Fredericksburg Restaurant Inspections: Living, Dead Insects Found
The Rappahannock Area Health District recorded violations at several facilities around Fredericksburg during recent restaurant inspections.
FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between May 31 and June 6.
In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.
The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

- Date inspected: May 31
- Total violations: 4
- This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
- The food storage shelving in the rear of the facility are bare wood.
- The ceiling HVAC vents were observed soiled.
- The string lights in the service area are not shielded, coated, or otherwise shatter-resistant.
- Panera Bread, 24 South Gateway Drive Fredericksburg, VA 22406
- Date inspected: May 31
- Total violations: 6
- Person in charge was unable to provide a verifiable employee health policy. Through discussion with employees they could answer questions regarding foodborne illnesses. EHS provided a sample copy of an employee health policy and suggested the person in charge discuss and have employees sign the policy and provide the verifiable form at each inspection.
- Observed employee eating and drinking in the kitchen.
- Observed 3 plastic cambro food containers with the food contact surface melted no longer making it cleanable.
- Observed the following non food contact surfaces in need of cleaning: 1. shelving throughout the facility 2. baking rolling racks 3. up splash of the coffee brewers.
- Observed multiple sheet trays in the baking area with a heavy build up of grime and debris.
- The following facilities were observed in need of cleaning: 1. ac vent 2. ceiling tiles stained and splashed with food debris 3. walls behind prep areas 4. floors underneath equipment.
- Allman's BBQ, 1299 Emancipation Highway Fredericksburg, VA 22401
- Date inspected: May 31
- Total violations: 5
- The following non food contact surfaces were observed soiled: the wire shelving throughout the kitchen, the metal shelving in the store room.
- The walls and the ceilings throughout the kitchen are stained with years of smoke from the smoker.
- The hoods over the cook's line and the filters in the hoods were observed heavily soiled with grease.
- Ventilation is not sufficient to keep rooms free of excessive vapors. Even with all fans turned on, the smoker area and the cook's line is smoky.
- The light intensity is below 50 foot candles where a food employee is working with food. Measures 8 foot candles near the chopper and 12 foot candles on the cook's line.

- Total violations: 6
- Person in charge was unable to provide written procedures for how the facility takes care of bodily fluid clean ups. EHS provided a sample copy, advised person in charge to adapt the plan according to the facility needs if needed.
- Observed a food employee scrub soapy hands for less than 7 seconds. EHS discussed with the food employee and the person in charge. Employee rewashed their hands.
- Observed both hand sinks in the kitchen did not have paper towels available. During the inspection paper towels were provided.
- Observed the following non food contact surfaces in need of cleaning: 1. wire racks above the ware washing and food prep sink 2. gaskets of reach in coolers/freezers 3. inside surfaces of fry freezer and meat freezer.
- Observed debris around the dumpster area.
- Observed the following facilities that require cleaning: 1. floor drains 2. ac vents 3. floors underneath equipment.
- Kentucky Fried Chicken #0159, 2838 Richmond Hwy, Stafford, VA, USA Stafford, VA 22554
- Date inspected: June 1
- Total violations: 13
- Person in charge could not provide written procedures for clean-up of vomiting or diarrheal events for the facility. Sample cleanup procedures were emailed to operator.
- Observed employee lathering hands under running water, hands are not cleaned properly due to soap being flushed down the drain.
- Observed ice chute at the soda machine in the dining room soiled.
- Through discussion with food employees the following food contact surfaces in contact with TCS foods are not being cleaned and sanitized every four hours: 1.Food thermometer. 2.Chicken breading station equipment. 3. Utensils at the hot holding cabinets.
- Observed one live crawling insect in the kitchen and numerous dead insects in the basement storage room, which a walk-in freezer is also located. Also observed harboraging conditions including damaged ceiling in the basement (evidence of water damage), considerable build-up of debris on nonfood contact surfaces of equipment, and oily residue on walls and floors of cooking equipment.
- Observed presence of numerous dead crawling insects in the basement.
- Observed the following equipment throughout facility in poor repair: 1. Ice buildup in reach-in freezer and upstairs walk-in freezer 2. Rusted dunnage racks inside the chicken walk in cooler.
- Observed mop sink and men's toilet room hand sink are no longer sealed adjacent wall.
- Observed multiple sheet pans on clean pan rack by three compartment sinks with moderate encrusted debris.
- Observed all non-foods contact surfaces throughout facility in need of cleaning.
- Observed the following in poor repair: Multiple ceiling tiles frames rusted, ceiling tiles are not secured to frames, gaps in ceiling tiles which conduit cables are run through the ceiling.
- Observed floors and walls throughout facility in need of cleaning.
- Observed air vents throughout factify in need of cleaning.

- Observed employee adjusting their face mask then proceed to prepare drinks for a customer without washing their hands. Person in charge instructed employee to wash their hands after touching their face mask.
- The walls around the mop sink and behind the 3 compartment sink in the kitchen were observed soiled.

- Date inspected: June 2
- Total violations: 4
- Observed employee rinsing out towels in the front hand sink.
- Observed several sponges near the 3 compartment sink. During discussions with the person in charge, it was discovered that the sponges are being used to clean utensils and other food-contact surfaces. Sponges were discarded during inspection.
- Observed ice cream scoops stored in standing water in the dripper well.
- The following non-food contact surface were observed soiled: 1-the upsplash of the shake mixers, 2-the outer surfaces of the ice machine, 3-the interior of the homestyle reach in cooler and freezer in the kitchen, 4-the outer surfaces of the deep fryer.

- Per discussion with person in charge the milkshake wand is currently only being sanitized . Discussed with person in charge adding wash , rinse and sanitizing method.
- Observed the backflow preventer in the mop sink is a manual and needs to be a automatic.
- Observed walls behind equipment and floors under equipment that need cleaning.
- Commonwealth Senior Living at Fredericksburg, 1001 Northside Drive Fredericksburg, VA 22405
- Date inspected: June 5
- Total violations: 2
- Observed tortellini (43 degrees F) improperly cooled in the 2 door upright cooler overnight. Observed tortelli stored in a deep and covered plastic container with condensation on the sides of the container. The tortellini were prepared on 6/4/23 based on the date marking sticker on the container. During the inspection the product was voluntarily discarded for safety. Discussed methods to facilitate rapid cooling.
- Observed improper cooling methods to cool tortellini pasta. Observed tortellini cooled in a deep and covered plastic container inside the 2 door upright cooler. Discussed methods to achieve rapid cooling.

- Observed deli slicer was observed soiled. through discussion with employee slicer is not being used. Slicer was cleaned and sanitized during inspection.

- Total violations: 17
- Improper hand washing procedures observed. Employee washed hands for less than 5 seconds. Observation was discussed with the person in charge, employee was instructed to re-wash their hands.
- A food employee was observed cleaning their hands at the three compartment sink. Employee was instructed to wash their hands only at the handwashing sink.
- Observed a food employee handle soiled dishes then donned gloves and engaged in food preparation without first washing their hands. Employee was instructed to wash their hands.
- Observed imitation crab being offered on the menu as crab meat.
- Observed an employee wash utensils with soap cooking and store without sanitizing the items.
- Observed several utensils, bowls, and pans to be soiled to the sight and touch.
- Observed two covered large plastic containers of rice in the walk in cooler overnight (45-52'F). Rice was discarded. PIC will use cooling logs.
- Observed rice (114'F) stored overnight in the rice cooker. Rice was discarded.
- A date marking system is not in proper use for the time/temperature control for safety (TCS) foods.
- Observed two covered large plastic containers of rice cold holding overnight in the walk in cooler at improper temperatures (45-53'F). Rice was discarded. EHS discussed proper ,cooling method with the person in charge and provided handouts.
- Observed the following: 1) five packet of completely thawed out tuna stored in the pep unit in the sushi area. The instructions on the package stated that the product has to be removed from the packet prior to thawing. The person in charge discarded the items.
- Observed uncovered pre-portioned bowls of salad stacked on top of each other without a barrier to protect the salad from contamination.
- Preset tableware was observed unprotected from contamination.
- Observed accumulation of debris on the following non contact surfaces: 1) refrigeration units, 2) shelving units, 3) fryer cabinets.
- Observed the following in need of repair: 1) re-caulk 3 compartment sink, 2) coving by the exterior door, 3) re-adjust ceiling tiles, 4) broken/cracked floor tiles.
- Observed the floors, walls, and ceiling throughout the facility are in need of cleaning.
- Observed glasses, three tumblers, a wallet, keys, and a speaker stored near food and equipment in the kitchen. The person in charge relocated the items.
- Taste of Trelawny, LLC, 100 Randolph Rd Fredericksburg, VA 22405
- Date inspected: June 6
- Total violations: 2
- Observed dust and grime on the ceiling tiles above the shelving unit used to store clean equipment, lids, and cooking utensils. PIC will re-locate the items to the warewashing area to be washed and sanitized, and will clean the ceiling tiles.
- No Quaternary Ammonium testing kit available to monitor the sanitizer concentration. EHS instructed person in charge to obtain some strips from the mobile unit until a kit is purchased- (corrected during the inspection).
- McDonald's #16370, 545 Ferry Road Fredericksburg, VA 22405
- Date inspected: June 6
- Total violations: 4
- No paper towel provided at the handwashing near the hot holding stating. Person in charge instructed an employee to provide paper towels (corrected during the inspection).
- Observed raw shell eggs in a carton stored directly near ready to eat eggs in a plastic package in the cold holding unit by the break room and in the walk in the cooler. Items were relocated- (corrected during the inspection).
- Time is being used as a public health control for the sliced American cheese without having the proper labeling to indicate the time that is four hours past the point in time when the food is removed from temperature control. Person in charge discarded the cheese (corrected during the inspection).
- Observed a hose attached to a Y connector at the mop sink with an air gap that was less than 1 inch above the flood level rim. During the inspection, the PIC created the required air gap between the hose and flood level rim. (corrected during the inspection).

- Date inspected: June 6
- Total violations: 5
- Employee was unable to provide documentation of training relating to the employee health policy for this facility.
- Per discussion with the person in charge the food contact surfaces for equipment and utensils (utensils, bowls used for chicken, pizza slicer) used for time/temperature control for safety foods (TCS) are not being washed every four hours or when contamination occurs as per the VA Food Regulations. EHS explained to the person in charge (PIC) cleaning frequency of utensils and equipment with TCS foods. PIC instructed employee to wash and sanitize the items.
- Observed a plastic bag of diced ham (73'F surface and frozen from the middle) stored in hot water (93'F- per discussion with the food employee, the ham was taken out of the freezer about an hour prior to inspection. Discussed proper thawing methods with the person in charge. PIC voluntarily discarded the ham after employee placed it in running soapy water.
- No covered receptacle in the female toilet room.
- Receptacles kept outside the food establishment are over filled and not kept covered with tight-fitting lids or doors.
- Greenfield Assisted Living of Spotsylvania, 9300 Onyx Court Fredericksburg, VA 22407
- Date inspected: June 6
- Total violations: 18
- Observed an uncovered drink near clean tableware and single service items on a table near the entrance of the kitchen. Drink was discarded during inspection.
- The water at the hand washing sink at the restroom that the employees use measured only 68'F.
- The surface of the cutting boards in the kitchen were heavily stained. Discussed the possibility of sanding down the surface of the cutting boards to remove these deep stains.
- The equipment being cleaned in the dish machine were not observed sanitized. There was no measurable sanitizer being dispensed into the machine, due to the fact that the sanitizer supply bucket was empty. A new container of sanitizer arrived during inspection.
- Observed Chicken Salad cold holding at improper temperature (44'F) in the walk in cooler. Salad was voluntarily discarded during inspection.
- Observed spray bottles filled with sanitizer stored near single service items. Bottles were relocated during inspection.
- The certified food manager could not provide a small diameter food temperature measuring device.
- Employees observed working in the food service area without proper hair restraints.
- Single service items observed unprotected from contamination. The silverware on the the pre-set tables were not wrapped or covered.
- The cutting boards in the kitchen were observed heavily scored.
- The quaternary ammonium and chlorine sanitizer test kits are expired.
- The interior surfaces of the dish machine were observed soiled.
- The following non-food contact surfaces were observed soiled: 1-The outer surfaces of the equipment in the kitchen, 2-the wire shelving in the walk in cooler, 3-the dish racks for the dish machine, 4-the stove top, 5-the backsplash of the cook top, 6-the bottom surfaces of the baking sheets and sauce pans, 7-the top of the dish machine.
- There is no backflow protection device on the fresh water line for the ice machine or the coffee maker.
- The walls near the dish machine and the floors under the equipment in the kitchen are in need of cleaning.
- Observed many pieces of unused equipment stored throughout the kitchen.
- The floor of the walk in cooler is covered in rust, and there are holes in the wall and ceiling near the dish machine.
- The ceiling vents in the kitchen and the filters in the exhaust system over the cook's line were observed soiled.
