Fredericksburg Health Inspections: Kitchens Need Deep Cleaning
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Aug. 16 and Aug. 22.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions.
They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

- Observed serval flying insect throughout the facility. EHS discussed with the person in charge having their pest control company come more frequently.
- Dickeys Barbecue Pit, 15411 Dahlgren Rd King George, VA 22485

- Observed employee drink without a lid located on a prep table. Drink was discarded during inspection.
- Observed all food employees without hair restraints.
- Observed the following in need of cleaning: 1) exterior and interior of equipment throughout the kitchen 2) shelving over the dump sink 3) standing fan/
- Observed the floors, ceilings, and walls in need of cleaning.
- Observed the hood vents heavily soiled and in need of cleaning.
- Hardee's of Princess Anne #2920, 1700 Princess Anne Street Fredericksburg, VA 22401
- Date inspected: Aug. 16
- Total violations: 12
- Observed one pan and one lid soiled in the clean dishes shelf. Soiled items were relocated to the ware washing area.
- Per discussion with the person in charge the following are not being cleaned (wash/rinse/sanitize) every 4 hours: 1. cutting boards; 2. cooking utensils; 3. fryer baskets; 4. tomato slicer. EHS discussed with the person in charge the need to do the 3-step cleaning of all items that come into contact with TCS items every 4 hours.
- Observed the interior of both microwaves soiled. Per discussion with the person in charge only one is in use. EHS discussed the need to clean the interior of all microwaves at least every 24 hours.
- Observed a food employee discard chicken tenders on a sheet pan from the front hot holding area. The tray came into contact with the trash can, and then the employee put the tray back into the hot holding area. Discussed the observation with the person in charge and the tray was taken back to the ware washing area.
- Observed several stainless-steel containers being stacked while still wet. EHS discussed with the person in charge the need to dry containers before stacking them.
- Observed cutting boards heavily scored. EHS discussed with the person in charge the need to either resurface or replace cutting boards.
- Observed ice buildup in the walk-in freezer and the reach-in burger freezer and a broken bottom rack in the walk-in freezer. EHS discussed defrosting freezers and repairing broken rack in the walk-in freezer.
- Observed the following in need of cleaning: 1. seals around the freezers and coolers; 3. shelving throughout the facility, including the walk in cooler and walk in freezer; 4. the outside of all equipment; 5. up splash of coffee makers; 6. backsplash of drink dispensers; 7. rolling carts. EHS discussed with the person in charge the need to do a deep cleaning throughout the facility of all equipment nonfood contact surfaces. Overall it is recommended for a DEEP and THOROUGH cleaning of all non food contact surfaces throughout the facility.
- Observed multiple sheet trays with the non food contact surface EXTREMELY soiled with carbon buildup. EHS discussed with the person in charge the need to clean items to prevent accumulation of debris. It is HIGHLY RECOMMENDED to purchase new sheet trays if they are no longer cleanable.
- Observed at the mop sink a Y-connector attached to a long hose with sprayer. EHS discussed with the person in charge the need to have a backflow preventer between the Y-connector and the hose attached to sprayer. At the time of the inspection the spray nozzle was hung from the faucet.
- Observed the following in need of maintenance: 1. regrout the floor in front of the ice machine; 2. replace the light shield in the upstairs dry storage area; 3. replace the lighting unit that is out in the upstairs dry storage area; 4. fill in gaps around conduit and old conduit box in the upstairs dry storage area. EHS discussed with the person in charge who advised there is a work order in for the lights, and the other items would be addressed.
- Observed the following: black mold like growth around and on the AC vents, walls and ceilings with build of grease and dust, and floors throughout the facility heavily soiled. It is recommended the URGENT NEED to do a deep cleaning throughout the facility.
- Basilico NY Deli & Italian Bakery, 7013 Harrison Rd Fredericksburg, VA 22407
- Date inspected: Aug. 17
- Total violations: 11
- Facility does not have written clean-up procedures for a vomiting or diarrheal event. EHS provided a sample procedure during the inspection to the person in charge.
- Observed a food employee prepping food wash their hands for less than five seconds. EHS demonstrated the correct procedure for washing hands. During the inspection the food employee immediately washed their hands correctly.
- Observed a food employee washing strawberries at the handwashing sink in the kitchen. EHS discussed with the employee during the inspection that handwashing sinks are for washing hands only.
- Observed in the kitchen at the handwashing sink by the dishwasher no handwashing signage is posted. EHS provided signage to the person in charge.
- Observed the following foods not separated to protect from contamination: 1. Raw chicken next to salami deli meat (reach-in cooler) 2. Raw pork above swiss cheese (walk-in cooler) 3. Raw beef patty next to shredded cheese ( pizza prep cooler) During the inspection all foods were relocated.
- Observed the following food contact surfaces in need of cleaning: 1. Interior of ice machine inside the kitchen 2. Interior of Ice shoot on the soda machine 3. Soda nozzles
- Observed salmon fully thawing in reduced oxygen packages in the reach-in cooler. EHS discussed with the person in charge removing the salmon from the reduced oxygen packages prior to thawing. Foods were voluntary discarded during the inspection.
- Observed serval flying insects throughout the facility. EHS discussed with the person in charge contacting and scheduling pest control services more frequently.
- Observed several stainless steel pans stacked wet on the clean dish rack. Pans were removed during the inspection to be washed and allowed to air dry.
- Observed three knives being stored between the prep table and the wall. During the inspection knives were removal to be washed and sanitized.
- Observed employees beverages in the food prep area. Beverages were relocated during the inspection.
- Tarntip Thai Cuisine, 507 Emancipation Hwy Fredericksburg, VA 22401
- Date inspected: Aug. 17
- Total violations: 7
- The ice machine is soiled.
- Observed 1 unlabeled bottle with bleach in the bar area. Person in charge labeled the bottle during the inspection.
- Observed rust on the shelving in the walk in cooler and shelves lined with cardboard boxes throughout the facility.
- Observed the following in need of cleaning: 1) standing fans. 2) shelving racks above the three compartment sink and dry storage area.
- The following are in need of repair: grouting in kitchen tiles and warped ceiling tiles, and resurface of mop basin.
- Observed the following in need of cleaning: 1) ceilings. 2) floors underneath equipment. 3) walls. 4) drains in the three compartment sink in the kitchen and bar area.
- Observed employee personal belongings throughout the kitchen: bottled drink on expo center and iphone charging on top of deep freezer.
- Stevenson Ridge, 6912 Meeting Street Spotsylvania, VA 22553
- Date inspected: Aug. 18
- Total violations: 8
- Observed hot water at the men's and women's toilet room handsinks at 74 degrees F. Contact plumber to evaluate hot water.
- Through discussion with the person in charge salmon may be served raw and/or undercooked. Packaging on frozen salmon does not indicate if the fish is aquacultured. Instructed the operator to contact fish supplier to obtain parasite destruction documentation and offer the fish fully cooked until received.
- Absence of proper disclosure to identify the raw animal foods (tuna, salmon) which may be prepared in the raw and/or ready to eat form upon a customer's request. During the inspection an asterisk was placed next to the menu item which directs customers to the reminder statement.
- Observed the dishwasher drainboard and 3 compartment sink are not sealed to the adjacent walls.
- Observed numerous cutting boards which are scratched, scored and heavily stained. Discussed resurfacing or replacing for food contact.
- The person in charge could not provide a Chlorine test kit to monitor sanitization of the low temperature dishwasher.
- Observed build-up of encrusted debris on non-food contact surfaces of baking and frying pans. Discussed methods to remove debris.
- Observed accumulation of debris on the following nonfood contact surfaces: Knife rack, lids of bus tubs (not in use), blender, undershelf above the range, gaskets of refrigeration equipment, reservoir in front of dishwasher including tubing for the line-of-sight dispensers and pans on dry storage used to hold packaged spices.
- Chipotle Mexican Grill, 5621 Plank Rd Suite 100 Fredericksburg, VA 22407

- Observed several stainless steel containers soiled to the sight and touch. Items were moved to be cleaned.
- Panera Bread #1006, 10059 Patriot Highway Fredericksburg, VA 22407
- Date inspected: Aug. 21

- Observed the dishwasher using Ultra San for sanitizing has no ppm when tested. EHS Instructed the person in charge not to use the dishwasher until the equipment can be repaired and properly sanitizing, During the inspection the person in charge contacted the service provider. EHS advised the facility to use the three compartment sink to wash, rinse and sanitize until the dishwasher is repaired.
- Papa John's Pizza #3474, 2769 Richmond Highway Suite# 119 Stafford, VA 22554

- Total violations: 6
- Observed the blade of the can opener heavily soiled. During the inspection the can opener was relocated to the 3 compartment sink.
- Observed the presence of flying insects in the facility.
- Observed the following items being stored in the employee bathroom: 1. papertowels 2. one bag of cleaning rags 3. employee aprons 4. sponges for warewashing During the inspection the items were relocated out of the bathroom.
- Observed the two hand sinks and 3 compartment sink need caulking.
- Observed wire shelving require a deep cleaning, especially in the walk in cooler and the non food contact surfaces of the pizza oven.
- Observed the following in need of cleaning: 1. walls around handsinks, 3 compartment sink and prep areas 2. ac vents 3. fan vents in the walk in cooler 4. floors underneath equipment of the back kitchen area.
- Vinny's Italian Grill & Pizzeria, 201 King’s Highway Suite #103-104 Fredericksburg, VA 22405
- Date inspected: Aug. 21
- Total violations: 15
- Observed the current employee health policy was missing Salmonella (non-typhoidal). EHS provided a copy of the current employee health policy.
- The person in charge was unable to provide EHS with written procedures for clean-up of vomiting and diarrheal events. EHS provided person in charge with a copy of written clean-up procedures using bleach.
- Observed one uncovered beverage and a covered plate with employee's food in the prep area. The person in charge had the employee remove the items to a designated area.
- Observed a food employee handling soiled dishes and then handling sanitized dishes without washing their hands. Another food employee put their necklace under their shirt and then go back to the prep area without washing their hands. The employees washed their hands before returning to their areas.
- Observed the paper towel dispenser at the ware washing handwashing sink not dispensing paper towels. The person in charge was unable to fix the problem, they will have it fixed.
- Observed the following: 1. raw hamburger meat stored next to hardboiled eggs with the shells removed in the prep RIC #2; 2. raw chicken stored next to the milk in the WIC; 3. raw shell eggs stored next to individual dressing containers. The person in charge separated the raw items from the ready to eat items.
- Through discussion with the person in charge it was found that the containers of pasta are being cooled at room temperature for about 30 minutes and then put in the WIC covered. The following was out of temperature due to cooling issues: 1. one medium stainless-steel container of spaghetti noodles 45 degrees F in prep RIC #1; 2. one medium stainless-steel container of spaghetti 47 degrees F in 1D RIC near dough station. Person in charge voluntarily discarded the items.
- Observed the following cold holding at improper temperature overnight: 1) one small stainless-steel container of raw shrimp 43-45 degrees F in prep reach in cooler (RIC) #1; 2) one medium stainless-steel container of raw hamburger 45-48 degrees F in prep RIC #2; 3) one large plastic container of homemade ranch dressing 43-45 degrees F in WIC; 4) one large stainless-steel container raw breaded chicken 42-43 degrees F in WIC; 5) one box raw chicken 44-45 degrees F in WIC; 6) one small stainless-steel container of garlic in oil on the counter at 74 degrees F near the prep area. The person in charge voluntarily discarded the items.
- Observed multiple containers in the RICs and the WIC not date marked. The person in charge had one of the food employees date mark everything that wasn't already date marked.
- Observed chemicals being stored next to and above single service articles, on the back shelving unit near the WIC. The person in charge relocated the chemicals.
- Observed bug spray stored in the kitchen by the 3 compartment sink. Item was moved to the office.
- Observed several cardboard boxes of food stored directly on the floor.
- The following nonfood contact surfaces were noted with an accumulation of grime and debris. 1) shelving units throughout the facility, 2) on top of the dishmachine, 3) fryer cabinets, 4) pizza oven.
- Observed the floors, walls, and ceiling (to include vents) in need of detailed cleaning.
- Observed the following in need of repair throughout the facility: 1) missing coving, 2) cracked floor tiles, 3) all sinks need to be re-caulked.
- Chick-Fil-A at Southpoint, 9900 Southpoint Parkway Fredericksburg, VA 22407
- Date inspected: Aug. 21
- Total violations: 1
- Observed small flying insects throughout the frontline area and drive-thru. EHS discussed with the person in charge contacting their pest control company to further assess the insect issues.
- Kenmore Club, 632 Kenmore Avenue Fredericksburg, VA 22401
- Date inspected: Aug. 22
- Total violations: 7
- Observed the interior of the microwave was heavily soiled.
- Observed the chemical sanitizing dish machine is not sanitizing-0ppm were observed being dispensed. EHS advised person in charge to manually sanitize dishes at the 3 compartment sink.
- Observed one spatula, one mesh colander and the blade of the can opener soiled. Items were stored on a hanging rack. During the inspection items were relocated to the warewashing area.
- Person in charge could not provide a thin probe thermometer.
- Person in charge could not provide a test kit for the chemical sanitizing dish machine or for the sanitizer used at the 3 compartment sink. EHS provided a small sample of the quat sanitizing test strips.
- The following non food contact surfaces were observed in need of cleaning: 1. wire shelving throughout 2. drawers used to hold utensils.
- Observed floors underneath equipment and walls behind equipment require cleaning.
- George's Pizza, 5306 Plank Road Fredericksburg, VA 22407
- Date inspected: Aug. 22
- Total violations: 2
- The food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- Facility does not have a signed employee Health Policy available. EHS provided employee health documents to the person in charge during the inspection.
- NY Grill Kabob House, 537 Warrenton Rd Fredericksburg, VA 22406
- Date inspected: Aug. 22
- Total violations: 24
- The person in charge could not provide certified food manager certificate but reported a staff member in a management or supervisory role had a certificate. Instructed the operator to email copy of the certificate.
- The person in charge could not provide verification employees have been trained on the employee health policy. The person in charge was able to communicate symptoms and illnesses requiring reporting and handouts were posted. Provided sample employee health policy during the inspection to train staff.
- The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Sample written procedures were provided during the inspection.
- Absence of hand soap at the prep area hand sink. Hand soap was provided during the inspection.
- Observed a plastic container inside the basin of the prep area hand sink. Equipment was removed during the inspection.
- Observed the following raw animal foods improperly stored in the walk-in cooler: Raw lamb over box of uncut tomatoes and a container of raw chicken stored over box of packaged lettuce. During the inspection the raw animal foods were relocated.
- Observed the meal slicer soiled to the sight. Instructed the person in charge to clean and sanitize the meat slicer prior to next use.
- Observed fryer baskets on top of the fryer soiled to the sight. The person in charge reported they were from the previous day. Equipment was removed for cleaning.
- Observed tongs stored overnight in foods stored in refrigeration units. Equipment was removed for cleaning during the inspection.
- Through discussion with the operator and observation, the following equipment in contact with non TCS foods were observed soiled: Can opener, soda nozzles and floor mixer. The soda nozzles and can opener were removed for cleaning during the inspection. Through discussion with the operator the pillows used to place naan bread inside the tandoor oven and the pizza dough roller are cleaned every 2 days. Instructed the person in charge to clean and sanitized these surfaces at least once a day.
- Observed deep and covered container of cooked chickpeas (47 degrees F) improperly cooled inside the walk-in cooler. The person in charge reported the chickpeas were prepared the evening prior and temperatures were not monitored during the cooling process. The person in charge voluntarily discarded the food during the inspection. Cooling handouts and cooling methods were discussed.
- Observed the following TCS foods cold holding at improper temperatures inside the pizza prep cooler: Shredded mozzarella cheese (49 degrees F), pizza sauce (48 degrees F), precooked sausage (51 degrees F), and sliced salami (52 degrees F). The person in charge reported the TCS foods were held in the unit overnight and voluntarily discarded for safety. Instructed the person in charge to contact technician for service to evaluate the cold holding capacity. Store TCS foods in another unit until repairs are completed.
- Absence of date marking system in the food establishment for TCS foods which held over 24 hours. Develop and implement a date marking system.
- Observed improper cooling methods to cool cooked chickpeas. Observed chickpeas in a deep and covered plastic container in the walk-in cooler. Chickpeas were observed at 47 degrees F and prepared the evening prior. Discussed cooling in smaller portions, using ice baths and monitoring of temperatures throughout the cooling process.
- Observed tongs stored in food containers in the prep cooler and the scoop stored inside the flour bin with the handles touching the food. Dispensing utensils were removed for cleaning.
- Observed single use cups and lids for sauces/dressings stored in a soiled plastic container. Single use articles were discarded and the containers were taken to the warewashing area for cleaning.
- Observed the following equipment in poor repair: Gaskets of deep freezers and pizza prep cooler are separating, ice build-up in deep freezers, and broken door of oven when health department staff attempted to open the door (fell off).
- Absence of data plate on dishwasher. Contact technician to install a data plate.
- Observed accumulation of debris on nonfood contact surfaces throughout the facility including shelving, cooking equipment, refrigeration gaskets and interior of refrigeration equipment, and beverage equipment, exterior reservoir of the dishwasher including tubing and exterior of stainless-steel containers on warewashing shelving. Strongly recommend a deep and thorough cleaning. Observed slime-like growth on nonfood contact surfaces under the soda machine tray. Instructed the operator to clean and sanitize the equipment prior to use.
- Observed hose with a sprayer (turned off) in absence of any backflow preventer. Hose was removed during the inspection.
- Observed the following in poor repair: Gaps in ceiling tiles around conduit cables, gap in wall behind dishwasher, damaged ceiling in the beverage area and burned-out ceiling lights.
- Observed the walls, ceilings including air vents throughout the facility are in need of cleaning.
- Observed hood vents above the cookline in need of cleaning, visible residue was observed.
- Observed employee foods and equipment stored with food and equipment for customers. Instructed the operator to segregate and label containers/areas for employee articles.
- Fredericksburg Moose Lodge #1655, 11917 Cherry Road Fredericksburg, VA 22407
- Date inspected: Aug. 22
- Total violations: 1
- Observed Eggs on the breakfast menu may be cooked to order upon a customer's request without a consumer advisory. Informed the person in charge the menu requires a consumer advisory for eggs which may be cooked to order. During the inspection the person in charger marked the eggs to order on the menu with an advisory.
- Quality Inn, 2310 Plank Road Fredericksburg, VA 22401
- Date inspected: Aug. 22
- Total violations: 11
- Through discussion and observation the facility is lacking in active managerial control due to the following: 1. employee on duty was not aware/trained on the facilities employee health policy 2. food contact surfaces are currently only getting washed and rinsed, no sanitize step. Food employee was not aware or trained on proper set up for 3 compartment sink and required concentration of the bleach sanitizing solution. 3. food employee could not answer basic food safety questions (cold holding/hot holding temperatures, sources of where food comes from for the facility, etc.). 4. MULTIPLE employees and employees children were observed walking through the kitchen area were food is stored.
- The facility does have an employee health policy in place, however the employee on duty, most recently hired 2-3 months ago, was not informed of the facilities employee health policy. During the inspection employee signed the policy. Per discussion with the employee there are only 2 employees that tend to the breakfast service, other food employee has signed the policy.
- Upon arrival to the facility for an inspection, observed one spoon and one plastic container sitting in the basin of the only hand sink of the facility. During the inspection a food employee removed the items. EHS discussed with the food employee to ensure a handwashing sink is used for hand washing only.
- Per discussion with the breakfast attendant, the facility is currently only washing and rinsing dishes. EHS discussed with the food employee to ensure all dishes are going through a wash, rinse and sanitize step. EHS trained employee on 3 compartment sink set up.
- Observed the interior of the microwave requires cleaning.
- Observed one spray bottle of comet cleaner and one container of fabuloso cleaner stored on a table next to a toaster and waffle maker (both pieces of equipment not in use). During the inspection chemicals were relocated to separate shelf.
- Observed the cereal containers offered for consumer self service are missing the required labeling.
- Observed a food employee working without a proper hair restraint.
- Observed 3 containers of plastic utensils with the food contact surface inverted upwards. During the inspection utensils were inverted.
- Observed the following non food contact surfaces require cleaning: 1. interior and exterior of the cooling/freezer units 2. upsplash of the coffee brewers 3. shelving in the kitchen 4. countertops under equipment.
- Observed the sink drain, floors and walls of the facility require cleaning.
- The Timbers Restaurant, 8026 Prosperity Way Ruther Glen, VA 22546
- Date inspected: Aug. 22
- Total violations: 12
- The person in charge was unable to provide written clean-up procedures for vomit and diarrheal events. EHS provided a copy of clean-up procedures.
- Observed the cutting boards along the prep units are heavily stained. Person in charge advised they will sand the boards down to remove the stains.
- Observed that the interior of the ice machine was soiled.
- Observed two ROP tilapia, with the packaging still intact, thawed in a stainless steel container in the refrigerator/freezer combination cooler. The person in charge voluntarily discarded the tilapia.
- Observed the following in need of re-caulking: 1. handwashing sinks in the kitchen; 2. one compartment sink near the dishwasher; 3. the metal counter coving behind the drink prep area. Reseal equipment to the wall.
- Observed the interior of the dishwashing machine heavily soiled.
- Observed the following soiled: 1. seals of the reach in freezer, halo heat warmer, pizza prep reach in cooler; 2. ovens under the stove top; 3. interior of pizza oven; 4. drawers under the stove top; 5. shelving throughout the facility; 6. outside of the plastic storage drawers.
- Observed no covered receptacle in the women's restroom. A covered receptacle needs to be provided.
- Observed the toilets in both the women's and men's restrooms soiled. All toilets need to be cleaned.
- Observed the following soiled: 1. walls in the backroom; 2. ceiling tiles; 3. light fixture covers.
- Observed two light fixtures in the kitchen area not working.
- Observed the air vents throughout the facility heavily soiled.
