Fredericksburg Health Inspections: Jersey Mike's, IHOP, Pizza Hut

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between March 22 and March 28.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Hampton Inn, 2965 Richmond Highway Stafford, VA 22554
    • Date inspected: March 23
    • Total violations: 1
      • Observed 3 packs of scrambled eggs stored on the counter that measured 66'F. The person in charge placed the eggs in the reach in cooler. Observed that cream cheese measured 45'F and chocolate milk measured 45'F. The items were moved into another reach in cooler.
  • Souls Grill, 3341 Fall Hill Avenue Fredericksburg, VA 22401
    • Date inspected: March 23
    • Total violations: 4
      • The facility does not have a person that is a Certified Food Protection Manager.
      • Observed a heavy build up of ice inside the upright freezer.
      • Observed a water leak under the 3 compartment sink. Person in charge stated they are aware and a plumber has been contacted.
      • Observed the AC vent and the wall behind the trash can in the kitchen require cleaning.
  • Ike & Rita's Bakery & Cafe, 1517 Princess Anne St Fredericksburg, VA 22401
    • Date inspected: March 23
    • Total violations: 10
      • Person in charge was able to provide the written procedures for how the facility cleans up bodily fluids. During inspection EHS provided copy.
      • Observed a food employee come in from outside, put on new gloves and started preparing food for customer without washing hands first.
      • Through discussion with the person in charge, the steam wand of the espressomachine only gets sanized. EHS discussed the required wash rinse sanitize of equipment in contact with TCS foods.
      • Observed 2 spray bottles ( bleach & quick san) stored above or near dry packaged goods ( flour & brown sugar). During inspection chemicals were relocated .
      • Person in charge was unable to provide a thin probe thermometer.
      • Observed the only food employee working while not wearing beard restraint.
      • Observed the following non food contact surfaces needing cleaning : 1. Up splash of the coffee brewer 2. interior surfaces of all cooling and freezing units 3. rolling car 4.space in between the stackable ovens 5. shelving throughout the kitchen.
      • Observed the faucet of the hand sink is leaky.
      • Observed half of the kitchen floor unsealed and not smooth and easy cleanable.
      • Observed the floors throughout the kitchen in need of deep cleaning.
  • Jersey Mike's Subs, 14 Stafford Lakes Pkwy 104 Fredericksburg, VA 22406
    • Date inspected: March 24
    • Total violations: 6
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility During the inspection the person in charge (PIC) contacted their supervisor and electronic copy was provided, PIC will print and post the procedures.
      • Observed a food employee touch the order display screen at the front of the house and handled receipts then touched ready to eat pickles without first discarding their gloves, wash their hands, and don new gloves. Employee was instructed to wash their hands.
      • Per discussion with the person in charge the slicer and the knives used for cutting the sandwiches that contain time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the VA Food Regulations. The items only get washed only once a day. EHS explained to the person in charge cleaning frequency of utensils and equipment with TCS foods.
      • Observed a pan of tomatoes (44-49'F) cold holding at improper temperature in the front of the house sandwich prep unit for about seven hours. Tomatoes were discarded.
      • Observed two cardboard boxes of iced tea packets stored directly on the floor and under pipes under the beverage dispensing unit in the front of the house.
      • The nonfood contact surfaces of the interior of all refrigeration units and the bread oven were noted with an accumulation of grime and debris.
  • 2 Southern Belles (NH T-683808), 75 Wagoneers Lane Stafford, VA 22554
    • Date inspected: March 24
    • Total violations: 2
      • Observed that the fryer is in need of detailed cleaning.
      • The person in charge could not provide chlorine bleach strips.
  • Sheetz, 15 Wyche Rd Stafford, VA 22554
    • Date inspected: March 24
    • Total violations: 1
      • Absence of backflow preventer on cold brew/nitro equipment which is connected to the water line. Instructed the person in charge a backflow preventer is required and provided a handout.
  • TnT'z Smoked BBQ (TNTBBQ2), 9 Cool Well Court Fredericksburg, VA 22406
    • Date inspected: March 27
    • Total violations: 1
    • Note: This facility was temporarily closed after the inspection due to issues providing fresh water to the unit's sink.
      • The person in charge could not provide written procedures for clean-up of vomiting and diarrheal incidents. Sample procedures were provided during routine inspection on 4/11/2022 and during today's inspection.
  • Taco Bell #33516, 3551 Plank Road Fredericksburg, VA 22407
    • Date inspected: March 27
    • Total violations: 4
      • Person in charge was unable to provide a verifiable way of an employee health policy. During the inspection EHS provided a copy.
      • Observed several stainless steel containers that were stored stacked while still wet.
      • Observed the following non food contact surfaces that require cleaning: 1. shelving above 3 compartment sink 2. shelving in the back storage room 3. outside surfaces of the rice/sugar/cinnamon twist containers 4. up splash of the tea brewer.
      • Observed the following in need of cleaning: 1. wall and ceiling tiles behind and above the freeze drink machine 2. stained ceiling tiles throughout the kitchen area.
  • Pizza Hut # 1505, 2869 Richmond Highway Stafford, VA 22554
    • Date inspected: March 27
    • Total violations: 13
      • Observed a food employee don single use disposable gloves to engage in food preparation activities without washing their hands. Instructed the food employee to discard gloves and wash their hands.
      • Observed numerous stainless-steel pans stored on wire shelving in the warewashing area and soiled metal spatulas which were stored in a soiled container at the pizza production area. All equipment was relocated to the dishwasher for cleaning. Food contact equipment may be exposed to debris due to build-up of debris of shelving.
      • Observed chemical spray bottle labeled degreaser which held an unknown clear liquid. Degreaser used in the food establishment is a purple color. The person in charge discarded the spray bottle during the inspection.
      • Observed plastic pans used to hold pizza dough stacked while wet after chemical sanitizing on wire shelving in the back storage area.
      • Absence of an alarm or visual means to inform staff of low low detergent or sanitizer levels.
      • The person in charge provided two QT-10 testing kits which expired October 2022. Instructed the person in charge to purchase QT-40 test kits.
      • Significant accumulation of encrusted debris observed on nonfood contact surfaces of pans in the food establishment.
      • Significant accumulation of grime and debris on nonfood contact surfaces throughout the facility including shelving, interior of refrigeration equipment including gaskets, exterior of cooking equipment and fans covers inside walk in coolers. Strongly recommend detailed cleaning of nonfood contact surfaces throughout the facility.
      • Observed a variety of unused equipment being stored on the premises. Discussed removing equipment from the premises with the person in charge.
      • Observed the floors, walls, ceilings, and ceiling vents throughout the facility is in need of cleaning. Strongly recommend thorough and detailed cleaning throughout the food establishment.
      • Observed two mops sitting in the mop sink basin. The person in charge relocated the mops during the inspection.
      • Observed the following in poor repair: missing grout near the warewashing area, damaged ceiling tiles in the back storage room, numerous blown out ceiling lights, cracked floor tiles and cracks in ceiling above the 1 door reach in freezer.
      • Observed accumulation of dust and debris on hood vents in the vicinity of the pizza oven and above the fryer.
  • IHOP #4501, 320 Worth Avenue Stafford, VA 22554
    • Date inspected: March 27
    • Total violations: 14
      • Observed the cook crack shell eggs then continue to handle the equipment without washing his hands. The person in charge addressed this with the employee.
      • Observed that the waffle irons, floor mixer, and coffee machines are in need of detailed cleaning.
      • Observed ceramic plates, ceramic saucers, tomato slicer, lemon slicer that were stored dirty to sight and touch.
      • Observed mixed cheese at 53'F, sliced tomato at 43'F, pancake mix at 45'f, pancake batter - 60'F, pancake batter - 45''F, salsa - 81'F, sliced tomatoes at 47'F, and 2% at 54'F all cold holding at improper temperatures.
      • Observed that items like the steak and hamburger are not annotated with an (*) concerning consumer advisory.
      • Observed a pest control device installed over the hand sink. The person in charge removed the device.
      • Observed that bacon was laying on the prep area of the flat top grill to be cooked when 2 cooks touched the bacon with their dirty aprons.
      • Observed that the employees are working in the kitchen with required hair restraints (correct) and beard restraints.
      • Observed wet wiping cloths stored on the counter in the wait staff area.
      • Observed stainless steel utensils stored with the food-contact surface facing upwards. The person in charge inverted the silverware.
      • Observed that the ice machine door is in disrepair and rusty.
      • Observed that the doors to the dish machine require cleaning.
      • Observed that the PSI gauge is not reading correctly.
      • Observed the following non-food contact surfaces that require cleaning: interior of the oven, cabinets under the fryer, and the fans inside of the reach in cooler.
  • Battlefield Middle School, 11120 Leavells Rd Fredericksburg, VA 22407
    • Date inspected: March 28
    • Total violations: 1
      • Following food items were observed cold holding at improper temp on serving line: Chicken pieces 105'F, RIce 103'F, ,Pasta 93'F. Items had been on the line for a hour after being reheated to 165'F. Items were placed on sheet pan and chilled in walk in cooler and chilled to 41 degrees.
