Fredericksburg Restaurant Inspections: Employee Licked Sauce Off Of Gloves

The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between May 3 and May 9.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • The Soup & Taco 2, 806 William Street Fredericksburg, VA 22401
    • Date inspected: May 3
    • Total violations: 7
      • Observed employee dump a sanitizer bucket down the handwashing sink.
      • Observed two pans of pinto beans at 125 degrees F and one pan of ranchero sauce at 119 degrees F being held in a hot holding steam table. Foods were on the steam table for 30 minutes prior to the inspection. Within 30 minutes a food employee reheated the foods, pinto beans at 170 degrees F and the ranchero sauce at 183 degrees F.
      • Observed two containers of salsa made the day prior cold holding in a reach in cooler at 47-49 degrees F. During the inspection the salsa was discarded.
      • Observed a hot shot pesticide dispenser hanging from a wall opening between the kitchen and bar area. During the inspection the pesticide dispenser was discarded.
      • Observed a bin of utensils near the hot holding area being stored with the food contact surface of the utensils facing up.
      • Observed the following physical facilities in need of cleaning: 1. AC unit above the hold holding station 2. Floors under equipment, especially floors surrounding the grease trap 3. Ceiling tiles throughout the kitchen area.
      • Observed the following in the facility in need of repair: 1.a hole in the wall above the 3 compartment sink 2. a hole in the ceiling in the hallway 3. cracked floor tiles.
  • Olla Cafe and Bar, 2777 Jefferson Davis Hwy #103 Stafford, VA 22554
    • Date inspected: May 3
    • Total violations: 19
      • Absence of active managerial control observed during the inspection. Observed improper handwashing and hand washing procedures, improper glove use and barehand contact with ready-to-eat foods.
      • Observed a food employee place soap on their hands and begin to lather without applying water. Instructed the employee on correct handwashing procedure and the employee washed their hands.
      • Observed a food employee enter the kitchen from the dining room and proceed to handle equipment without washing their hands. Instructed the employee to wash their hands.
      • Observed a food employee handling avocados which were being used to prepare guacamole with their bare hands. Avocados were voluntarily discarded and the employee washed their hands and donned disposable gloves.
      • Observed improper glove use by food employees during the inspection. Observed a food employee preparing a sauce wearing disposable gloves, lick the sauce on their glove for tasting and then proceed to use the same glove to handle the container being used to prepare the sauce. During the inspection the employee was instructed to remove gloves and wash their hands. Also observed a food employee rinse their hands wearing disposable gloves in the kitchen hand sink. Instructed the food employee to remove gloves and wash their hands. Discussed correct glove use and provided handout.
      • Observed three pitchers of water uncovered at the bar area near guest seating. Plastic wrap was placed on the opening of the pitchers during the inspection.
      • Observed accumulation of encrusted debris on the food contact surfaces of the charbroiler. The charbroiler was not in use upon arrival.
      • Observed the ice chute of the ice machine soiled.
      • Observed the following TCS foods cold holding at improper temperatures: 1- 3 door reach in cooler: cheese and pepper mix for pupusas (43 degrees F) and black beans (44). The foods have been in the unit overnight and were voluntarily discarded for safety. Observed the unit overstocked. Discussed food storage in the unit with the operator. 2- Coleslaw (43 degrees F) stored in a stainless steel container on ice. Observed ice only contacting the bottom of the stainless-steel container. The coleslaw was pulled from refrigeration at 11:00 a.m. More ice was added to the ice bath during the inspection. Cool to 41 degrees F, use or discard by 3:00 p.m.
      • Update the consumer advisory to place the asterisk next to the steak, which may be cooked to order for Olla Parriada. Currently the asterisk is placed next to a description of the menu item which also includes other animal derived foods which are fully cooked.
      • Observed employees preparing beverages and food without proper hair restraints.
      • Observed ice cream scoops stored in a container of water at 76 degrees F. Recommended storing utensils in a clean and dry container and removing for cleaning and sanitizing every four hours.
      • Observed numerous ladles suspended from the shelving unit with the food contact surface less than one foot from the prep table. Discussed relocating the utensils with the operator.
      • Observed the following equipment stored while wet after sanitizing: 1- stack of stainless-steel containers and 2- Blender at the bar. The equipment was removed to the 3 vat sink for cleaning.
      • The Quaternary Ammonium testing kits provided by the operator expired in 2014 and 2021. Instructed the operator to purchase new test kit.
      • Absence of alarm or audio or visual means to inform staff of low sanitizer or detergent levels for the dishwasher.
      • Observed both lids for the dumpster are cracked and broken. The dumpster does not have a lid which can be adequately closed.
      • Observed numerous cardboard boxes stored on the ground in the dumpster area.
      • Observed employee personal articles stored on top of blender and shelving over cleaned equipment (eyeglasses, cell phone and speaker). Personal articles were relocated during the inspection.
  • Holiday Inn Express & Suites, 16348 Gateway Lane King George, VA 22485
    • Date inspected: May 4
    • Total violations: 1
      • Consumer self-dispensing bulk food bin on the buffet line containing cold cereal not properly labeled.
  • Eileen's Bakery & Cafe, 1115 Caroline St Fredericksburg, VA 22401
    • Date inspected: May 4
    • Total violations: 8
      • No certified food protection manager available.
      • Nontyphoidal Salmonella is not included in the employee health policy used by the facility. EHS provided an updated employee health policy for the employees to read and sign.
      • Person in charge stated that time is being used as a public health control for the breakfast sandwiches, however, could not provide documents and the items were not labeled.
      • Observed in-use utensils stored in water at a temperature of 69'F by the sandwich prep area. Person in charge provided dry knives and will discontinue storing them in standing water below 135'F to comply with the VA Food Regulations.
      • Observed several stainless steel containers and bowls stacked on the shelving unit while wet after cleaning and sanitizing. PIC instructed an employee to dry them using disposable paper towels.
      • Observed two cold holding displaying unit is not maintaining the required temperature of 41'F or below. First unit (43'F VDH thermometer) was used to store breakfast sandwiches that were being monitored by time as public health control- the sandwiches contained eggs and meat. The second unit (47'F VDH thermometer) was used to store pre-portioned pasta dinner to be sold cold and heated later by the consumer. The person in charge (PIC) lowered the thermostat on both units. The stove was being used without the hood filters being sealed in place properly. PIC placed the filters in place during the inspection.
      • Observed the following in need of repair: 1) ceiling tiles, 2) floor tiles, 3) broken light shield.
      • Observed the following employees' personal belongings stored near food and equipment. 1) tools stored over the reach in freezer door (corrected) 2) personal bag and keys stored over single service articles on the shelving unit (corrected) 3) a cell phone stored over equipment (corrected).
  • Cedar Forest Elementary School, 3412 Massaponax Church Road Fredericksburg, VA 22408
    • Date inspected: May 4
    • Total violations: 2
      • Observed in one of the kitchen hand sinks basin a container of milk and in one the ware washing hand sinks basin a pair of dish washing gloves. During the inspection the container of milk was discarded and the gloves were relocated.
      • Observed the interior of the ice machine requires cleaning.
  • Rodney E. Thompson Middle School, 75 Walpole Street Stafford, VA 22554

      • Observed a food employee lather their hands for less than 10 seconds. Instructed the employee on handwashing procedures and the employee washed their hands.
  • Subway, 15 South Gateway Drive Suite #117 Fredericksburg, VA 22406

      • Food establishment did not have a designated person in charge.
      • Improper hand washing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
      • Observed the following items not cold holding at the proper temperatures in prep sandwich cooler was out of temperature for 2 hours . 1. Tomatoes 52'F 2. lettuce 58'F 3. Sliced beef 46'F 4. Chicken 52'F Person in charge placed items in walk in cooler.
      • Observed two sandwich prep coolers on the front line not maintaining proper temperatures. Vegetable cooler is at 67'f and the meat cooler is at 53'f. The following items were out of temperature 1. Sliced tomatoes 52'f 2. Lettuce 58'f 3. Tuna salad 47'f 4. Beef 46'f 5.Chicken 52'f
      • Observed a personal cellular phone, keys, a purse, a covered bottle stored near equipment and food.
  • Snack Shack, 8901 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: May 5
    • Total violations: 1
      • No Certified Food Protection Manager (CFPM) available for facility. Provide copy of certificate once obtained.
  • Daybreak Coffee (UB43693), 1 Autumn Drive Stafford, VA 22556
    • Date inspected: May 7

      • Observed wet wiping cloth sitting on the counter. The person in charge placed the cloth in sanitizer.
  • Royal Farms #237, 1221 Warrenton Road Fredericksburg, VA 22406

      • The employee health policy for the facility does not cover non-typhoidal salmonella. EHS provided an updated employee health policy to be read and signed by employees.
  • Quickway Hibachi, 28 South Gateway Drive #107 Fredericksburg, VA 22406
    • Date inspected: May 8
    • Total violations: 17
      • The person in charge could not provide written procedures for clean-up of vomiting and diarrheal events. Sample procedures were provided during routine inspection on 5/6/2022 and during today's inspection. Written procedures were provided in English, Spanish and Chinese.
      • Observed two employees enter the front cook line from the dining room and apply hand sanitizer without washing their hands prior to proceeding in food preparation tasks. Also observed a food employee set up a wiping cloth bucket with Chlorine sanitizer and proceed to relocate items into the walk-in cooler without washing their hands. Instructed food employees to wash their hands and discussed the use of hand sanitizers with the person in charge.
      • Observed a food employee lather their hands under running water in the back kitchen hand sink. Hands were not effectively washed due to hand soap being flushed down the drain. Instructed the employee on correct handwashing and the employee washed their hands.
      • Observed food contact surfaces of the meat and vegetable slicer soiled to the sight. Clean equipment prior to next use.
      • Observed build-up of debris on the plastic lip inside the ice machine.
      • Observed top layer of fried rice in a rice warmer between 130- 134 degrees F. The person in charge reported the fried rice was prepared and placed in the unit at 10:00 a.m. Observed the rice warmer filled to the top with fried rice which may be limiting the hot holding ability of the equipment. The top layer of rice was removed to the flattop to reheat at 165 degrees F or above. Observed the fried rice reheated to 184 degrees F by 11:22 a.m.
      • The person in charge could not provide documentation of time as a public control used for cut cabbage and sushi rice. Signed documentation was provided on 5/29/2019. Signed documentation was emailed to the operator during the inspection. Ensure signed documentation is available for review during inspections. Sushi rice and cut cabbage were adequately marked with the 4 hour discard time.
      • Observed one chemical spray bottle unlabeled. The person in charge reported the spray bottle contained straight bleach. The spray bottle was labeled during the inspection.
      • Observed raw tuna thawing inside the walk in cooler with the reduced oxygen packaging intact. The fish was found in a semi-frozen state and the packaging was cut open.
      • Observed the following dispensing utensil improperly stored: 1- Scoop stored with the handle sitting inside the drawer of raw chicken. The scoop was removed for cleaning. and 2- Ice scoop stored with the handle sitting inside the ice. The ice scoop was removed and properly stored on a hook on the side of the equipment.
      • Observed wire cart in the back of the house and shelving in the front cook line and walk in cooler are rusted. Discussed sealing options with the person in charge. Equipment is no longer cleanable.
      • Observed the cutting board on the sushi cooler and block cutting board stored behind the 3 vat sink are scratched and scored. Replace or resurface cutting boards.
      • Observed accumulation of grime and debris on nonfood contact surfaces throughout the facility including interior and exterior of refrigeration equipment, shelving, and hot holding and cooking equipment. Recommend detailed cleaning of the food establishment.
      • Observed accumulation of encrusted debris and oily residue on the exterior and top of the oil receptacle.
      • Observed the oil receptacle lid in the dumpster area is not sealed, the lid does not create a tight seal. Observed considerable accumulation of grease and encrusted debris which may be prohibiting proper closure.
      • Observed the floors, walls, ceiling and floor drains throughout the facility are in need of cleaning, including inside the toilet rooms and caulked surfaces.
      • Observed the following in poor repair: Blown out ceiling lights, cracked floor tiles in the back of the house and rusted stainless steel half wall inside the men's toilet room.
  • America's Best Wings, 315 Garrisonville Road Stafford, VA 22554
    • Date inspected: May 8
    • Total violations: 19
      • Facility does not have a certified food protection manager.
      • The person in charge could not provide written documentation that the employees have been trained in reference to employee health.
      • The person in charge could not provide written documentation concerning the cleaning of bodily fluid accidents.
      • Observed no hand soap at 2 of the hand sinks in the kitchen.
      • Observed no paper towels at two of the hand sinks in the kitchen.
      • Observed that two cans of ketchup are damaged.
      • Observed that the interior of the microwave is soiled.
      • Observed that several knives (corrected) and the inside of several plastic containers are soiled to sight and touch.
      • Observed that the meat slicer and ice chute are soiled to sight and touch.
      • Observed that the in-house made potato salad and cold slaw that was made on Saturday were not dated for disposition.
      • Observed two chemical spray bottles that were not labeled on the bottom shelf of the prep table.
      • Observed two chemical spray bottles that were not labeled on the bottom shelf of the prep table.
      • The following non-food contact surfaces require cleaning: outside of several plastic containers, all of the storage shelves near the prep line and underneath the drink machine, can opener holder, cabinets underneath all of the fryers, tops of the tongs, and the interior of the reach in freezer.
      • Observed that the sprayer hose is sitting below the flood rim on the 3-compartment sink. Observed that there is a sprayer nozzle attached to a hose at the mop sink without a backflow prevention device installed.
      • Observed that the sandwich prep is leaking on the inside.
      • Observed that the following physical facilities require cleaning: all of the light shields, floor drains, and the air vent covers in the walk in cooler.
      • Observed heavily soiled aprons hanging on a pole next to the food in the dry storage area.
      • Observed only 25 foot candles of light in the prep area.
      • Observed that the hood system requires cleaning.
  • Fish N Grill Deli, 10908 Courthouse Road #105 Fredericksburg, VA 22408
    • Date inspected: May 9
    • Total violations: 8
      • Observed no paper towels at hand sink in prep kitchen.
      • Facility is not retaining the lids of the shellstock that is offered on the menu. The shellstock information is on the lids of the oyster and clam containers and the person in charge stated that the lids were being discarded.
      • Observed the following items in prep cooler by fry station out of temperature. 1.Pico de Gallo (39f) 2. Shredded cheese (45f) 3.Slice tomatoes (48f) 4. Crab soup (50f) Person in charge corrected on site items were placed in walkin cooler.
      • Observed and unlabeled bottle of degreaser in the prep kitchen. Person in charge labeled bottle during inspection.
      • Observed utensils with the food contact surface facing upwards on the clean pot rack in prep kitchen.
      • Interior of reach in cooler doors in the main kitchen is not cleanable. Observed a soft insulation inside of doors.
      • Observed two hoses connected to mop sink in prep kitchen without back flow prevention. Hose connected by fry station in main kitchen not equipment with back flow prevention devices. Person in charge corrected on site.
      • Observed hood vents in both kitchen heavy soiled with grime and debris.
  • Katora Coffee, 615 Caroline Street Fredericksburg, VA 22401
    • Date inspected: May 9
    • Total violations: 1
      • The following non food contact surfaces were observed in need of cleaning: 1. top of oven in the back kitchen 2. racks holding clean equipment 3. interior of cooling unit in the back kitchen.
