Fredericksburg Health Inspections: Flies Found In Multiple Kitchens

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between July 11 and July 18.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Vinny's Italian Grill, 20 Plantation Drive Suite 135 Fredericksburg, VA 22406
    • Date inspected: July 11
    • Total violations: 2
      • Observed presence of small flying insects in the dining rom (3-4), female toilet room (1) and back kitchen (3-4). Contact pest control for inspection and services as needed and provide copy of invoice.
      • Observed presence of dead flying insects on the front windowsill, electrical room and on small traps.
  • Subway (Bowling Green), 316 W Broaddus Ave Bowling Green, VA 22427
    • Date inspected: July 11
    • Total violations: 10
      • Observed an uncovered drink stored on the prep table. Drink was discarded during inspection.
      • Observed employee washing their hands for less that 5 seconds. Employee was instructed in proper handwashing techniques.
      • The back door of the facility is not self-closing.
      • Observed employees wearing watches and bracelets while preparing food.
      • Observed 2 employees preparing food without hair coverings.
      • The cutting boards along the deli counter are scratched and scored.
      • The nonfood contact surface of the garbage bag covering the slicer is not food-safe.
      • The water faucet on the 3 compartment sink does not appear to be working.
      • Both bathroom doors are not tight fitting and self-closing.
      • Observed cell phones and keys stored on the prep table and a handbag hanging from the clean pan rack.
  • Kokee Tea, 1672 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: July 11
    • Total violations: 4
      • Per discussion with the person in charge equipment that comes in contact with TCS food items (blender, spoons, etc.) currently only gets washed and rinsed in between uses. EHS discussed with the person in charge the need for wash rinse and sanitize step every 4 hours.
      • Observed food employee working while wearing wrist jewelry.
      • Observed leaking water from the fan/condenser inside of the walk in cooler, also observed an accumulation of water on the floor. All foods are covered in the walk in cooler and did not observe any immediate contamination from the water leakage. Highest recorded temperature of the walk in cooler was 44 degrees F. TCS items in the walk in cooler (milks) were relocated to a different cooler during the inspection.
      • Observed walls behind the 3 compartment sink, floor drain and mop sink basin in need of cleaning.
  • Quality Inn, 543 Warrenton Rd Fredericksburg, VA 22406
    • Date inspected: July 11
    • Total violations: 3
      • Observed an employee handle chemicals then prepared waffle mix without first washing their hands. Employee was instructed to wash their hands.
      • Soap was not provided at the only hand washing sink in the kitchen. PIC provided soap.
      • Observed commercially processed pre cooked eggs (122-126'F) stored in hot holding unit in the buffet area. Eggs were placed in the unit less than an hour prior to temperature check. Eggs will be discarded if not used within 30 minutes when breakfast is over.
  • Harmony at Falls Run, 60 Brimley Drive Fredericksburg, VA 22406
    • Date inspected: July 12
    • Total violations: 11
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection. Person in charge (PIC) will take the exam within 10 days.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS provided a handout to be posted.
      • No paper towels provided at two of the hand washing sinks located in the kitchen. Paper towels were provided during the inspection.
      • Observed the interior of ice machine to be soiled. Ice machine will be drained, cleaned, and sanitized.
      • Observed the following hot holding (149'F) at improper temperatures for less than an hour in the hot holding unit located by the cookline: 1) a container of cooked shrimp (129'F), 2) a container of pureed carrots (119'F). Items were reheated to 165'F. Hot holding unit was set to 175'F.
      • Observed spoons stored with the lip contact surface facing different directions. EHS discussed proper utensils storage with the person in charge.
      • Observed the following equipment not properly sealed to the adjoining wall: 1) hand washing sink by the two compartment sink, 2) dishmachine drainboard, 3) three compartment sink.
      • No temperature measuring device for the hot water dishmachine.
      • Observed the following non-food contact surfaces soiled: all handwashing sinks, two door refrigerator, exterior of the knives' holder, exterior of the stove, exterior of the dishmachine, exterior of the ice machine.
      • Observed a Y connector connected to the water line at the mop sink without the proper back flow prevention device. The hoses are being used for two portable air conditioning units.
      • Observed the floors (to include drains), the ceiling (to include light shields), and the walls throughout the facility in need of cleaning.
  • Royal Farms #335, 2216 Plank Rd Fredericksburg, VA 22401
    • Date inspected: July 12
    • Total violations: 4
      • The food-contact surfaces of the potato slicer and the chicken pan screens were observed soiled.
      • The chicken tenders in the hot holding case only measured 130'F. Person in charge voluntarily discarded chicken tenders during inspection.
      • The nonfood contact surface of the following equipment has accumulations of grime and debris: 1-the sides of the deep fryers in the kitchen, 2-the outer surfaces of the oven on the line, 3-outer surface of the hot holding unit, 4-outer surfaces of the hot holding drawer unit, 5-the up splash of the customer service soda dispenser.
      • The following physical facility items were observed soiled with grime and debris: 1-the walls throughout the kitchen, 2-the floors throughout the kitchen, 3-the swinging door between the self serv area and the kitchen.
  • Royal Farms #430, 4 Commerce Pkwy Fredericksburg, VA 22406
    • Date inspected: July 12
    • Total violations: 7
      • Observed a food employee come from the walk-in cooler from relocating boxes of foods and without washing their hands put gloves on and start handing cooked chicken. Observed another food employee move a trash can with gloves on then proceeded to change his gloves without washing his hands. The food employees washed their hands after EHS instructed.
      • Observed a empty soda, wet wiping cloth and a metal pan inside the hand sinks located in the kitchen and on the front line. During inspections items were removed.
      • Observed no paper towels at the only hand sink in the kitchen. During inspection paper towels were provided by a food employee.
      • Through discussion with a food employee the following food contact surfaces in use and in contact with TCS food are only being cleaned once every eight hours. During inspection items were removed to be washed, rinsed and sanitized. 1. Food thermometer 2. Chicken battering utensils.
      • Observed at the customer self-server beverage area the reach in cooler is holding the following TCS items at improper temperatures, through discussion with the person in charge items were stored in the reach in cooler for a couple days. All items were voluntary discarded during inspection, removed metal pans and added ice before adding new products. 1. Three cartons of oat milk (47 degrees) 2. Three cartons of Half and Half ((48 degrees) 3. Two plastic jugs of whole milk (50 degrees) 4. fourteen plastic container of coffee creamer (50-55 degrees).
      • Observed in the walk-in cooler water dripping from the fan on to packaged and covered foods. The person in charge rewrapped containers that had water on top and relocated all foods.
      • Observed a missing backflow device on the hanging hose above the mop sink.
  • Starbucks Coffee Company #23175, 821 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: July 12
    • Total violations: 5
      • Observed absence of hand drying provision at the handwashing sink in the front of the house. The person in charge provided disposable paper towels at the sink during the inspection.
      • Per discussion with the person in charge the food contact surfaces for the blenders and the utensils used for time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the VA Food regulations. The items sometimes get washed every six hours depending on the work load. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods.
      • Observed three refrigeration units without temperature measuring devices. EHS placed thermometers in the units during the inspection.
      • The person in charge could not provide an irreversible registering temperature indicator to monitor sanitization of equipment in the hot temperature dishwasher.
      • Observed equipment connected directly to the water line at two dump sinks in the front of the house with no back flow prevention device. The equipment is used to rinse the blenders and pitchers.
  • Padrinos II, 3249 Richmond Hwy Suite #104-106 Stafford, VA 22554
    • Date inspected: July 12
    • Total violations: 13
      • The facility does not have a person that is a Certified Food Protection Manager.
      • Person in charge was unable to provide written procedures for bodily fluid clean up. EHS provided a sample copy, advised person in charge to adapt the sample copy if needed.
      • Observed a gloved food employee working with foods (shredding cheese) go to the walk in cooler and attempted to come back and keep working with foods. EHS had a discussion with the person in charge regarding the observation.
      • Observed coffee in the handwashing sink of the kitchen.
      • Observed several soiled knives stored on a metal strip hanging above the 3 compartment sink.
      • Observed the following foods cold holding out of temperatures due to foods being stored in equipment not working properly (pizza prep unit and sandwich prep unit): ham-50 degrees F cheese-50 degrees F cooked onions- 50 degrees F mayo- 50 degrees F shredded mozzarella- 50 degrees F noodles- 50 degrees F ham- 48 degrees F cooked mushrooms- 48 degrees F pepperoni- 48 degrees F sliced mozzarella-48 degrees F ricotta cheese- 48 degrees F alfredo sauce 47-degrees F Per discussion with the person in charge foods had been inside the units for 1 hour. EHS advised person in charge to relocate foods to the walk in cooler. Before inspection was over relocated foods were at 41 degrees F.
      • Observed a container of antifreeze on the floor of the front kitchen area. During the inspection the person in charge disposed of the container.
      • Observed the following: 1. heavy build up of ice in all of the deep and upright freezers in the facility. 2. the sandwich and pizza cooling prep units are not maintaining required cold holding temperatures. (Sandwich prep unit 50 degrees F and pizza prep unit 48 degrees F).
      • Observed cardboard used to line equipment at cook line and on the floor in front of the cook line.
      • Observed numerous individual pizza pans with a build of grime on the outside of the pans (the non food contact surface of the pans).
      • Non food contact surfaced throughout the facility are in dire need of a deep cleaning. Sides of equipment at the cook line and inside surfaces of ALL cooling equipment in the facility were extremely soiled.
      • Walls, floors under equipment, ac/ceiling vents throughout the facility are in dire need of deep cleaning. EHS advised person in charge to also do a deep cleaning of the walls floors and ceilings of the walk in cooler.
      • Observed 2 large holes/openings behind the wall of the mop sink, also observed cracked floor tiles throughout the facility.
  • I Luv Teriyaki, 370 Garrisonville Road Suite #123 Stafford, VA 22554
    • Date inspected: July 12
    • Total violations: 8
      • Observed a food employee tending to the cash register attempt to put on a new pair of gloves to cut chicken without hand washing first. EHS discussed with the person in charge and employee.
      • Person in charge was unable to provide a thin probe thermometer (metal stem thermometer was available).
      • Observed several stainless steel containers that were stacked while still wet.
      • Observed rusted shelving throughout the facility and cardboard lining shelves and floors in the front service area.
      • In the back kitchen area observed handles missing from the upright cooler/freezer and a bottom panel cover missing from the 3 door upright cooler.
      • Person in charge was unable to provide a test kit for the chlorine used as sanitizer in the facility.
      • Observed the following non food contact surfaces in need of cleaning: 1. wire shelving throughout 2. inside surfaces off the coolers and freezers.
      • Observed floors underneath equipment and walls behind food prep areas and warewashing area require cleaning.
  • Pizza Hut Powhatan, 1224 Powhatan Street Fredericksburg, VA 22401
    • Date inspected: July 13
    • Total violations: 4
      • Observed two bins of stored clean utensils with food debris. During the inspection bins and utensils were relocated to the warewashing area.
      • Observed a bin stored silverware with the food contact surface facing upwards. During the inspection the person in charge relocated utensils to the warewashing area and discussed storing utensils with the food contact surface inverted.
      • Observed the following facilities in need of cleaning: 1. floors of the walk in cooler 2. ceiling tiles of the back prep area were observed with dust.
      • Observed coving missing around the mop sink basin and floor grout missing from the back cleaning room.
  • Dunkin Donuts / Baskin Robbins, 5231 James Madison Pkwy King George, VA 22485
    • Date inspected: July 13
    • Total violations: 2
      • Observed three sheet pans of fully cooked and previously frozen individually wrapped chicken salad and chorizo wraps at 61 degrees at 9:45 am. Through discussion with the person in charge wraps were pulled out the same day at 5:00 am. During inspection foods were relocated to the walk-in cooler .
      • Observed soiled ice cream scoops stored in the dipper well without running water. Scoops were removed to be washed and sanitized during inspection and water was turned on.
  • McDonald's #1630, 1212 Emancipation Highway Fredericksburg, VA 22401
    • Date inspected: July 14
    • Total violations: 10
      • Observed in the front hand sink pink/red liquid residue splashed inside the sink basin and in the back kitchen hand sink food residue (lettuce, pickles, tea) were observed inside the basin.
      • Observed one box of raw shell eggs stored above boxes of sliced cheese in the walk in cooler. During the inspection EHS relocated the shell eggs to a bottom shelf.
      • Observed a food employee manually washing dishes at the 3 compartment sink, observed employee doing a wash and rinse step but no sanitizing of the dishes was being performed. EHS discussed with the person in charge, person in charge instructed employee to sanitize dishes.
      • Observed the following cold holding at improper temperatures due to a cold holding unit not working properly, (thermometer inside the unit stated temperatures of the unit were 50 degrees F): 1. 3 containers of lettuce at 54 degrees F 2. 10 containers of sliced cheese at 51 degrees F Per discussion with the person in charge foods had been inside the unit since 6 am, temps were taken at 2pm. Foods mentioned were discarded for safety. Also observed one container and a half of breakfast burritos cold holdng between 44-47 degrees F. Burritos had been prepared and placed in the unit since 6am, temps were taken at 2pm. EHS observed burritos covered in the container and stacked. During the inspection burritos were discarded. EHS discussed ambient/cooling methods with the person in charge.
      • Observed the presence of flying insects in the kitchen area.
      • Observed food employees working while wearing wrist jewelry.
      • Observed food employees working while not wearing a hair restraints.
      • Observed clean and stored equipment that was stacked while wet.
      • Observed the back of the walk in freezer condenser/fan and the upright freezers with a build up of ice.
      • Observed the absence of a backflow prevention device at the frozen beverage machine that is directly connected to a water line.
  • Arby's, 418 Garrisonville Rd #118 Stafford, VA 22554
    • Date inspected: July 14
    • Total violations: 12
      • Observed employee put soap on their hands, then rub their hands together under running water (washing away the soap) for about 7 seconds.
      • The interior surfaces of the microwave ovens on the line were observed soiled. Ovens were cleaned during inspection.
      • The interior surfaces of the ice machine were observed soiled.
      • Did not observed the slicer being sanitized after being cleaned.
      • Breaded fried onions for which time rather than temperature is being used as a control is not properly labeled. Onions were marked with a 5 hour discard time. Onions were marked with a 4 hour hold time during inspections.
      • Observed many flying insects in the facility.
      • Observed an employee preparing food who was not wearing hair restraints.
      • The cutting boards along t he make line were scratched and scored.
      • The following non-food contact surfaces were observed soiled: 1-the walk in cooler door, 2-the outer and inner surfaces of the cheese dispenser, 3-shelving throughout the kitchen, 4-the outer surfaces of the shake machine, 5-sides of the ice machine, 6-the toaster, 7-the hot holding drawer unit, 8-the inside bottom of the line reach in freezer, 9-the upsplash of the soda dispenser in the dining room and drive thru, 10-outer surfaces of the ovens, 11-the floor fan, 12-the HAACP log book, 13-the shelving in the walk in cooler.
      • Observed a spray nozzle attached to a hose that was under pressure at the mop sink. Faucet was not equipped with a backflow protection device. Water was turned off during inspection.
      • The following physical facility items were observed in need of cleaning: 1-the floors throughout the kitchen, especially under the equipment in the kitchen, 2-the walls throughout the facility, 3-the floors, walls and ceiling of the walk in cooler.
      • Observed a cell phone laying on the prep table. Phone was removed during inspection.
