Fredericksburg Health Inspections: McDonald's, Denny's, Tila's Cocina on Board

The Rappahannock Area Health District recorded violations at several Fredericksburg area restaurants during recent inspections.

FREDERICKSBURG, VA — Inspectors recorded violations at several Fredericksburg restaurants during inspections from July 6 to July 12.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Pizza Hut Powhatan, 1224 Powhatan Street Fredericksburg, VA 22401
    • Date inspected: July 6
    • Total violations: 7
      • Person in charge was unable to provide an employee health policy. EHS provided a copy. Food employees read and signed.
      • Observed a food employee scrub soapy hands under running water for less than 3 seconds. EHS instructed food employee to properly rewash thier hands.
      • Observed a small pizza pan in the back prep room stored soiled, food debris was visible. Pizza pan was moved to the warewashing area.
      • Per discussion with the person in charge the utensils at the salad bar currently get switched out twice a day during shift change (every 5-6 hours). EHS advised person in charge to switch out utensils at least every 4 hours.
      • The following non-food contact surfaces were observed in need of cleaning: 1. All of the metal wire racks holding equipment, dried foods, racks of walk in cooler and walk in freezer. 2. The upsplash of the soda machine. 3.The gaskets of the reach in coolers and freezers were heavily soiled with black growth. 4. The interior surfaces and racks of reach in coolers and freezers. 5. Handles of equipment throughout kitchen.
      • The following were observed in need of cleaning: 1. Floor drains 2. Floors underneath equipment and floors throughout the kitchen 3. Walls around the dishroom/dishmachine 4. Walls behind equipment 5. Floors of the walk n freezer
      • Observed the coving is missing along the wall of the back of the office and in the entrance of the mop/chemical storage area.
  • Starbucks Coffee, 1699 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: July 6
    • Total violations: 7
      • Observed the handsink by the ice machine had no soap available. During the inspection the person in charge provided soap.
      • Observed no paper towels were available at the handsink by the ice machine. During the inspection the person in charge provided paper towels.
      • Observed the interior part that makes direct contact with the ice machine was soiled with black and pink overgrowth.
      • Observed the concentration of a wiping cloth bucket was 0ppm. Person in charge changed out the sanitizer in the bucket (200ppm).
      • Observed two containers of half and half cold holding in a cooling unit at improper temperatures (47-48 degrees F). Person in charge could not say how long the half and half had been in the unit for and discarded the items. EHS advised to relocate other TCS items that were put in the unit less than 1 hour from the time of inspection.
      • Observed the upsplash of the coffee brewers are in need of cleaning.
      • Observed the following in need of cleaning: 1. Floor drains 2. AC and ceiling vents of the kitchen area, front service areas and bathrooms. 3. Floors underneath equipment. 4. Walls behind equipment.
  • Tila's Cocina on Board (UUF-8211 VA), 7638 Courthouse Road Spotsylvania, VA 22551
    • Date inspected: July 6
    • Total violations: 20
      • The person in charge could not provide documentation of employee health training but was knowledgeable of symptoms requiring reporting. Provided sample employee health policy in Spanish.
      • Observed dishwashing detergent, handsoap and a pair of tongs in the only handwashing sink on the mobile unit. The items were removed from the handsink during the inspection.
      • Observed mold like watery growth inside a bag of cut tomatoes which were held inside the prep cooler. The person in charge voluntarily discarded the tomatoes for safety.
      • Observed a carton of shell eggs stored over uncut produce in the small reach in cooler. The eggs were relocated during the inspection.
      • Observed utensils and food contact equipment throughout the mobile unit in need of cleaning. Instructed the person in charge to clean and sanitized equipment prior to use.
      • Inadequate cooling of prepared rice observed inside the prep cooler. Observed two plastic bags of cooked rice (51 degrees F) which the person in charge reported were prepared two days prior. The person in charge reported temperatures are not monitored during the cooling process. The person in charge voluntarily discarded the rice for safety. The mobile unit is not approved to cool rice at this time due to limited space.
      • Observed container of prepared coleslaw (46 degrees F) and sliced cheese (46 degrees F) cold holding at improper temperatures inside the prep cooler. The person in charge reported the coleslaw was prepared and held in the unit overnight and the cheese has been in the unit since the day prior. The person in charge reported the mobile unit is connected to a power source at their residence when not operating. The person in charge voluntarily discarded the foods for safety. Instructed the person in charge to discontinue use to cold hold TCS foods and contact a technician for repair.
      • Observed shell eggs (48 degrees F) cold holding at improper temperatures inside the prep cooler. The person in charge voluntarily discarded the eggs for safety.
      • Observed absence of consumer advisory for eggs which may cooked to order upon a customer's request. Provided consumer advisory regulation during the inspection.
      • Through discussion with the person in charge, the water used to fill the freshwater tank was obtained from the owner's residence which is on a private well. Informed the person in charge mobile units are not permitted to fill freshwater tanks with water from their private residence. The person in charge provided a service area agreement for obtaining and disposal of wastewater from an approved facility. Instructed the person in charge to flush and disinfect the fresh and wastewater tanks and plumbing lines.
      • Improper cooling methods are being conducted on the mobile unit to cool cooked rice. Rice is cooked and held for an unknown period of time prior to completing cooling in sealed and densely packed plastic bags inside the walk in cooler. Cooling of the rice is not authorized on the mobile unit at this time. Instructed the person in charge to discontinue this process and submit procedures for approval.
      • Observed absence of ambient air thermometers inside the prep cooler and deep freezer.
      • The person in charge could not provide a thin-probed food thermometer.
      • Observed two white cutting boards which are scratched and scored. Discussed replacing or resurfacing to ensure the food contact surfaces are smooth and easily cleanable.
      • Observed the following equipment in poor repair: Numerous broken and scorched wooden spoons which are no longer sealed, two cutting boards which the plastic non-slip sides are peeling and cracked and the top of the freezer has become rusted. Instructed the person in charge to discontinue use of the wooden spoons.
      • The person in charge provided a Chlorine test kit which expired October 2021. Instructed the person in charge to purchase a new test kit.
      • Observed accumulation of debris on food contact surfaces throughout the mobile unit, including inside refrigeration, shelving and bins used to hold single use articles and equipment. Recommend deep cleaning of the mobile unit.
      • The person in charge could not provide a backflow preventer to prevent backflow/backsiphonage of water when filling the fresh water tank.
      • The person in charge could not provide a food grade hose for inspection to verify it is composed of food grade/safe materials.
      • The floors and walls throughout the mobile unit are in need of cleaning.
  • Quality Inn, 543 Warrenton Rd Fredericksburg, VA 22406
    • Date inspected: July 7
    • Total violations: 3
      • Observed the following items cold holding overnight at improper temperatures in the homestyle refrigerator (46'F VDH thermometer) near the two compartment sink: 1) a carton of low fat milk (45'F), 2) a carton of whole milk (46'F), 3) six containers of fruit yogurt (43'F-47'F) Items were discarded and thermostat was lowered in the unit- EHS checked unit after thirty minutes (41'F VDH thermometer).
      • Observed one of the two cooling units used to store TCS without an internal thermometers.
      • Observed three plastic containers of single service articles stored directly on the floor by the homestyle refrigerator. PIC relocated the items.
  • Holiday Inn Express, 10235 Patriot Hwy Fredericksburg, VA 22407
    • Date inspected: July 7
    • Total violations: 7
      • Observed personal cell phone stored on prep table in kitchen area of food facility.
      • Observed food employee in kitchen area of food facility not wearing a proper hair restraint such as a hat or hair net as required.
      • Observed box of coffee stored on floor in kitchen area of food facility.
      • Hand soap was not readily available at handwashing sink in kitchen area of food facility. Soap was provided at handwashing sink.
      • Paper towels were not readily available at handwashing sink in kitchen area of food facility. Person in Charge relocated paper towels to handwashing sink.
      • Observed food employee eating at prep table in kitchen area of food facility. The food employee was informed on ensuring eating is done in a designated area.
      • Facility does not have an available signed Employee Health Policy. Employee Health Policy was provided to the Person in Charge and was reviewed and signed by available employees.
  • Subway (Bowling Green), 316 W Broaddus Ave Bowling Green, VA 22427
    • Date inspected: July 7
    • Total violations: 3
      • The rear outer door of the establishment is not self-closing.
      • A backflow prevention device is not provided on the freshwater supply hose for the ice tea maker.
      • The men's and women's restroom doors are not provided with self-closing doors.
  • Dunkin Donuts & Baskin Robins, 305 Garrisonville Road Suite #102 Stafford, VA 22554
    • Date inspected: July 7
    • Total violations: 4
      • No covered receptacle in the female toilet room.
      • Observed a hose on the floor that was attached to a U connector at the mop area with an air gap that was less than 1 inch above the flood level rim. During the inspection, the PIC created the required air gap between the hose and flood level rim.
      • Per discussion with the person in charge the blenders and the spoons used for dairy items dairy are not being washed every four hours or when contamination occurs as per the requirements of the VA Food Regulations. The items sometimes get washed every six hours at the end of the shift. EHS explained to the person in charge cleaning frequency of utensils and equipment used with time/temperature control for safety (TCS) foods.
      • Improper handwashing procedures observed. Observed a food employee wash their hands for less than 10 seconds. Discussed proper hand washing procedures with PIC.
  • Le Grand Cru Catering LLC, 275 Decatur Road Stafford, VA 22554
    • Date inspected: July 7
    • Total violations: 2
      • Observed a food employee pour contents of the wiping cloth bucket down the front handsink. Instructed the person in charge handsinks are to used for handwashing tasks only.
      • Observed two first aid kits stored on top of the microwave oven. The first aid kits were relocated to a bottom shelf to prevent contamination.
  • South Vietnam House, 1623 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: July 7
    • Total violations: 4
      • Observed the blade of the can opener was observed soiled. Person in charge moved the can opener to the 3 compartment sink.
      • Observed the interior lip of the ice machine was soiled.
      • Observed the microwave by the cook line was observed soiled.
      • Observed a pot of white rice hot holding at 100 degrees F. Per discussion with the person in charge the rice was left in the cooker overnight. Person in charge discarded the items for safety.
  • Mama's Pizza & Subs, 5028 Plank Road Fredericksburg, VA 22407
    • Date inspected: July 7
    • Total violations: 1
      • Observed tuna salad and tzatziki sauce in prep unit with incorrect date marking. Observed spaghetti and cheese in 2-door refrigerator not date marked as required. Person in Charge dated food properly.
  • El Pino Mexican Restaurant, 4211 Plank Road Fredericksburg, VA 22407
    • Date inspected: July 7
    • Total violations: 10
      • Observed food employee washing hands at prep sink rather than handwashing sink as required. Food employee was instructed and proceeded to wash hands at handwashing sink.
      • Observed soap and paper towels provided at prep sink. Person in Charge removed soap to prevent handwashing at prep sink.
      • Facility has a consumer advisory on the menu stating that shrimp may be undercooked per customer request. The person in charge could not provide parasite destruction for shrimp. The person in charge was instructed to discontinue offering shrimp undercooked unless parasite destruction can be ascertained. The person in charge agreed to comply.
      • Observed residue on non-TCS food-contact surface of margarita dispenser.
      • Facility uses time control for cheese, tomatoes, & sour cream, but facility does not have written procedures in place. Person in Charge was provided with Time as a Public Health Control documentation that was filled out during inspection
      • Observed medicine in cabinet in wait station area stored next to spices and sauces. Person in Charge relocated medicine to prevent contamination.
      • Observed spray bottle in warewashing area of food facility without label indicating common name of chemical.
      • Observed hydrogen peroxide containers in cabinet in wait station area stored next to spices and sauces. Person in Charge relocated items to prevent contamination.
      • Observed holes in walls throughout kitchen area of food facility.
      • Observed personal food stored over salsa in 1-door refrigerator. Food employee relocated food items to prevent contamination.
  • Four Seasons Family Restaurant, 5201 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: July 8
    • Total violations: 13
      • Person in charge did not have a valid and updated employee health policy. EHS provided a copy, person in charge discussed the foodborne illness policy with employees.
      • Observed a food employee touch toast with bare hand. EHS discussed bare hand contact and glove usage with food employees.
      • Observed the handsink by the back door did not have paper towels. During the inspection the person in charge provided paper towels.
      • Observed ice in the handwashing sink of the servers area.
      • Observed the mens bathroom does not have a handwashing sign.
      • Observed the chemical sanitizing dish machine was not sanitizing (0ppm). Person in charge instructed employees to manually sanitize dishes at the 3 compartment sink until a technician comes to service the dish machine.
      • Observed the menu does have a consumer advisory statemement but it does not identify which menu items are made to order and served undercooked.
      • Observed dried foods, flour, rice etc, stored in unlabeled bins. During the inspection the person in charge labeled the bins.
      • Observed a food employee working without a beard restraint.
      • Observed an employee drying dishes with a cloth towel.
      • Observed several boxes of single service use articles (napkins, soup containers, etc), being stored on the floor of the dry storage room.
      • The following non-food contact surfaces were observed in need of cleaning: 1. Inside all cooling units 2. The wire shelves holding clean equipment, dried foods, boxed goods, etc. 3. Handles of equipment throughout kitchen. 4. The upsplash of the coffee brewers at the servers station.
      • Observed the walls of the kitchen and floors underneath equipment and dry storage rooms require cleaning.
  • Popeye's, 10717 Courthouse Road Fredericksburg, VA 22408
    • Date inspected: July 8
    • Total violations: 4
      • Paper towels were not available at back handwashing sink. Person in Charge refilled paper towel dispenser.
      • Observed rice and coleslaw located in the walk-in refrigerator held for greater than 24 hours not date marked as required.
      • Drive-thru window observed left open and subject to the entry of pests. Person in Charge closed window.
      • Walls by trash cans throughout kitchen area of food facility were observed with residue and debris and are in need of cleaning.
  • McDonald's of Stafford #10738, 2862 Jefferson Davis Highway Stafford, VA 22554
    • Date inspected: July 8
    • Total violations: 12
      • Observed a food employee sanitize a surface with a wet wiping cloth with their bare hands and then proceed to place a liner inside a plastic tray used to hold hamburgers. Instructed the food employee to wash their hands. The person in charge removed the liner and try for cleaning and sanitizing. Discussed correct handwashing when changing tasks with the person in charge.
      • Through discussion with the person in charge food contact surfaces of prep tables, which are in contact with TCS foods, are being sanitized within 4 hours in absence of a wash step. Instructed the person in charge food contact surfaces in contact with TCS foods are required to be washed and sanitized. The person in charge agreed to comply.
      • Observed the following TCS foods cold holding at improper temperatures: Yogurt (57 degrees F) inside the smoothie cooler and half and half dairy creamer (49 degrees F) inside the creamer unit. The person in charge reported the TCS foods were held in the units longer than 4 hours and voluntarily discarded for safety. Observed water at the base of the smoothie cooler and considerable ice build up inside the creamer unit. Both units were taken out of service for repair.
      • Observed in use Quaternary Ammonium sanitizing solution in the 3 compartment sink above 500 ppm when tested with a testing kit. The solution temperature exceeded 100 degrees F. The person in charge discarded the sanitizing solution and titrated to 80 degrees F (tested at 73 degrees F) and 200 ppm. Instructed the person in charge to resanitize the food contact equipment.
      • Observed absence of ambient air thermometers inside the walk in freezer, hamburger deep freezer, smoothie cooler and half and half creamer unit.
      • Observed plastic food containers and container of various utensils stored on shelving in the warewashing area stacked while wet after chemical sanitizing.
      • Observed the following equipment in poor repair: Significant ice build-up inside various reach in freezers and the creamer dispenser, observed significant accumulation of water at the base of the smoothie cooler, kick plate is missing under the hamburger deep freezer which is accumulating build up of debris, and the walk in freezer flooring is lifting up.
      • Observed accumulation of debris on the following nonfood contact surfaces: Top of the dishwasher, floors of walk in cooler and freezer, inside reach in refrigeration/freezers, bun toaster curtain, upsplash of the coffee and iced tea brewers, french fry hot holding unit, upsplash of the juice machine and Smoothie machine surfaces. During the inspection the upsplash of the iced tea and coffee brewers were cleaned.
      • Observed a hose connected to a U shaped connector at the mop sink with the hose sitting below the flood rim of the sink. Two hoses connected to chemical dispensers were also attached.to this connection. Absence of adequate backflow prevention was observed during the inspection. During the inspection the person in charge cut the hose to create an air gap exceeding requirements.
      • Observed absence of a covered receptacle inside the women's toilet room.
      • Observed the following in need of cleaning: Floors under equipment and shelving, and caulk along handsinks.
      • Observed accumulation of debris on vent filters above the flattop grill and chicken fryers.
  • Dunkin Donuts / Baskin Robbins, 5231 James Madison Pkwy King George, VA 22485
    • Date inspected: July 8
    • Total violations: 8
      • Person in charge could not provide verification of employee training in regards to Employee Health. Sample Employee Health Policy was left with the person in charge.
      • Observed that paper towels were not provided at the handwashing sink near the fresh donut display case. Paper towels were provided during inspection.
      • The (cavity, door seal) of the microwave oven was observed soiled. Oven was cleaned during inspection.
      • Observed a bottle of mouthwash a table near to go containers. Mouthwash was relocated during inspection.
      • Employees observed working in the food service area without proper hair restraints.
      • Dispensing utensils improperly stored between uses. Observed soiled ice cream scoops stored in the dipper well without running water. Water was turned on during inspection.
      • The following non-food contact items were observed soiled: 1- the base of the table mixer, 2- the top of the hot chocolate machine, 3- the donut stuffer, 4- the shelving under the counter at the ice cream station.
      • Employees are not using the lockers provided for storage of person items. Observed a cell phone, keys, a pocket knife and clothing on top of a prep table with single service items. Person items were relocated during inspection.
  • Bamboo, 10007 Patriot Hwy #123 Fredericksburg, VA 22407
    • Date inspected: July 8
    • Total violations: 2
      • Left handwashing sink in front prep area does not have hot water available. Another handwashing sink is available in the front prep area. Person in Charge will not use handwashing sink until hot water is repaired.
      • Facility uses time-only for rice and tapioca beads and do not have procedures in place for Time Control. Person in Charge was provided with Time Control documentation that was completed and submitted during inspection.
  • Beazley Concession (VA 716-161), 18130 Beazley Lane Milford, VA 22514
    • Date inspected: July 8
    • Total violations: 1
      • No Certified Food Safety Manager.
  • Domino's Pizza #6053, 2606 Richmond Hwy Stafford, VA 22554
    • Date inspected: July 11
    • Total violations: 9
      • The person in charge was unable to provide documentation of training relating to the employee health policy for the major food borne illnesses, online training was provided only on Norovirus. EHS provided an employee health policy sample.
      • Improper handwashing procedures were observed. A food employee washed their hands for less than 10 seconds. Observation was discussed with the person in charge.
      • Observed a food employee use the hand washing (HW) sink as a dump sink- an employee's beverage was poured n the HW sink. Observation was discussed with PIC.
      • Per discussion with the person in charge (PIC) the food contact surfaces for the counters and utensils used with time and temperature control for safety (TCS) foods are not being washed every four hours or when contamination occurs as per the VA Food Regulations. The items get washed at the end of the night shift. EHS explained to PIC cleaning frequency for food contact surfaces for TCS foods.
      • Improper cooling was observed. Condensation was observed on the pizza sauce container (43'F) that was stored overnight at the reach in cooler near the pizza oven. The sauce is made with tomato sauce that requires ambient cooling. EHS instructed PIC to discard the sauce as it didn't reach 41'F within 4 hours of preparing as per the VA Food Regulations.
      • Observed the following: 1) a large container of pizza sauce (60°F), 2) a small container of marinara sauce (59'F) stored on the counter for about 3 hours- PIC could not provide documentation allowing the facility to use time as a public health control (TPHC)- containers were not labeled with a discard time. PIC relocated the sauces to the walk in cooler. 3) a large container of pizza sauce (65'F) stored on a shelf under the counter overnight. Pizza sauce was discarded. 4) six pre-package salad stored for 3 hours in the display cooling unit- salads were made with shredded cheddar cheese (42-43'F) and cut lettuce (42-43'F)- salads were moved to the WIC.
      • Observed the following: 1) a large container of pizza sauce (60°F), 2) a small container of marinara sauce (59'F) stored on the counter for about 3 hours- PIC could not provide documentation allowing the facility to use time as a public health control (TPHC)- containers were not labeled with a discard time. PIC relocated the sauces to the walk in cooler. 3) a large container of pizza sauce (65'F) stored on a shelf under the counter overnight. Pizza sauce was discarded. 4) six pre-package salad stored for 3 hours in the display cooling unit- salads were made with shredded cheddar cheese (42-43'F) and cut lettuce (42-43'F)- salads were moved to the WIC.
      • Observed the pizza prep cooling units and the salad reach in cooling unit without temperature measuring devices.
      • Observed a leak at the 3 compartment sink and the handwashing sink faucet in the back of the house.
  • Camellia Colony of Chancellor's Village, 12100 Chancellors Village Ln Fredericksburg, VA 22407
    • Date inspected: July 11
    • Total violations: 5
      • Wait station handwashing sink was not provided with signage to ensure employees properly wash hands.
      • Observed raw ground beef stored over raw intact meat in walk-in refrigerator. Person in Charge relocated food to prevent contamination.
      • High-temperature dish machine observed sanitizing at 145F on temperature measuring device. Facility will utilize 3-compartment sink to sanitize equipment.
      • Observed turkey, ham, and cheese, stored in walk-in refrigerator, held for greater than 24 hours not date marked as required. Person in Charge date marked food properly.
      • Observed spray bottle of cleaning chemicals stored on shelf with food contact equipment in warewash area of food facility. Spray bottle was relocated.
  • Quickway Japanese Hibachi, 5422 Southpoint Plaza Way Fredericksburg, VA 22407
    • Date inspected: July 11
    • Total violations: 13
      • Observed food employee washing hands at 3-compartment sink. Food employee was instructed to wash hands at handwashing sink.
      • Observed food items (flour & sugar) stored uncovered in back prep area. Lids were put on containers to prevent contamination.
      • Chlorine concentration in sanitizer bucket observed at 200ppm at 78F. Person in Charge changed out sanitizer solution and was remeasured at 100ppm.
      • Observed steak (101F) stored on ledge of stove, preventing food from holding at required temperature of 135F or higher. Food was reheated on stove.
      • Observed Time as a Public Health Control used for rice, mixed vegetables, spring rolls, and dumplings, without proper procedures or documentation in place. Person in Charge was provided with Time as a Public Health Control documentation that was signed and provided a copy for file. Person in Charge noted times on food items using time control.
      • Observed medication stored on shelf above prep table. Person in Charge relocated medicine to prevent contamination.
      • Observed food containers in back prep area that were not labeled with the common name of the food contents.
      • Observed approximately 10 fly-like insects in back prep area of food facility.
      • Observed wiping cloths stored out of sanitizer on prep tables in back prep area.
      • Observed rice scoop stored in container of water in front prep area.
      • Observed single-use container lids used on containers in back prep area used to store flour.
      • Observed no backflow prevention device on mop sink hose attachment.
      • Observed personal jacket and personal food stored on shelf with containers of spices in back prep area. Observed personal food stored on prep table in front prep area.
  • McDonald's, 1 Village Center Drive Fredericksburg, VA 22406
    • Date inspected: July 11
    • Total violations: 22
      • Observed absence of active managerial control in the food establishment. During the inspection the person in charge could not communicate how some equipment is being cleaned, monitoring of TCS food temperatures during cold holding was not being conducted, the person in charge could not communicate correct sanitizing concentration for Quaternary Ammonium sanitizer, improper handwashing procedures and glove use, and Quaternary Ammonium sanitizing solutions were not being monitored and found exceeding requirements. Education was provided during the inspection.
      • The person in charge provided signed employee health policies for only two employees. The person in charge was able to communicate symptoms and illnesses requiring reporting. The facility has numerous blank policies to train staff. Instructed the person in charge to ensure employees are adequately trained on symptoms and illnesses requiring restriction or exclusion.
      • Observed an employee use the telephone with their bare hands and then proceed to handle single use articles (take out food packaging ) without washing their hands between tasks. Instructed the food employee to wash their hands.
      • Observed a food employee later their hands under running water at the front handwashing sink. Hands were not effectively washed due to handsoap being flushed down the drain. Discussed proper handwashing and instructed the food employee to rewash their hands.
      • Observed a food employee rinse a plastic food container in the front handsink and observed five to six chemical spray bottles in the warewashing handsink. Informed the person in charge handsinks are to be used for handwashing tasks only. The chemical spray bottles were removed from the handsink during the inspection.
      • Observed the same food employee touch their facemask wearing single use disposable gloves and then proceed to handle single used packaging used to place sandwiches in (take out containers). Instructed the food employee to remove their gloves and wash their hands and discard the single use article. Contamination was prevented
      • Observed food contact surface of the muffin steamer equipment soiled. The equipment was not being used during the inspection.
      • Through discussion with the person in charge, the food thermometer is being sanitized only, in absence of a cleaning step. Discussed cleaning and sanitizing of food thermometer during the inspection.
      • Observed container of various utensils on shelving in the warewashing area which had oily residue. The equipment was relocated to the 3 compartment sink for cleaning.
      • Observed containers of sliced cheese (44, 45 degrees F) cold holding at improper temperatures inside the reach in cooler. The person in charge could not communicate when the cheese was placed in the cooler and removed the cheese to the office to be discarded. Observed bulk milk (43, 45 degrees F) cold holding at improper temperatures inside the McCafe cooler. The person in charge could not communicate how long the milk has been in the unit and voluntarily discarded for safety. Observed water leaking into a cup inside the unit and a considerable amount of water leaking onto the floor under the unit. The person in charge took the equipment out of service until evaluated by a technician.
      • Observed two containers of sliced tomatoes and shredded lettuce, which time in lieu of temperature control is used (4 hours), were being used past the 4 hour time period. The discard time was labeled as 1:16 p.m. for all containers, observed violation at 1:20 p.m. The person in charge voluntarily discarded the tomatoes and lettuce. Ensure TCS foods are discarded within 4 hours.
      • Observed in use Quaternary Ammonium sanitizing solution in the sanitizer basin of the 3 compartment sink at 500 ppm when tested with a testing kit. The solution temperature was 80 degrees F and the solution was cooled to 75 degrees F for testing. The person in charge discarded the sanitizing solution during the inspection and removed the equipment to be washed and sanitized. Discussed manual titration of the sanitizing solutions under the dispenser is serviced by a technician.
      • Observed container of coffee machine cleaning tablets stored on top of the McCafe machine. During the inspection the cleaning tablets were relocated to the office.
      • Absence of ambient air thermometers inside the Smoothie and McCare coolers.
      • Observed presence of flying insects in the kitchen during the inspection. Contact pest control for service and treatment as needed.
      • Observed the following equipment in poor repair: Water at the base of the smoothie cooler, water leaking into a cup inside the McCafe cooler, damage air curtain inside the walk in freezer.
      • Observed accumulation of grime and debris on the following nonfood contact surfaces: Upsplash of coffee brewers and juice machines, pre-wash spray nozzle at the 3 compartment sink, interior surfaces of the convection oven, shelving located to the right of the flattop grill, interior cabinet below the ice cream machine, inside fryers, interior surfaces of the fry freezer, interior of McCafe and smoothie cooler.
      • Observed the following plumbing in poor repair: Leak under the handsink by the french fry freezer, plumbing under the 3 compartment sink is not centered over the drain which is causing water to flow onto the floor when the sinks are drained, leak under the McCafe cooler and low water flow in the men's toilet room handsink. Discussed contacting a plumber to realign the plumbing under the 3 compartment sink to ensure wastewater drains into the floor drain.
      • Observed hose connected to a Y connecter at the mop sink which was below the flood rim of the sink. During the inspection the person in charge cut the hose to create an adequate air gap.
      • Observed both doors of the recyclable dumpster opened
      • Observed significant accumulation of boxes and equipment in the refuse enclosure.
      • Observed the following in need of cleaning: Floors in the warewashing area and behind and underneath the ice cream, Smoothie and McCafe equipment, and ceiling light covers.
  • Fortune King George Chinese Restaurant, 8137 Kings Highway King George, VA 22485
    • Date inspected: July 11
    • Total violations: 2
      • The prepared ready-to-eat (RTE) sweet and sour chicken and pork egg rolls in the walk in cooler are not labeled with a ""consume by"" date. Items were dated during inspection
      • The non-food contact surfaces of the hood filter vents and the interior of the deep fryer cabinets were observed heavily soiled with grease.

      • Observed a food employee wash hands at a food prep sink. EHS explained to the food employee to use the handsink only for handwashing. Employee washed thier hands at the handsink.
      • Observed no paper towels were available at the handsink by the banquet room servers station or in the meat cooler handsink. Paper towels were provided during inspection.
      • Observed the interior of the kitchen ice machine and the ice machine by the banquet room servers station require cleaning.
      • Observed numerous stored sheet trays and pans with a very heavy build of grease build up and grime. EHS advised to the person in charge to replace sheet trays and pans when the build up is no longer easy to clean.
      • Observed the presence of flying insects.
      • Observed the metal wire racks throughout the facility, in dry storage, cooling and freezing storage are rusted.
      • Observed the following non-food contact surfaces in need of cleaning: 1. Metal wire racks holding clean equipment, dried foods, inside cooling and freezing units. 2. Inside and outside surfaces of reach in coolers. 3. Inside the fryer cabinets.
      • Observed the following in need of cleaning throughout the facility 1. Floor drains 2. AC/Ceiling vents 3. Floors behind and underneath equipment 4. Walls behind equipment and in the warewashing area
  • Gourmeltz, 10013 Jefferson Davis Highway Fredericksburg, VA 22407
    • Date inspected: July 12
    • Total violations: 5
      • Observed food employee put beard net on and then proceed to don gloves without washing hands. Person in Charge informed employee to wash hands after touching face and prior to donning gloves.
      • Observed food items (pork 115F, beef 110F, tomato soup 125F) held in hot holding cabinet holding at temperatures below 135F for less than 4 hours. Food employee reheated food to 165F.
      • Observed food employee preparing food while wearing jewelry on wrists.
      • Observed food employees in prep area without proper hair restraints such as hats or hair nets.
      • Observed utensils stored in container of water in front prep area of food facility.
  • Vinny's Italian Grill & Pizzeria, 201 King’s Highway Suite #103-104 Fredericksburg, VA 22405
    • Date inspected: July 12
    • Total violations: 9
      • Observed a food employee place hand soap on their hands and lather their hand under running water at the hand sink located near the convection ovens. Hands were not effectively cleaned due to hand soap being flushed down the drain during hand washing. Observation was discussed with the person in charge (PIC).
      • Observed several trays of raw shell eggs on the shelf in the walk in cooler (WIC) above ready to eat food and vegetables. EHS informed person in charge to relocate the shell eggs to a lower shelf with the raw meats that requires the same cooking temperature.
      • Per discussion with the person in charge the food contact surfaces for equipment and utensils used for time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the VA Food Regulations. The items get washed only once at closing time. EHS explained to the person in charge cleaning frequency of utensils and equipment used with TCS foods.
      • The chlorine sanitizer solution in the dishmachine that was being used to wash dishes measured 0 PPM. PIC changed the chlorine bucket (50PPM).
      • Observed condensation on two large plastic containers of spaghetti (44'F) that were cooked the night prior. Spaghetti was discarded. Proper cooling was discussed with the PIC.
      • Observed the following time/temperature control for safety (TCS) foods stored overnight at improper temperatures in the sandwich prep unit (SPU) and in the pizza prep unit (PPU): in SPU: 1) cooked Spaghetti (45'F), 2) cooked lasagna (45'F)- in PPU: 5) a container of crumbled sausage (47'F), 6) a container of crumbled (bucket of pizza sauce (54'F) per- Items were discarded.
      • Observed a spray bottle of glass cleaner stored near food and utensils. Bottle was relocated.
      • Observed a bag of spinach stored in standing water (69'F) that was taken out of the freezer less than an hour prior- EHS discussed proper thawing methods. Spinach was taken to the WIC to properly thaw.
      • The reach in cooler (45'F) by the shelving units in the back of the house and the pizza prep unit (43'F) in the front of the house need to be evaluated.
  • Mexico Palace, 10001 Patriot Highway Fredericksburg, VA 22407
    • Date inspected: July 12
    • Total violations: 5
      • Observed the dishmachine not operating properly in accordance with the VA Food Regulations. The concentration of the chlorine on the surface of the dishes is 0 PPM. PIC contacted a technician to come immediately to evaluate the unit. EHS instructed PIC to use 3- compartment sink to wash, rinse, and sanitize the dishes until the unit is repaired.
      • Observed a stainless steel container of chicken mixed with tomato sauce (110-127'F) in the hot holding unit (HHU). Per discussion with the employee the chicken was cooked the night prior, cooled properly, placed in the walk in cooler overnight, then it was brought out an hour prior to the inspection and placed in the HHU with the tomato sauce that was opened from a can an hour prior. Employee failed to ensure that the chicken was reheated to 165'F and the temperature the item reached 165'F after all ingredients have been mixed together, prior to placing in the HHU. Item was reheated on the stove to 165'F.
      • Observed the following cold holding overnight at improper temperatures at the cooling unit by the tortilla chips station: 1) cut lettuce (43'F), packaged macaroni and cheese (43'F), whole milk (47'F), a container of chicken nuggets (47'F), a bag of chicken nuggets (47'F), cooked mashed potatoes (44'F). PIC discarded the items and will use ice and time as a public health control method until the unit is repaired. PIC contacted a technician to come in and evaluate the unit.
      • Observed the following in need of repair: 1) water collecting in the prep unit by the tortilla chips area, 2) Reach in cooler in need of repair, 3) cooling drawers in need of repair, 4) can opener is being held in place by wood and duct tape.
      • Observed the following in need of repair: 1) leak in the prep sink, 2) leak in the mop sink, 3) No water access at the hand washing sink near the prep sink.
  • Denny's #9575, 5325 Jefferson Davis Hwy Fredericksburg, VA 22408
    • Date inspected: July 12
    • Total violations: 15
      • Observed food employee washing hands less than 20 seconds.
      • Observed food employee touch cooked cheese burger with bare hand. Person in Charge voluntarily discarded food.
      • Observed food employee with gloved hands touch soiled reach-in door and with same gloved hands touch sliced tomatoes and onions.
      • Observed food employee with gloved hands touch soiled reach-in door and with same gloved hands touch clean plates without washing hands and changing gloves.
      • Observed microwaves are soiled.
      • Observed that the inside of stainless steel bowls and pots are soiled.
      • Observed cheese burger being prepared undercooked (medium well) and upon discussion with Person in Charge and review of menu that burgers are to be prepared fully cooked.
      • Observed the following foods not being kept frozen before being prepared to serve: Waffle Fries, French Fries, Pre-cooked Potatoes
      • Observed multiple stainless steel pots and pans being stacked and stored without being properly air dried.
      • Observed test strips for dishwasher sanitizer are soiled and expired.
      • Observed the following non-food contact surfaces soiled: Reach-in Coolers, Underneath fryers, food prep units.
      • Observed water hose with spray nozzle attached without backflow preventer attached. Person in Charge removed spray nozzle.
      • Observed the following soiled: Floors, walls, ceiling in food prep and ware washing space. Walk-in Freezer floors. Mop sink area. Floor Drains.
      • Observed ceiling tiles with paint peeling and caving in.
      • Observed that the lighting around flat top grill is 12 foot candles.
  • McDonald's Restaurant #3264, 10601 Patriot Highway Fredericksburg, VA 22408

      • Observed an employee beverage stored on a rack with clean equipment. Person in charge discarded the beverage.
      • Observed a food employee filling up a water pitcher to use in the kitchen at one of the handwashing sinks. EHS and person in charge discussed with the food employee the proper usage of handwashing sinks.
      • Observed several food employees wearing wrist jewelry.
      • Observed the caulking of the handsink by the warewashing area and the caulking of the 3 compartment sink is missing.

      • Employees observed working in the food service area without proper hair restraints.
      • The follow in non-food contact surfaces were observed soiled: 1-The cinnamon roll storage bin, 2-the floor fan, 3-the top of the fry hot holding unit.
      • Observed person items, such as cell phones and keys stored on the make line and front server's area. Items were removed during inspection.

      • Observed the person in charge was unable to provide an employee health policy. EHS provided a written copy, person in charge discussed with the food employees and had them sign the policy.
      • Observed a food employee scrub soapy hands for less than 10 seconds. Person in charge instructed food employee to rewash their hands properly.
      • Observed the handwashing sign was missing in the mens bathroom. EHS provided a handwashing sign.
      • Observed no soap was available at the handwashing sink of the mens bathroom. Person in charge provided soap during the inspection.
      • Observed a food employee working without a beard restraint.
      • Observed the racks behind the back kitchen area require cleaning.
      • Observed the walls behind equipment and food prep areas, floors underneath equipment and the ac/ceiling vents throughout the facility require cleaning.

      • Chicken not thawed properly. Observed chicken thawing in a sink of standing water. Chicken was removed from the sink and placed in the walk in cooler.
      • Observed fly strip full of flies hanging over the food slicer in the kitchen. Strip was removed during inspection.
      • Foil pans were observed being washed and re-used in the facility.

      • Observed food employee handle personal cellular phone then changed gloves and was in the process of handling ready to eat food without their hands. Employee was instructed to wash their hands. Observation was discussed with PIC.
      • Observed two bottles of all purpose cleaners stored near single service articles on the shelving unit in the back of the house. PIC relocated bottles.
      • Observed several what appeared to be fruit flies in the back of the house near the manager's desk.

      • Observed employee walk into kitchen with a child.
      • Observed cups stored in handwashing sink. Person in Charge relocated cups to allow access to handwashing sink.
      • Observed chlorine container stored in cabinet next to food equipment. Person in Charge relocated chemical bottle to prevent contamination.
