-compartment sink and dish machine.
      • Observed 2 lights out over the dish machine and the wall by the hand sink is deteriorating.
      • Observed that the walls and floors require cleaning.
  • The Ultimate Food Operation (VA 19-740), 4300 Prestwould Court Fredericksburg, VA 22408
    • Date inspected: July 3
    • Total violations: 6
      • No certified food protection manage (CFPM) available. Person in charge (PIC) has a certificate of course completion. PIC will email EHS once registered to take the CFPM exam. EHS provided a brochure with instruction on how to sign up to take the exam.
      • PIC is unable to provide documentation of training relating to the employee health policy for the mobile unit. EHS provided an employee health policy sample.
      • Observed a container stored in the handwashing (HW) sink and the HW sink was used to till the sanitizer bucket. Corrected on site.
      • Observed the following TCS foods cold holding at improper temperatures for 45 minutes: 1) cut watermelon (47'F), 2) cooked Jerk Chicken (51'F), 3) cut cabbage (49'F), 4) raw hamburger (49'F). PIC will cook the hamburger and will reheat the jerk chicken to 165'F before the 2 hours of allowed time have passed. PIC will discard the watermelon and cabbage if not used within 4 hours.
      • Ready to eat jerk chicken and cut watermelon were not date marked.
      • Observed bread stored over dish soap and sanitizer and all purpose disinfecting spray stored over the hot holding (HH) unit.
  • Rosie's Pizza & Doener Kebab (VA92-589), 33 Perchwood Dr 101 Fredericksburg, VA 22405
    • Date inspected: July 3
    • Total violations: 2
      • Observed 2 cutting boards scratched and scored. The person in charge reported they come into contact with food. Cutting boards are no longer easily cleanable.
      • Observed accumulation of debris on the following nonfood contact surfaces - Interior lids of pizza ovens, shelf under pizza ovens, fan, non food contact surface of the can opener. The can opener was removed for cleaning.
  • 716 Slice (VA 7ONE6), 7206 Hearthside Trail Fredericksburg, VA 22407
    • Date inspected: July 3
    • Total violations: 1
      • Observed that the dough mixer requires cleaning.

      • Observed food employee not wash their hands when switching between food prep and handling soiled equipment.
      • Observed handwashing sink blocked by speed rack.
      • Observed three pans of cooked chicken under walk-in cooler condenser not covered.
      • Observed food employee with gloved hands place their hands in their pocket and attempt to continue preparing food without changing gloves and washing hands.
      • Observed the following equipment soiled: Inside of ice machine and can opener.
      • Observed that made to order foods are not properly marked on the menu.
      • Observed sanitizer at 500 parts per million.
      • No food temperature devices located in the facility.
      • Observed test strips for dishwasher not in the facility.
      • Observed that dishwasher does not have correct detergent connected.

      • Observed the following in need of repair: Walls near walk-in cooler, Walk-in cooler door, Walk-in freezer door.
  • Crazy Dumpling, 1320 Emancipation Hwy Fredericksburg, VA 22401
    • Date inspected: July 5
    • Total violations: 3
      • Observed several food employees use the handwashing sinks to refill food containers, rinse dishes and store dirty dishes. EHS explained to the person in charge and food employees the purpose for handwashing sinks. Items were removed from the handink.
      • Observed the following: 1. Containers of raw shrimp, beef and raw shell eggs stored above RTE foods in an upright cooler and in a prep cooler. Person in charge rearranged items in the coolers. 2.Trays of food uncovered in the deep and uprigh freezer.
      • Observed the dish washing machine of the facility was not sanitizing and the sanitizer dispenser at the 3 compartment sink was not working properly (sanitizer solution was not dispensing). EHS advised person in charge to send an employee to the store to purchase chlorine. Before the inspection was over the facility had the 3 compartment sink set up to properly to wash, rinse and sanitize dishes.
  • Burger King, 740 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: July 5
    • Total violations: 14
      • The person in charge could not provide documentation of employee health training, reported the documents were removed from the facility. Food employees were knowledgeable of symptoms and illnesses requiring reporting and restriction. Instructed the person in charge to email signed employee health policies.
      • Through discussion with the person in charge the milkshake wand, which is attached to the ice cream machine, is being washed and sanitized once daily. The person in charge reported the equipment is sanitized between uses. Instructed the person in charge to wash and sanitize equipment at least once every four hours and she agreed to comply.
      • The food establishment is using time as a public health control, in lieu of temperature control, for sliced ham for a period not to exceed 4 hours without prior notification to the health department. The facility is adequately marking the container to include the discard date. Provided time as a public health control documentation during the inspection. Ensure records are maintained in the food establishment.
      • Observed Peracetic Acid vegetable wash is exceeding manufacturer requirements. Observed 11.76 ML and manufacturer requirements state between 5.88 and 7.81 ML when the solution was tested with the testing kit. Instructed the person in charge to discontinue use and contact technician for service.
      • Observed the drive-thru window is not self-closing, employees need to manually close the window. Observed the window open while waiting to provide order to guests in the drive-thru during the inspection.
      • Observed presence of flies in the back of the food establishment. Instructed the person in charge to contact pest control for service and treatment as needed.
      • Observed absence of hair restraints for two employees working in the kitchen.
      • Observed warewashing area handsink and the dump sink in the back of the establishment are not sealed to the adjacent walls.
      • Observed the following in poor repair: Significant ice build-up inside the 1 door upright freezer and hamburger deep freezer, and left hood vent above the hamburger grill is not sealed to the equipment. Discussed ensuring food packages are covered inside the 1 door upright freezer due to accumulation of ice inside the unit.
      • Observed accumulation of debris on the following nonfood contact surfaces: Interior of refrigeration equipment, shelving throughout including inside the walk in cooler, floors inside the walk in cooler, hot holding units, back and inside doors of fryers and oil transport equipment.
      • Observed absence of adequate backflow prevention at the mop sink. Observed a long hose attached to a spray nozzle, which was turned on, attached to a Y-connector at the mop sink faucet. The mop sink has an atmospheric vacuum breaker attached upstream of the Y-Connector. Instructed the person in charge to install a dual check valve with atmospheric vent to the hose at the Y-Connector. During the inspection the water was turned off and the person in charge reported the hose would not be used. The nozzle could not be disconnected during the inspection dur to significant accumulation of duct tape attached, the hose was leaking.
      • Observed the following in poor repair: Blown out ceiling lights in the kitchen and ceiling light cover by the hamburger grill is to sealed to the light fixture, observed it is bulging out.
      • Observed cleaning is needed on floors under shelving and equipment, ceiling tiles in the drive thru area and the mop and dump sinks.
      • Observed hood vents above the hamburger grill soiled and in need of cleaning.
  • Firehouse Subs, 1465 Stafford Market Place 101 Stafford, VA 22556
    • Date inspected: July 5
    • Total violations: 2
      • Per discussion with the person in charge the food contact surfaces for equipment and utensils (utensils and cutting boards) used for time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the requirements for the VA Food regulations. The items get wiped with a dry towel and get washed at closing time. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods. Items were removed to be washed, rinsed, and sanitized.
      • Observed several clear food containers stored on the shelving unit near the 3 compartment sink that were stacked while wet after cleaning and sanitizing. Corrected during the inspection.
  • Starbucks Coffee Company, 821 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: July 5
    • Total violations: 9
      • Observed a food employee wearing single use disposable gloves use a wiping cloth held in Quaternary Ammonium sanitizer on a surface and then proceed to prepare a customer's beverage order using the same gloves. Instructed the food employee to remove gloves and wash their hands. Discussed correct glove use with the person in charge.
      • Observed in house prepared vanilla cream (44 degrees F) improperly cooled in the 2 door dairy cooler. The person in charge reported the vanilla cream was prepared at 5:00 a.m. and temperature was observed at 1:30 p.m. Observed the vanilla cream stored in a deep plastic covered container. The person in charge voluntarily discarded the vanilla cream for safety. Discussed placing vanilla cream in smaller batches and monitoring internal temperatures to ensure ambient cooling is achieved within 4 hours.
      • Improper ambient cooling methods observed in the food establishment. Observed in house prepared vanilla cream improperly cooled to 41 degrees F within 4 hours. Observed vanilla cream prepared from refrigerated ambient air ingredients and stored in a deep and covered plastic container. Discussed cooling in smaller batches to achieve ambient cooling within 4 hours.
      • Observed the drive thru window is not self closing, observed drive thru window is left open when not receiving or providing customer orders. Ensure drive thru window is self closing when not in use.
      • Observed food employees preparing beverages without proper hair and beard restraints.
      • Observed wet wiping cloths at the espresso machines are not submerged in Quaternary Ammonium sanitizing solution, portion of the wiping cloths are exposed above the solution. Observed the wiping cloth container is shallow with a small amount of sanitizer. Instructed the person in charge to ensure wiping cloths are fully submerged in sanitizing solutions.
      • Observed the front handsink and left side toilet room handsink are no longer sealed to the adjacent walls.
      • Observed accumulation of debris (trash bags) behind the dumpsters inside the enclosure.
      • Observed floors under equipment and walls in the warewashing area in need of cleaning.
  • Wing Stop #483, 5635 Plank Road Fredericksburg, VA 22407
    • Date inspected: July 5
    • Total violations: 1
      • Facility does not have an available signed Employee Health Policy. Person in Charge was provided with Employee Health Policy that was reviewed and signed by all available employees.
  • Vocelli Pizza, 7008 Salem Fields Blvd Fredericksburg, VA 22407

    • Total violations: 2
      • Person in charge was unable to provide a thin probe thermometer.
      • Person in charge was unable to provide a test kit for the chemical sanitizer used in the facility.

    • Total violations: 3
      • Observed mixed vegetables stored in walk-in refrigerator held for greater than 24 hours not being date marked as required
      • Observed chemical sanitizer at 3-compartment sink at 100ppm, which is below the required amount for proper sanitization of equipment. More sanitizer was added and chemicals were then observed at 200ppm.
      • Food containers stored on shelves next to 3-compartment sink were observed with food residue on interior food contact surfaces of equipment. Food containers were brought to 3-compartment sink to be washed and sanitized.
