Fredericksburg Health Inspections: Kickback Jack's, Warrior Coffee

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during inspections between Aug. 3 and Aug. 9.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Comfort Inn & Suites, 24058 Welcome Way Dr Ruther Glen, VA 22546
    • Date inspected: Aug. 3
    • Total violations: 7
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • The person in charge could not provide written verification that trailing concerning Employee Health had been provided to employees. A sample Employee Health Police was provided to the person in charge.
      • Employee was observed arranging ice and limes in a clear water dispenser with their bare hands. Employee was instructed to wear gloves while handling ice and limes.
      • The interior of the microwave oven was observed soiled. Oven was cleaned during inspection.
      • Observed many of what appeared to be fruit flies throughout the facility.
      • The hanging fan in the kitchen and the outer surfaces of the cereal dispenser on the serving line were observed soiled.
      • The counter top between the sinks in the kitchen (damaged) and the shelf inside the cabinet below it (water damage) are in need of repair.
  • Country Lane Catering, 120 Auction Drive Fredericksburg, VA 22406
    • Date inspected: Aug. 3
    • Total violations: 8
      • Observed the following TCS foods cold holding at improper temperatures inside the prep cooler: Cole slaw (44 degrees F) and cut lettuce (49 degrees F). The person in charge reported the TCS foods were pulled from the walk in cooler at 8:30 a.m., temperatures were observed at 9:52 a.m. During the inspection the person in charge relocated the foods in the unit to the walk in cooler to cool to 41 degrees F or less by 12:30 p.m. Discussed contacting a technician for service and discontinuing to use the unit unless it is capable of adequate cold holding of TCS foods at or below 41 degrees F or less.
      • Observed the following TCS foods not date marked to identify the date of preparation: Three trays of in house prepared meatloaf which the person in charge reported was prepared on 7/31/2022. Also observed the following commercially processed TCS foods which were not dated to identify the date of opening or discard: Opened container of milk, fresh mozzarella cheese balls and sausage gravy. The person in charge reported the milk and cheese were opened on 7/31/2022 and the sausage gravy was pulled from the walk in freezer on 7/31/2022. During the inspection the person in charge placed date labels on the containers to identify the date of opening and pull date from the freezer (sausage gravy). Discussed date marking requirements during the inspection.
      • Observed a lighter stored in a drawer with utensils. During the inspection the lighter was removed to prevent cross contamination.
      • Observed three boxes of soda syrup stored under the warewashing area handsink. During the inspection the boxes were relocated to prevent contamination.
      • Observed multiple boxes of packaged food on a dunnage rack stored under leaking and condensed water lines in the walk in freezer. During the inspection the dunnage rack was relocated to prevent contamination. Contact technician for service.
      • Observed one white, blue and black scratched and scored cutting board. Discussed replacing or resurfacing the cutting boards during the inspection. The surfaces are no longer smooth and easily cleanable.
      • Observed the Quaternary Ammonium test kit was discolored and expired on Jan 1, 2021. Instructed the person in charge to purchase a new test kit.
      • Observed accumulation of debris on the following nonfood contact surfaces: Upsplash of the front coffee brewer, interior doors and base of the convection oven, shelf over the range, rug inside the walk in freezer and rubber strips used to prevent slipping of pans. During the inspection the upsplash of the coffee brewer and the rubber strips were cleaned.

      • A splash guard is needed between the hand washing sink and the prep table by the cook line to prevent contamination.

      • Observed that the facility does not have written procedures pertaining to the clean up of bodily fluids. EHS provided a sample.
      • Observed damaged cans of soy sauce. The cans were relocated.
      • Observed foods in the sandwich prep unit and walk in cooler that are not protected from possible cross contamination.
      • Observed that the interior of the ice machine require cleaning.
      • Observed cut cabbage sitting on the counter at 66'F, pooled eggs in the sandwich prep unit at 49'F, and lo mein in the walk in cooler at 83'F colding holding at improper temperatures.
      • Observed that there is a fly strip installed over the dish drain.
      • Observed foods stored on the floor in the walk in freezer. The person in charge relocated the foods.
      • Observed employees preparing foods without wearing proper hair restraints. All employees donned proper hair restraints.
      • Observed that all the storage shelves, inside of the tops of the tongs, interior of the cabinets on the fryer, and the beverage tubes.
      • Observed that the handles to the faucet on the hand sink near the wok station is leaking.
      • Observed that the floors, walls, and ceiling throughout the kitchen is in need of detailed cleaning to include all of the air vent covers.
  • Pizza Hut, 5305 Jefferson Davis Hwy Fredericksburg, VA 22408
    • Date inspected: Aug. 4
    • Total violations: 6
      • Paper towel dispenser was not operational at warewash handwashing sink. Person in Charge provided available paper towels at handwashing sink.
      • Soda nozzles observed with residue build-up on food-contact surfaces. Equipment was brought back to warewash area to be washed and sanitized.
      • Observed clean food equipment in warewashing area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting).
      • Observed wall damage in back dry storage area of food facility.
      • Observed unused 1-door refrigerator and 2-door refrigerator in kitchen area of food facility.
      • Ceiling in pizza prep area of food facility observed with an accumulation of static dust and is in need of cleaning.
  • Dickeys Barbecue Pit, 15411 Dahlgren Road King George, VA 22485

      • the vents in the ventilation hood were observed soiled with grease and grime.
      • The ceiling tiles over the serving line and the stainless steel wall behind the deep fryer were observed soiled.
      • The following non-food contact equipment was observed soiled: 1-the door handles on the reach in freezer, 2-the shelf below the prep table, 3-outer surfaces of the hot holding cabinet, 4-the front of the line hot holding unit, 5-the gaskets on the prep unit, 6-both induction burners, 7-the grey cart.
      • Observed an employee working with food with a beard without a beard guard.
      • There were no paper towel at either hand sink in the kitchen. Paper towel was provided during inspection.
  • Warrior Coffee, 35 Walpole Street #115 Stafford, VA 22554
    • Date inspected: Aug. 4
    • Total violations: 18
      • Upon arrival the barista reported the manager/owner was not available. Instructed the employee to contact the owner who designated her to be the person in charge. Informed the employee during all hours of operation the facility is required to have a designated person in charge.
      • The person in charge, who is not a certified food protection manager, could not communicate clean-up procedures for vomiting and diarrheal events, is not monitoring disposition of commercially prepared and portioned TCS foods, is not adequately cleaning and sanitizing equipment in contact with TCS foods and is not monitoring sanitizer concentrations or could communicate correct Quaternary Ammonium concentration ranges. Education was provided and handouts emailed to the owner to adequately train staff.
      • The person in charge could not communicate or provide written procedures for clean-up of vomiting and diarrheal events. Handouts to develop written procedures were provided during routine inspection conducted on 7/23/21. Handouts were provided during the inspection.
      • Through discussion with food staff the following equipment are being sanitized only, absence of a wash and rinse step: Cutting board at the prep cooler and food thermometers. Demonstrated cleaning and sanitizing of food thermometers during the inspection. Instructed the person in charge to clean and sanitize equipment in contact with TCS foods at least once every four hours. The person in charge could not communicate how and how often the steamer wands for the espresso machine are cleaned and sanitized. Informed the person in charge the steamer wands, which are in contact with TCS beverages, are required to be cleaned and sanitized once every 4 hours. Instructed the person in charge to clean and sanitize equipment in contact with TCS foods at least once every four hours.
      • Observed significant build up of debris on the interior of the ice machine. Instructed the person in charge to discontinue use and clean the equipment. Through discussion with the person in charge the plastic buckets used to hold coffee beans are not being cleaned and sanitized, the bins are being refilled when empty. Instructed the person in charge to clean and sanitize the plastic buckets when they are empty.
      • Observed the following equipment soiled: 5 knives stored on knife rack behind the panini presses, food contact surfaces of the meat slicer. The knives were removed for cleaning.
      • Observed Quaternary Ammonium sanitizer in the sanitizer basin of the 3 compartment sink and a sanitizer spray bottle at 0 ppm. The sanitizer basin and spray bottle were not actively sanitizing equipment. The sanitizers were discarded during the inspection and the sanitizer spray bottle was filled with new sanitizer which was measured at 200 ppm and 84 degrees F.
      • Observed considerable build up of residue on the food contact surfaces of two panini presses and covered skillet.
      • The following commercially processed ready-to-eat (RTE) foods in the 3 door upright cooler were held for more than 7 days after the container was opened: Four containers of tomato bisque with a portioning date of 7/26/2022 (should have been discarded on 8/2/2022), one container of portioned macaroni and cheese with a portioning date of 7/27/22 (should have been discarded on 8/3/22), commercially processed hard boiled eggs with a portioning date of 7/23/2022 (should have been discarded on 7/29/2022). Also observed one container of in house prepared pippidou cheese spread with a prep date of 7/23/22 (should have been discarded on 7/29/22) and a container of in house prepared salsa with a prep date of 7/23/2022 (should have been discarded on 7/29/2022). During the inspection the foods were voluntarily discarded. Discussed date marking and disposition requirements. The facility appears to be holding TCS foods for a total of 8 days from opening/preparation.
      • Observed Quaternary Ammomium sanitizer spray bottle was not labeled to identify the contents. The spray bottle was labeled during the inspection.
      • Observed the back door of the food establishment does not fully close through the self-closing mechanism, Observed gap along the right hand side by the door handle. Manual closing is required. to create a tight seal.
      • Observed the only employee working in the kitchen without proper hair restraints. Discussed wearing a hat or other hair restraint to prevent contamination of food.
      • Observed three small stainless steel pitchers with the food contact surface facing upward without overhead protection. The equipment was inverted to prevent contamination.
      • Observed plastic stirring straws uncovered and stored in a cup by the bar where customers sit, and may be exposed to contamination.
      • The person in charge provided a Quaternary Ammonium test kit which expired June 2022. Instructed the person in charge to purchase a new test kit. Also observed the test papers discolored.
      • Observed accumulation of debris on the following nonfood contact surfaces: Interior of the prep cooler and 3 door upright cooler, including gaskets and exterior vents, panini press, base of the convection oven, meat slicer, interior of front cabinets, upsplash of the coffee brewer.
      • Observed the following in poor repair: Coving has separated from the wall under the 3 vat sink, blown out ceiling lights in the kitchen and damaged ceiling light covers, covers are extruding from the ceiling light frame.
      • Observed the following areas in need of cleaning: Interior of mop sink, air vents throughout, walls in the kitchen, floor drains, and wood trim in the toilet rooms.
  • Taco Bell/KFC, 5400 James Madison Parkway Dahlgren, VA 22485
    • Date inspected: Aug. 4
    • Total violations: 8
      • There was no measurable sanitizer being dispensed into the 3 compartment sink. Sanitizer was added manually and a repair call was made during inspection.
      • The cavity of the microwave oven at the rear of the kitchen was observed soiled. Oven was cleaned during inspection.
      • Observed 2 employees working with food without proper beard guards.
      • The counter at the self service drink station nees to be re-sealed.
      • Observed a heavy buildup of ice inside of the walk in freezer. There was ice clogging the condenser fans, covering the floor and shelves.
      • The following non-food contact surfaces were observed soiled with grime and debris: 1-the inner and outer surfaces of all hot holding cabinets, 2-the cabinet of the deep fryer near the thermolizer, 3-the top of the chicken fryers, 4-the hot holding drawer unit.
      • The following physical facility items were observed soiled: 1-the caulking along the 3 compartment sink, 2-the walls at both drive thru stations and the wall behind the breading station, 3-the top of the 1/2 wall behind the chicken fryers, 4-the floors under all of the equipment in the kitchen, 5-the floor underneath the self-service drink station.
      • The ceiling intake vents on above the make line were observed soiled.

      • The person in charge was unable to provide documentation of training relating to the employee health policy for this facility. EHS provided an employee health policy sample.
      • Observed a plate of food on the prep table near single service articles and equipment, per discussion with the PIC, an employee was eating when inspector started the inspection. Plate was re-located.
      • Food employee was observed handling chemicals then engaged in food preparation without first washing their hands. Employee was instructed to wash their hands.
      • No paper towel provided at the handwashing sink in the back of the house.
      • Observed raw shell eggs stored on the shelf in the prep unit near ready to eat pre-made sandwiches.. Eggs were re-located to a lower shelf with the raw meats that require the same cooking temperature.
      • The stainless steel measuring cups stored on the shelving units above the prep table were observed soiled to the sight and touch.
      • Observed the following TCS foods cold holding at improper temperatures in the walk-in-cooler (50'F): rice 46', pork 45'F, cut tomatoes (45'F), raw chicken (43'F), cooked chicken (44'F) PIC states WIC door was left open for about 30 minutes as they had just received a food delivery and they were arranging the boxes. EHS advised PIC not to leave door open for a long period of time. Items were placed in the freezer for a quick chill.
      • Observed dead insects on a sticky tape by the mop sink and by the cookline- tapes were removed.
      • The exterior door which is located in the kitchen is not tight-fitting. This may allow the entry of insects or rodents.
      • Observed several flying insects throughout the facility. Person in charge could not provide an invoice from at pest control company.
      • Observed canned foods and single service articles stored in a shed without proper overhead protection.
      • Observed employees in the back of the house preparing food without proper hair restraints.
      • Observed the floors and ceiling (to include vents) are in need of cleaning.

      • Observed an accumulation of bio-film-like residue on interior of ice machine.
      • Observed food containers stored in warewash area with residue and are in need of cleaning and sanitizing.
      • Observed sliced cheese holding at 50F in prep unit for less than 4 hours, food was relocated to cool. Observed shredded cheese holding at 54F in cold well for less than 4 hours, Person in Charge will use time-only and discard after 4 hours.

      • The person in charge could not provide documentation concerning training in employee health.
      • Facility does not have procedures for cleaning of bodily fluids.
      • Observed employees washing their hands in the 3-compartment sink. EHS discussed this with the person in charge.
      • Observed the employee touch her hair several times and then started preparing foods without washing her hands. EHS discussed this with the person in charge.
      • Observed the employee rinse his hands in the 3-compartment sink and dry them with a cloth towel.
      • Observed that there are no paper towels at the hand sink.
      • Per conversation with the person in charge, the equipment is not being sanitized. EHS discussed this at length with the person in charge.
      • Observed that the interior of the ice machine require cleaning.
      • Observed whole milk at 47'F and almond milk at 43'F in the reach in cooler cold holding at improper temperatures. EHS requested that the products be discarded.
      • Observed that the ready to eat foods like the sliced deli meats, cooked chicken, and cooked bul-go-gi are not dated for disposition.
      • Per conversation with the person in charge, he is spraying pesticides inside of the establishment. EHS discussed this with the PIC.
      • Observed bulk containers of foods that have been removed from their original packages that were not labeled.
      • Observed employees are working without proper hair restraints.
      • Observed equipment stored in water that measured 109'F, 93'F, 88'F, and 86'F.
      • The facility does not have any sanitizer.
      • Observed that the following non-food contact equipment must be cleaned: in between all floor-mounted equipment, outside of the ice machine, inside of all refrigeration units, all of the storage shelves, tops of all of the equipment, and the inside of the hot holding well.
      • Observed that the floors, walls, ceiling in the rear prep area are in need of detailed cleaning to include the air vent covers and the air vent covers in the restrooms.

      • Observed dispensing utensils that were soiled to sight and touch. The person in charge placed the items in the 3-compartment sink.

    • Total violations: 7
      • Observed 3 soiled rubber spatulas over a prep table. During the inspection, the spatulas were placed in the dishwasher area. (Corrected)
      • Observed 2 large plastic containers of cut romaine lettuce (47F - 48F) that were prepared on 08/07/2022. During the inspection, the PIC discarded the lettuce. (Corrected)
      • Observed several metal food containers on a shelf that were wet while stacked.
      • Observed eight cracked/chipped rubber spatulas hanging over a prep table. During the inspection, the PIC discarded spatulas. (Corrected)
      • Observed several walls in the kitchen and behind the dishwasher that are in need of cleaning.
      • Observed the floor tiles throughout the kitchen in need of regrouting.
      • Observed a purse, a personal beverage, and a cell phone on the prep table next to the tabletop mixer. During the inspection, the food employee relocated the personal items. (Corrected)

    • Date inspected: Aug. 8
    • Total violations: 29
      • Absence of active managerial control observed during the inspection. The person in charge is not monitoring cold holding temperatures of TCS foods, is aware of mechanical issues with the walk in cooler without procedures in place to monitor or relocate TCS foods, improper glove use observed, improper reheating for hot holding and the person in charge is not monitoring sanitizing solutions or clean condition of equipment. Discussed requirements during the inspection.
      • Observed an employee dry their hands on a dry wiping cloth on a a prep table. Instructed the person in charge rewash their hands and use the disposable paper towel to dry their hands.
      • Observed prep area handsink is blocked by a trashcan and linens and chemical spray bottles are on the prep area handsink. During the inspection the items were relocated to permit accessibility.
      • Observed a food employee place raw thin sliced steak used to prepare a cheesesteak wearing single use disposable gloves and then proceed to pull a roll out of a bag. The raw steak was covered with wax paper which covered only a portion of the raw steak. Instructed the food employee to remove gloves and wash their hands. Contamination was prevented.
      • Observed a box of shell eggs stored on top of a container of mayonnaise inside the beer walk in cooler. The eggs were relocated during the inspection.
      • Observed a food employee place raw shrimp stored in portioned bags into the deep fryer using single use disposable gloves, with the gloves contacting the interior and exterior of the bag in contact with the raw shrimp and then proceed to handle tongs. Instructed the food employee to remove gloves and wash their hands. Discussed correct glove use with the person in charge.
      • Observed interior of multiple microwave ovens soiled and observed four soiled fryer baskets. The fryer baskets were removed to the 3 compartment sink for cleaning.
      • Observed the following equipment in contact with Non-TCS foods soiled: Ice machine and 2 bar beverage guns and potato dicer.
      • Through discussion with food staff the kettle drum is rinsed out and scrubbed with water and sanitized in absence of a wash step. Discussed clean in place methods of cleaning the equipment and informed staff the kettle is required to be cleaned and sanitized.
      • Observed 170 ml of Lactic Acid sanitizing solution in the sanitizer basin of the 3 compartment sink when tested with a testing kit. The test kit states the concentration range is 272-700 ml. Worked with staff to manually titrate sanitizing solutions. A technician arrived during the inspection and the sanitizing solutions were being dispensed within normal limits. The sanitizer was not actively being used to sanitize equipment.
      • Observed the four knives attached to the magnetic knife rack in the prep area soiled. The knives were removed to the 3 compartment sink for cleaning.
      • Observed chicken wings in the hot holding unit at 113 degrees F. The person in charge reported the wings were previously cooked and cooled and reheated to 140 degrees F at 11:00 a.m., temperatures were observed at 12:30 p.m. The wings were removed to the convection oven to reheat to 165 degrees F or higher. Informed the person in charge foods cooled in the facility are required to be reheated to 165 degrees F or higher for hot holding. Observed chicken wings between 168 - 172 degrees F by 1:00 p.m.
      • Observed the following TCS foods cold holding at improper temperatures: 1- Walk in cooler: swiss cheese (47 degrees F), American cheese (47degrees F), cream cheese (45-46 degrees F), precooked hard boiled eggs (45 degrees F), milk (47degrees F), cooked ground beef (44 degrees F), blue cheese (45 degrees F), sliced deli ham (45-50 degrees F), philly cheese steak egg rolls (49-50degrees F), cooked spare ribs (47-48 degrees F), mashed potatoes (45 degrees F), in house prepared chili (46 degrees F), in house prepared cajun cream dressing (45 degrees F), in house cooked chicken wings (45-47 degrees F), cooked pasta (46 degrees F), cooked potatoes (45 degrees F). The person in charge reported the TCS foods have been in the unit overnight and voluntarily discarded for safety. The foods delivered at 10:00 a.m.. were relocated to the beer walk in cooler and placed on ice as applicable. The person in charge reported the fans are under repair and a technician was contacted during the inspection. Discussed placing food in ice baths/food grade bags until repairs are completed and monitor the temperature of TCS foods every 2-3 hours. Discussed limiting deliveries until the unit is repaired. 2- Prep cooler #1 - Spinach artichoke dip (50 degrees F), pico de gallo salsa (50 degrees F), cooked pasta (55 degrees F), cut cabbage (57 degrees F) and sliced turkey (56 degrees F). in the top rail of the unit. The person in charge reported the food was placed in the unit at 10:00 a.m. Observed some containers overfilled. Due to the heat at the cook line discussed placing small portions in the top of the unit and keeping the doors closed when not in use. The person in charge will use time as a public health control and placed a note to discard the food at 2:00 p.m. 3- Drawer cooler #1 - Raw shrimp (51 degrees F), cooked chicken (58 degrees F) and cooked onions (56 degrees F). The person in charge reported the food was placed in the unit at 10:00 a.m. The person in charge will use time as a public health control and placed a note to discard the food at 2:00 p.m. The person in charge reported the food was placed in the unit at 10:00 a.m. Due to the heat at the cook line discussed placing small portions in the top of the unit and keeping the doors closed when not in use. Discussed placing ice in food grade bags and having the HVAC system checked. The temperature in the facility may be affecting the cold holding capacity of the units.
      • Observed a Peroxide Disinfectant bottle stored on top of the dishwasher. During the inspection the spray bottle was relocated to prevent contamination.
      • Observed insect control device located above the back prep table near the potato dicer.
      • Observed presence of large and small flying insects in the facility. Also observed harboraging conditions including significant accumulation of water on the floors and soiled equipment. Contact pest control for inspection and service and eliminate harboraging conditions.
      • Observed food employees without proper hair and beard restraints.
      • Observed ice cream scoops stored in a percolator which was turned off sitting in room temperature water. During the inspection the percolator was turned on.
      • Observed plastic food storage containers in the warewashing area shelving which were stacked while wet after sanitizing.
      • Observed two stacks of small plates at the bar area, near the bar stools, which were stored with the food contact surface facing upward. During the inspection the plates were pushed under the bar shelving to prevent contamination. Discussed potential contamination by guests sitting at the bar.
      • Observed single use trays and wax paper used to line baskets in contact with food stored on a table next to prep cooler #3 with the food contact surface facing upward without any overhead protection. During the inspection the top tray was discarded due to being soiled and the top tray and baskets were inverted to prevent contamination.
      • Observed the following equipment in poor repair: Rust build up on the drain of the handsink by the ice machine, light out under the front and back hood vents, broken fan in the walk in cooler, currently only two of the three fans are working.
      • Observed multiple scratched and scored cutting boards in the facility.
      • Observed the 3 compartment sink and dishwasher drainboards are no longer sealed to the adjacent walls.
      • Observed accumulation of debris on the following nonfood contact surfaces: Dishwasher curtains, interior and exterior of refrigeration and cooking equipment including fryers, build up on the flattop grill, bulk container lids, shelving and build up of encrusted debris on pans used to cook chicken wings.
      • Observed the handsinks in the kitchen in need of cleaning due to buildup on the sides of the sinkguards and basins.
      • Observed significant build up on the exterior of the oil receptacle in the refuse enclosure.
      • Observed accumulation of debris in the dumpster enclosure.
      • Observed 12.7 foot candles of light at the dishwasher.

      • Observed a employee attempt to continue cutting raw chicken with gloved hands after using her personal cell phone. EHS instructed the employee to discard gloves and wash hands properly.
      • Observed the following: 1. A food employee was rinsing a wiping cloth at one of the kitchen handsinks. 2. A metal scrub pad inside one of the kitchen handsinks. 3. A server attempted to wash serving scoops in one of the kitchen handsinks. 4. Food debris inside one of the kitchen handsinks. EHS spoke with person in charge about the proper usage for handsinks.
      • Observed no handsoap at one of the kitchen handsinks. Person in charge provided soap during the inspection.
      • Observed the interior upper surfaces of the ice machine was heavily soiled with black growth.
      • Observed the hot water sanitizing dish machine was not sanitizing with hot water. VDH thermometer registered a temperature of 133 degrees F. EHS advised person in charge to manually sanitize dishes at sink.
      • The following items were cold holding at inadequate temperatures due to equipment not in proper working order. All food were at buffet lines. EHS advised person in charge to use time as a public healthcontrol for items mentioned until cold holding units get fixed. Foods had been in units less than 2 hours when temperatures were taken. 1. 4 slices of cheesecake-68 degrees F 2. One pan of cheese- 49 degrees F 3. One pan of spinch-47 degrees F 4. One pan of iceberg lettuce-47 degrees F 5. One pan of corn, black bean and chopped tomatoes-49 degrees 6. One pan of chopped hard boiled egg-44 degrees F 7. One pan of cottage cheese-45 degrees F 8. One pan of pasta salad-45 degrees F 9. One pan of crab salad-45 degrees F 10. One pan of potato salad-45 degrees F 11. One pan each of cut watermelon, cut cantaloupe, cut honey dew melon 45 degrees F

      • The (cavity, door seal) of the microwave oven is observed soiled. Oven was cleaned during inspection.
      • The can opener blade was observed soiled. Blade was cleaned during inspection.
      • The food contact surfaces of the large white cutting beard near the prep sink (stained) and the bulk bin scoops hanging over the bulk bins were observed soiled. Items were taken to the 3 compartment sink to be cleaned.
      • The prepared ready-to-eat (RTE) pork egg rolls in the refrigeration unit is not properly dated for disposition. Egg rolls were made the day prior. Rolls were date marked during inspection.
      • Observed many flies throughout the facility.
      • The screen door in the rear of the kitchen has large tears in it leaving large gaps.
      • Plastic cups and tin cans without handles were observed being used as scoops for cornstarch, sugar and rice. The scoops that should have being used were observed hanging on the wall behind the bins. Person in charge said that they will instruct employees to use the scoops.
      • The bulk storage bins for sugar, rice and cornstarch were observed soiled.
      • The walls throughout the kitchen are in need of a thorough cleaning. Walls are heavily soiled in some areas.
      • The ventilation hoods and hood filters were observed soiled with grime and grease.

      • Observed damaged cans of mushrooms on the storage shelf.
      • Observed numerous pizza pans that require cleaning.
      • Observed pepperoni at 45'F and mozzarella cheese at 44'F cold holding at improper temperatures. The PIC placed the items in the cold holding unit.
      • Observed a spray bottle of glass cleaner hanging on the front hand sink (corrected) and 2 spray bottles hanging on the equipment rack near the 3-compartment sink.
      • Observed several plastic containers that are stacked wet.
      • Observed that the pizza cutter blades are in disrepair.
      • Observed that the following non-food contact surfaces require cleaning: plastic crates in the walk in cooler, can opener holder, tops of the tongs, sprayer nozzle, and the floor fan.
      • The floors, walls, and ceiling requires cleaning to include all of the air vent covers and the floor-wall junctures.
