Fredericksburg Restaurant Inspections: Violations At 12 Locations
A dozen restaurants around the Fredericksburg area were cited with violations during recent inspections from the health department.
FREDERICKSBURG, VA — During recent inspections by the Rappahannock Area Health District, 12 locations in Stafford and Fredericksburg recorded violations. The inspections took place between Nov. 24 and Nov. 30.
As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.
Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:
- Starbucks (Kiosk @ Giant) #0234, 550 Celebrate Virginia Parkway Fredericksburg, VA 22406
- Date inspected: Nov. 29
- Total violations: 6
- The person in charge could not provide documentation of employee health policy but was able to discuss health reporting requirements.
- Observed a food employee remove food contact equipment from the sanitizer basin of the 3 compartment sink and then proceed to handle single use beverage containers, to prepare a customer's beverage order on two separate occasions, without washing their hands prior to handling the single use beverage container.
- Through discussion with the person in charge, the steamer wands used prepare hot beverages which are in contact with TCS beverages (milk, soy milk, etc.) are being cleaned and sanitized once daily. Instructed the person in charge food contact equipment in contact with TCS foods and beverages are required to be cleaned and sanitized at least once every 4 hours. The person in charge reported the wands are being sanitized between uses. During the inspection the wands were removed for cleaning and sanitizing.
- Observed two rolls of paper towels, which have been removed from their protective wrapping, stored inside the cabinet under the handsink which may be exposed to splash. Discussed relocating the items to prevent contamination.
- Observed accumulation of debris on the following non-food contact surfaces: 1- Upsplash of both coffee brewers and 2- interior door and tray of the convection oven.
- The walls under the 3 compartment sink and under the countertop used to stored packaged beverage toppings are noted in need of cleaning.
- Peter Chang, LLC, 1771 Carl D. Silver Pkwy Fredericksburg, VA 22401
- Date inspected: Nov. 29
- Total violations: 14
- The facility does not have an employee with a valid Food Protection Manager certificate.
- Observed 3 moldy heads of cabbage in the walk in cooler. Person in charge discarded the cabbage during the inspection.
- Observed three containers of chopped celery, garlic and a sauce stored uncovered in the walk in cooler. During the inspection the person in charge covered the foods with plastic.
- Observed two microwaves and the inside surfaces of the ice machine are in need of cleaning.
- Observed the meat slicer is heavily soiled.
- Observed the facility is not using a date marking system of any foods being prepared in the facility. EHS discussed discussed date marking methods with the person in charge.
- Observed a container of frozen pork being thawed out at room temperature on a shelf. EHS discussed proper thawing methods with the person in charge. Container of pork was moved to finish thawing in the reach in cooler
- Observed 4 large containers of flour, sugar, tapioca flour, and rice without labeling.
- Observed a stack of stainless steel containers stored while wet.
- Observed single service use containers of chicken granules being reused to hold other foods.
- Observed the 3 deep freezers in the kitchen with a heavy accumulation of ice.
- Observed the following non-food contact surfaces observed in need of cleaning: 1. Shelving used to hold dried foods and equipment throughout the kitchen and walk in cooler area heavily soiled. 2.Clean and remove food debris inside surfaces of the cooling units including the gaskets.
- Observed the following: heavily cracked floor tiles going into the walk in cooler and underneath the prep table. Floor tiles missing underneath the fryers. Caulking needs replacing at the 2 handsinks and prep sink in the kitchen.
- The following were observed in need of cleaning: 1.Floors underneath all equipment. An accumulation of grease/water was observed underneath fryer cabinets. 2. AC and fan vents throughout kitchen and bathrooms. 3. Ceiling tiles throghout kitchen were observed stained.
- Subway/Auntie Anne’s Pretzel (Inside Wal-Mart), 11 Village Parkway Fredericksburg, VA 22406
- Date inspected: Nov. 29
- Total violations: 11
- Observed the in-use Quaternary Ammonium sanitizing solution in the sanitizer basin of the 3 compartment sink at 75 degrees F and 300 ppm. The person in charge disposed of the solution and retitrated to 77 degrees F and 300 ppm. Instructed the person in charge to sanitized the equipment which was in the sanitizing solution.
- Observed one chemical spray bottle which was not labeled to identify the contents and another spray bottle which was mislabeled as oven cleaner but contained dish detergent. The person in charge reported the unlabeled spray bottle was window cleaner. The spray bottles were labeled to identify the contents during the inspection.
- Observed the dispensing system at the 3 compartment sink is dispensing Quaternary Ammonium sanitizer at 500 ppm. Worked with the person in charge to manually titrate to 300 ppm and 77 degrees F. Instructed the person in charge to contact the technician to evaluate the unit and manually titrate the sanitizer, adding addtional water as needed, to obtain a concentration of 200 - 400 ppm and a solution temperature at least 75 degrees F.
- Observed three bins of various condiments/ingredients which were not labeled to identify the contents.
- Observed fly paper strips suspended from the ceiling near the back prep table and affixed to a wall above the Icee machine. Instructed the person in charge to remove the devices from the current location which could contaminate food and equipment.
- Observed a food employee begin to use a wet wiping cloth, held in Quaternary Ammonium sanitizer, to wipe the cutting board of sandwich prep unit #1. The lid was open and exposed foods in the front of the unit were at risk for contamination. The person in charge lowered the lid to protect food.
- Observed the mixer, used to prepare pretzel dough, is stored less than one foot from the back handsink which may contaminate the food during preparation processes. Discussed relocating the mixer or installing a sink guard on the left hand side of the sink to prevent contamination.
- Observed the cutting board of sandwich prep units #1 and #2 scratched and no longer easily cleanable. The person in charge reported sanwiches are cut on the cutting boards.
- The facility has two Quaternary Ammonium test kits which expired on 7/31/2021. Instructed the person in charge to purchase a new test kit.
- Accumulation of debris was noted on the following non-food contact surfaces: 1- Base of the pretzel oven, 2- top of the ice well unit , 3- shelving in the Auntie Anne's side of the facility and 4- base of the Turbo Chef.
- Observed the interior of the ceiling light covers in dry storage area and the ceiling vent in the back of the house in need of cleaning.
- Umi Japanese Fine Dining, 1500 Jefferson Davis Highway Fredericksburg, VA 22401
- Date inspected: Nov. 29
- Total violations: 1
- Observed an under counter reach cooling unit registering at 44-45 degrees using VDH thermometer.
- Vinny’s Italian Grill & Pizzeria, 11105 Leavells Road Unit 1 Fredericksburg, VA 22407
- Date inspected: Nov. 29
- Total violations: 17
- The person in charge was unable to provide documentation of training relating to the employee health policy for this facility.
- Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
- Soap was not provided at the hand washing sink in the employees' restroom. Person in charge (PIC) places soap at the sink.
- Paper towels or another hand drying method was not provided at the hand washing sink in the employees' restroom. PIC placed paper towels at the hand washing sink.
- Observed several uncovered food containers.
- Observed an employee wash dishes with soap, rinse with water, and store in the drying area, failing to properly sanitize the equipment. EHS informed the manager and the manager provided training to the employee during the inspection.
- Observed the following TCS foods cold holding at improper temperatures: Prep unit 1: four containers of pasta (43-45'F), prep unit 2: 2) sliced turkey (44'F), prep unit 3: 3) raw chicken (44-45' F) 4) raw tilapia (53'F), walk-in cooler: 5) milk (44'F), 6) three containers of cooked pasta (45'F), 7) a container of pizza sauce (44'F) 8) a container of roast beef (43'F). The person in charge re-located the fish and relocated the other items to the walk-in-cooler to bring them to 41'F. The maintenance company was contacted and came in to fix the unit while the EHS was still conducting the inspection.
- The facility does not have a date marking system for time/temperature control for safety (TCS) foods.
- Observed several flies throughout the facility.
- Observed employees in the back of the house without proper hair restraints. Employees wore hats during the inspection.
- Observed several wet wiping cloths on the counter. The person in charge placed the wet cloths in the sanitizer bucket. Food employee placed the towels in the sanitizer bucket during the inspection.
- Observed food containers stacked while wet on the shelving units in the dry storage area. The person in charge relocated the items to allow for air drying.
- The walk-in cooler and the prep unit were observed in need of repair. The person in charge contacted the maintenance company and the units were repaired during the inspection.
- The following nonfood contact surfaces were noted with an accumulation of grime and debris. 1) the interior and exterior of the reach in freezer, 2) shelving units, 3) the interior of the ovens.
- Observed the following in poor repair: 1) sealant between the coving and the wall near the prep sink has become eroded, 2) blown out light in the kitchen, 3) ceiling tiles is not sealed to the frame and rusted ceiling tile frames in the dry storage area, 4) missing and broken floor tiles under the dump sink, 5) re-caulk all sinks throughout the facility.
- The following areas were noted in need of cleaning: the walk-in cooler floor, the walk-in-freezer floor, ceiling vents, ceiling tiles, floors, and walls throughout the facility:
- Observed employees' personal belongings stored next to food and equipment throughout the facility.
- Billikens Smokehouse, 623 Caroline St Fredericksburg, VA 22401
- Date inspected: Nov. 30
- Total violations: 8
- Observed several cracked tiles of the kitchen floor.
- Observed floor drains and floors underneath equipment are in need of cleaning.
- Observed the caulking of one handsink in the kitchen and the caulking of the 3 compartment sink is missing.
- Observed a heavy presence of fruit flies in the kitchen.
- Observed a fly strip with numerous dead flies hanging above and /or near a food prep area.
- Observed three containers of dressings, blue cheese and ranch in a cooling unit with made on dates of 11/13, 11/20 and 11/16, and one gallon of buttermilk with sell by date of 11/6. Person in charge discarded the foods for safety.
- Observed the bar handwashing sink did not have soap available. During the inspection the person in charge provided soap.
- Observed the men and women's bathroom is missing the handwashing sign.
- McDonalds #7430, 419 Chatham Heights Road Fredericksburg, VA 22405
- Date inspected: Nov. 30
- Total violations: 7
- Observed an employee leave the office and then proceed to the drive thru beverage station without washing their hands. The employee was engaging in preparation of beverages for guests. Discussed handwashing requirements with the person in charge and the employee was instructed to wash their hands.
- Observed two boxes of food stored under leaking water lines in the walk in freezer. Observed frozen water on the exterior of the boxes and on the floor; The boxes were relocated to prevent contamination. Instructed the person in charge to contact a technician and discontinue storage of food under the condenser unit until repairs are completed.
- Observed container of numerous wiping cloths held in Chlorine sanitizing solution at 50 ppm and the sanitizer specifications state the concentration is required to be 100 ppm. The person in charge instructed an employee to discard and retitrate to 100 ppm.
- Observed two stacks of plastic food storage containers which were stacked while wet after chemical sanitization.
- Observed the following in poor repair: lid of the hamburger deep freezer is dented which does not allow a tight seal, interior surfaces of multiple plastic food containers in the warewashing shelving area are begining to flake and appear melted, ice build up in the 2 door reach in freezer #2.
- The Quaternary Ammonium and Chlorine testing kit in the facility have expired. Instructed the person in charge to purchase new test kits.
- Accumulation of debris observed on the following non-food contact surfaces: Fans in the food establishment, interior of drawer cooler #2, base of 2 door reach in freezers #1 and #2, inside fryers and upsplash of coffee brewers.

- Total violations: 3
- The person in charge could not provide trainig document for the employee health policy. The person in charge had the documents signed during the inspection.
- The person in charge could not provide a thin probe thermometer.
- Observed that the light shields are missing in the kitchen.
- Hunan Garden, 923 Caroline Street Fredericksburg, VA 22401
- Date inspected: Nov. 30
- Total violations: 7
- Observed an employee drinking from an uncovered coffee cup in the kitchen. The employee was directed to dispose of the drink.
- The wire storage shelving units in the walk in cooler are heavily corroded and not easily cleanable.
- The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: 1-The outer surfaces of the reach in freezer, 2-the outer surfaces of the homestyle reach in freezer, 3-the shelving in the dry storage room, 4-the gaps between the wok station, the deep fryers and the cook top on the cook's line, 5-the deep fryer vents, 6-the top of the reach in cooler on the cook's line, 7-the outer surfaces of the walk in cooler, 8-the top of the cook top.
- Observed that the following physical structures are being cleaned as often as necessary: 1-the floor under the deep fryers, 2-the caulking along the 3 compartment sink (moldy), 3-the walls in the kitchen.
- The ceiling tiles in the kitchen are porous and not easily cleanable.
- Less than 50 foot candles of light was noted under the hood on the cook's line. Only 15 foot candles were observed.
- The inner and outer surfaces of the ventilation hood over the cook's live were observed soiled with grease and grime.
- Marian Manor Home For Adults, 177 Old Potomac Church Road Stafford, VA 22555
- Date inspected: Nov. 30
- Total violations: 12
- Observed a food employee change tasks then engaged in food preparation without first washing their hands and donning gloves. Employee was asked to wash their hands.
- Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. EHS instructed the food employee to rewash their hands, discussed correct hand washing procedures.
- Observed raw chicken that was thawing over shelled eggs in the reach-in-cooler.
- Observed three bins of food contact utensils stored in soiled containers. The food employee removed the items to be washed, rinsed, and sanitized.
- The dishwasher is not sanitizing (0 ppm) equipment and utensils. The EHS discussed sanitizing the equipment and utensils at the 3 compartment sink (3CS), until the dishwasher can be repaired. The dishmachine was repaired during the inspection.
- Observed the following Time/Temperature Control for Safety (TCS) Food cold holding at improper temperatures: 1) a container of sweet peas (43'F), a container of lasagna (45'-46'). The items were discarded.
- Per discussion with the food employee, the date marking on the containers is for the discard date, observed two containers that exceeded that date. The food employee suggested that their colleague must have written the preparation date instead of the discard the date. EHS discussed the importance of having all employees use only one system.
- Observed no visual or audible alarm on the dishmachine.
- The following nonfood contact surfaces are noted with an accumulation of grime and debris: 1) the interior of the reach in freezers, 2) shelving units, 3) interior of drawers, 4) oven.
- Observed a leak under the 3-compartment sink.
- The following areas were noted in need of cleaning throughout the facility: ceiling (including vents), floors, and walls.
- Observed the following in need of repair: 1) ) several ceiling tiles throughout the facility need to be re-adjusted. 2) broken tile at corner at the hand washing sink, 3) broken light shield.
- Fairfield by Marriott, 10330 Spotsylvania Avenue Fredericksburg, VA 22408
- Date inspected: Nov. 30
- Total violations: 3
- The person in charge was unable to provide written verification the employees had received training regarding Employee Health. Sample Employee Health Policy was provided to the person in charge.
- A coffee urn was observed in the hand wash sink in the kitchen rendering it impossible to wash hands in sink. Urn was removed during inspection.
- The yogurt (43'F) and milk (44'F) in the reach in cooler was observed cold holding at improper temperatures. It was discovered that the door was not closed all of the way. Door was closed and temperatures returned to proper levels.
- Ledo Pizza & Pasta, 9849 Jefferson Davis Highway Fredericksburg, VA 22407
- Date inspected: Nov. 30
- Total violations: 2
- Observed sliced cheese (43, 44 degrees F) and pre-cooked meatballs (45 degrees F) cold holding at improper temperatures in the pizza prep cooler. The person in charge reported the foods have been in the unit overnight and voluntarily discarded for safety. Observed the foods stored by the door of the unit. The temperature was lowered in the unit during the inspection. Recommended placing TCS foods in the back of the unit.
- Observed the dishwasher drainboard and front hand sink are not sealed to the adjacent wall or countertop.
