Fredericksburg Health Inspections: Living And Dead Crawling Insects

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Nov. 15 and Nov. 21.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • John J. Wright Center, 7565 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Nov. 15
    • Total violations: 2
      • Through discussion with the person in charge the dishwasher has not worked in 15 years.
      • Observed following physical facilities in poor repair : 1.Several ceiling tile displaced throughout the dry storage area and the back door. 2. Light shield in dry storage is missing a shield 3. Air vents in dry storage.
  • Courtyard Marriott Stafford Quantico, 375 Corporate Drive Stafford, VA 22554
    • Date inspected: Nov. 15
    • Total violations: 5
      • Observed can opener soiled. Item removed to 3-comp sink for cleaning.
      • Observed the following TCS foods held at improper temperature in the prep unit located in the bar service area at 9:30 am: shredded mozzarella cheese (48 degrees F), salsa (47 degrees F), cream cheese (45 degrees F), greens salad (48 degrees F), and egg sandwich (48 degrees F). Items were placed in unit at 6:30 am and were being ready to be taken back to the walk in cooler. Items have one hour to be brought down to 41 degrees F. The temperature of the unit read 39 degrees F. The TCS items were double panned in stainless steel containers. Recommended to use one single stainless steel pan when storing food in unit.
      • Observed two wiping cloth buckets in the bar service area with a sanitizer concentration of 0ppm. Sanitizer in wiping cloth buckets were replaced.
      • Observed the following in need of cleaning: 1) fryers. 2) oven (inside kitchen). 3) oven (bar service area).
      • Observed missing ceiling tile in front of the expo line. PIC stated HVAC system is being replaced and left without putting ceiling tile back.
  • Papa John's Pizza, 411 Chatham Heights Rd Suite 111 Fredericksburg, VA 22405
    • Date inspected: Nov. 15
    • Total violations: 7
      • Per discussion with the person in charge (PIC) the food contact surfaces for equipment and utensils used for time/temperature control for safety foods (TCS) are being washed every once a day. EHS discussed the cleaning frequency of utensils and equipment used for TCS foods with PIC.
      • Observed flying insects throughout the facility.
      • Observed the back door of the food establishment does not have a tight seal, observed light coming from the bottom of the door.
      • Observed the 3 comp sink not sealed to the adjacent wall.
      • The floors, walls, and ceiling are in need of cleaning throughout the facility.
      • Observed the following in poor repair: Cracked coving near the ware washing hand sink, cracked floor tiles by the back door and the 3 compartment sink, missing coving by handwashing sink and hand washing sink under the 3 compartment sink, and missing ceiling tile and vent cover in the employee toilet room.
      • Observed employees jackets over pizza boxes and pizza sauce near the employees' restrooms and the warewashing area.
  • Livingston Elementary School, 6057 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Nov. 15
    • Total violations: 2
      • Observed the following equipment in poor repair : 1. The clothing dryer inside the kitchen is heavily rusted on top. Through discussion with the person in charge the kitchen staff does not use the dryer only the maintenance department. 2. Hood vent filters have gaps
      • Observed the following physical facilities in disrepair: 1. Tiles under the three compartment sink are lose and rusted 2. Ceiling tiles throughout the facility have gaps 3. Air vent by the prep table has a hole.
  • Country Inn & Suites, Dahlgren, 4755 James Madison Parkway King George, VA 22485
    • Date inspected: Nov. 15
    • Total violations: 2
      • Observed scrambled eggs (125'F) and link turkey sausage (122'F) hot holding at incorrect temperatures on the customer service line. Food had been on the line for over an hour. Person in charge voluntarily discarded the food and replaced it with items at the correct temperatures.
      • There is no covered receptacle for sanitary napkins in the toilet room used by females.
  • Papa John's Pizza, 411 Chatham Heights Rd Suite 111 Fredericksburg, VA 22405
    • Date inspected: Nov. 15
    • Total violations: 7

    • Date inspected: Nov. 16
    • Total violations: 1
      • Facility was unable to provide a certified food protection manager. Person in charge ServSafe certificate expired 8/20/23.
  • Bob Evans Restaurant #537, 10011 Southpoint Parkway Fredericksburg, VA 22407
    • Date inspected: Nov. 16
    • Total violations: 7
      • Observed two employees lathering hands under running water while washing their hands. EHS discussed with employees handwashing procedures. Employees rewashed their hands during the inspection.
      • Observed the interior of microwave soiled to site on the cook line and prep line.
      • Observed three plates on the clean dish rack soiled on the food contact surface. The person in charge removed the plates to be cleaned.
      • Observed food employees not wearing hair restraints while handling foods.
      • Observed the following equipment in poor repair : 1.Shelving rusted holding single service containers 2.Toaster on top is chipped and peeling.
      • Observed the following non-food contact surfaces in need of cleaning: 1. Can rack 2. Tray holder in server section 3. Interior of biscuit warmers 4. Interior of juice machine 5. Interior of all ovens 6. All shelving throughout the facility 7. Exterior of the grill on cook line.
      • Observed hood vents on the cook line are in need of cleaning.
  • Target, 1090 Stafford Market Place Stafford, VA 22554
    • Date inspected: Nov. 16
    • Total violations: 2
      • Per discussion with the person in charge, the steamer wand and blenders only gets cleaned and sanitized once daily. Both need to be cleaned and sanitized every four hours since they come in contact with TCS foods.
      • No valid sanitizer testing strips. Expired on September of 2023.
  • Bearded Monkey, 564 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: Nov. 16
    • Total violations: 1
      • Backdoor of the food establishment is not self-closing or tight sealing and requires weather stripping along the bottom. Seal conduit cables through the ceiling to prevent entry of pests.
  • SH Coffee Station, 101 Hospital Center Blvd. Stafford, VA 22554
    • Date inspected: Nov. 17
    • Total violations: 2
      • No food thermometer available to measure the temperature of time/temperature control for safety foods. PIC obtained a thin probe thermometer from the office.
      • No quaternary Ammonium test kit available to ensure that concentration for the sanitizer solution used to sanitize food contact surfaces meets the Virginia Department of Health Food Regulations. EHS provided some strips. PIC will order a kit today.
  • My Casa Restaurant LLC, 2018 College Avenue Fredericksburg, VA 22401
    • Date inspected: Nov. 20
    • Total violations: 3
      • Observed the interior of the microwave requires cleaning.
      • Observed a gallon of milk in the walk in cooler with an expiration date of 11/15. During the inspection person in charge discarded the milk.
      • Observed the ice scoop handle making direct contact with the ice in the front service soda machine.
  • Outback Steakhouse, 308 Worth Avenue #101 Stafford, VA 22556
    • Date inspected: Nov. 20
    • Total violations: 9
      • Observed several plastic containers on shelving in the ware wash area with debris on them after being cleaned and sanitized. Items were relocated to dishmachine for cleaning.
      • Observed the interior ceiling of ice machine soiled.
      • Observed the following TCS foods held at improper temperature in a pull out drawer cooling unit on the cookline at 11:15am: 1) sliced cheddar cheese: 60 degrees F. 2) sliced swiss cheese: 59 degrees F. 3) sliced provolone cheese 60 degrees F. Items were placed in unit at around 6:00-7:00am. Temperature of unit read 56 degrees F. All items were discarded. The power cord of the unit was unplugged. After being plugged back in, the temperature of unit lowered to 41 degrees F.
      • Observed packaged lobster thawing in sitting water with ice cubes in a prep sink. The temperature of water read 32 degrees F. The water was drained and lobster was placed under running water reading 70 degrees F.
      • Observed several plastic containers wet while stacked on a shelve in the warewash area. Items were relocated to dishmachine for cleaning.
      • Observed ice build up on the vent system and boxed frozen foods in the walk in freezer. Items were relocated.
      • Observed butter oil used on grills to cook food stored in a two chemical spray bottles. The spray bottles are not food grade safe. Bottles were discarded during inspection.
      • PIC could not provide test strips for chemical sanitizer.
      • The following are in need of cleaning throughout the kitchen: 1) floors underneath equipment. 2) ceiling tiles. 3) ceiling vents. 4) floor drains.
  • Hunan Garden, 923 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Nov. 20
    • Total violations: 19
      • The person in charge was unable to provide proof of training on the employee health policy but was able to give the names of some common foodborne illnesses and symptoms. A sample copy was provided to the person in charge for all employees to receive training and sign.
      • Person in charge was unable to provide written instructions on the clean-up of vomiting and diarrheal events. Written procedures handout was provided to the person in charge.
      • There were no paper towels at the hand washing sink in the kitchen. Person in charge said that they had ran out of paper towels earlier in the day.
      • The one handwashing sink in the kitchen was blocked by two large bowls of food sitting on top of stools. The stools with food were moved away from the handwashing sink.
      • Observed a can of bamboo shoots and a can of tomato soup in the dry storage area that were dented. A food employee removed the cans.
      • Observed the following: 1. a large stainless-steel container of meat in the freezer uncovered, with freezer burn; 2. raw shell eggs in the walk-in cooler next to a container of garlic and red sauce; 3. raw shrimp stored in the prep cooler next to cooked noodles; 4. the same tongs were being used for raw and ready-to-eat items in the prep cooler; 5. a soiled red cloth covering a large bowl of rice. The raw items were relocated, the freezer burned meat and rice touching the soiled cloth, were voluntarily discarded by a food employee.
      • Observed the following cold holding in the prep cooler out of temperature: 1. noodles at 43 degrees F; 2. cooked shredded chicken at 43 degrees F; and 3. raw chicken at 43 degrees F. Person in charge advised all items had been in the unit since 11:00am, it was 4:30pm, so they were voluntarily discarded. Person in charge lower temperature and if it doesn't hold they will call for repair, and put items on ice in the unit to maintain proper temperature.
      • After discussing pest control with the person in charge a package, not in English, was shown. A food employee advised it was a pesticide powder that was purchased from a foreign items shop. The same employee advised they had used it before, by adding water and spraying after closing, but that they no longer used it. Person in charge was advised to remove chemical from the premises.
      • Person in charge was unable to provide a thin probe thermometer.
      • Observed the following: 1. the kitchen back door and basement front door both have a large gaps between the door jamb and the door itself; 2. hole in the wall near the 3-compartment sink; 3. gaps around the conduits in the ceiling throughout the kitchen and in the dry storage area; 4. gap in the wall right above the 3-compartment sink; 5. ceiling tiles not seated properly, bowed from water damage, and with holes in them. Person in charge was advised to close up all holes and fix ceiling tiles.
      • Observed live flying insects and both live and dead crawling insects in the kitchen area. Person in charge was advised to call pest control immediately to be serviced within the next 48 hours.
      • Observed the following foods being held in cardboard boxes or on sheet trays lined with cardboard, from boxes that other foods were shipped in: 1. three box lids, one large box, and one sheet tray of fried chicken pieces; 2. two pans of eggplant; 3. one small box of shrimp rolls, some were also inside of plastic grocery bags; 4. one small box of pork rolls; and 5. one small box of beef rolls. Also, observed a rice being stored in a large plastic bin that had a large crack in the side, that was laced together with a thick string. All food was discarded.
      • Observed the 3-compartment sink and handwashing sink with damaged and missing caulking.
      • Person in charge was unable to provide a quaternary ammonium sanitizer test kit.
      • Observed the non-food contact surfaces of equipment throughout the facility heavily encrusted in grease and food debris.
      • Observed ceiling tiles throughout the kitchen, restrooms, and bar area are made of absorbent material. Also, there was unfinished wood in multiple locations in the kitchen.
      • Observed multiple cracked and missing floor tiles throughout the kitchen and dry storage area.
      • Observed the walls and floors throughout the facility heavily soiled.
      • Observed one piece of equipment in the kitchen next to the flat top not being used, and two large pieces of equipment in the bar area not being used. The equipment is accumulating dirt and food debris. The person in charge was advised to remove all unnecessary items.
