Fredericksburg Restaurant Inspections: Violations At 15 Locations

The Rappahannock Area Health District found violations at several restaurants across Stafford County and the City of Fredericksburg.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected multiple restaurants across the region from June 30 to July 6; they found more than a dozen restaurants with violations.

As coronavirus restrictions continue to loosen around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department also performs a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

    • Date inspected: June 30
    • Total violations and corrective actions: 5
    • Violations:
      • Observed the cook wash his hands in the 3-compartment sink.
      • Observed employee touch his pants several times without washing his hands before preparing foods and donning new gloves. EHS spoke to the employee several times about this.
      • Observed the cook handle cooked potatoes with his bare hands.
      • Observed equipment in the sanitizer sink where the solution measured at 100 ppm
      • Observed sliced meats, cut tomatoes, and cut lettuce that were not date marked.
  • Hardee’s of Stafford #2942, 567 Warrenton Road Fredericksburg, VA 22406
    • Date inspected: June 30
    • Total violations and corrective actions: 6
    • Violations:
      • Observed a food employee handle a numbered food tag and then proceed to cook food at the cook line wearing the same single use disposable gloves. The food employee was preparing to handle utensils used at the cook line.
      • Observed the following TCS foods holding at improper temperatures: Ham/cheese bundles (56, 56 degrees) in the 1 door reach in cooler by the cook line, sliced tomatoes (54 degrees F), Sliced ham (49 degrees F) and sliced cheese (43 degrees) holding in the cold holding well connected to the prep cooler, and single use containers of milk (50 degrees F) holding in a container of ice at the drive thru area, ice was observed in contact with the bottom quarter of the container only. The person in charge reported the TCS foods were placed in the units between 8:30 and 9:30 a.m.
      • Observed in use Quaternary Ammonium sanitizer at 500 ppm in the three compartment sink. The sanitizer is exceeded required concentration for safety. During the inspection the solution temperature was 139 degrees F, solution sample was cooled to 78 degrees for testing.
      • Observed absence of ambient air thermometers inside the front beverage cooler and prep unit.
      • Observed accumulation of debris inside and exterior doors of refrigeration equipment with the exception of the walk in coolers, interior shelving along the front counter ares, wheels of equipment in the cooking area and top of the hamburger deep freezer.
      • Observed the floors near cooking equipment, ceiling vents and ceiling tiles in the kitchen in need of cleaning.
      • Observed light intensity less then 10 foot candles of light near the fryers. Observed numerous burned out ceiling lights in the kitchen.
  • Dalia's African Cuisine, 364 Garrisonville Rd Suite #101 Stafford, VA 22554

    • Violations:
      • Observed the employee rinse her hands in the pot with hot water where the boiling eggs are being prepared.
      • There is no hand soap at the only hand sink.
      • Observed that no paper towels were available at the hand sink in the kitchen.
      • Observed uncovered jerk chicken in the walk in freezer. Observed uncovered foods and spices on the storage shelves.
      • Observed cooking equipment, ceramic bowls, and serving dishes soiled.
      • Observed rice and beans at 47'F, yellow rice at 49'F, and soup at 50'F in the walk in cooler.
      • Observed rices and soups that were made yesterday at 2 pm not date marked.
      • Observed several trays of jerk chicken that was uncovered that was made this morning stored below equipment to cool.
      • Observed several flies and small gnats inside of the kitchen.
      • Observed a lot of equipment stored with the food contact surfaces exposed.
      • The walk in cooler is in disrepair.
      • The entire kitchen is in need of detailed cleaning. Observed food on the floors behind the equipment. The floor drain under the 3-compartment sink has some type of food on the top of the drain cover. The walk in coolers floors need cleaning.

      • Observed the following TCS foods holding at improper temperatures in the walk in cooler which have been in the unit overnight: Cut lettuce ( 43 degrees F), Sour cream ( 44 degrees F), sliced American Cheese (43 degrees F), whipping cream (43 degrees F), sliced ham ( 42 degrees F), sour cream (44 degrees F), feta cheese (45 degrees F), ricotta cheese (43 degrees F), cooked eggplant (43 degrees F).

      • Observed employees drinks stored on the prep tables in the kitchen.
      • Observed employee handling dirty equipment then handling clean equipment without washing her hands in between. Observed employee cleaning the prep tables with gloved hands, change gloves, then continue to prepare for the lunch meal without washing her hands between glove usage. EHS addressed this with the employee.
      • Observed that the top of the coffee machine, juice macine, can opener, and the interior of the ice machine have accumulations of grime and debris. The person in charge cleaned the items during the inspection.
      • Observed that the inside of the microwave requires cleaning.
      • Observed a sanitizer spray bottle stored beside the open container of single service portioning cups on the bottom shelf of a prep table. The person in charge removed the spray bottle.
  • Jimmy John’s, 1291 Carl D. Silver Pkwy Fredericksburg, VA 22401

    • Total violations and corrective actions: 6
    • Violations:
      • The person in charge (PIC) could not provide an employee health policy for the food employees. EHS provided a sample copy of the employee health policy. During the inspection, the PIC had the food employees sign the employee health policy agreement.
      • Observed a plastic pitcher in the handsink next to the oven. During the inspection, the pitcher was relocated
      • No hand soap at the handsink next to the oven. During inspection, hand soap was provided.
      • Food employee could not provide a food temperature measuring device.
      • Observed several plastic food containers on the drying rack that were stacked while wet after cleaning and sanitizing.
      • The light intensity in the dry storage area is 4.2 foot candles.
  • Firehouse Subs, 1465 Stafford Market Place 101 Stafford, VA 22556
    • Date inspected: July 1
    • Total violations and corrective actions: 7
    • Violations:
      • Observed a food employee handle Quaternary Ammonium sanitizer with their bare hands and then proceed to touch the edges of a cutting board in the front prep area.
      • Observed a food employee touch the point of service monitor wearing single use disposable gloves and then proceed to touch pre-cooked steak to prepare a sandwich for a customer using the same gloves.
      • Observed meatballs (129, 130 degrees F) hot holding at improper temperatures in the hot holding steam well. The person in charge reported the meatballs have been in the unit since 10:30 a.m, temperature was taken at 2:00 p.m.
      • Observed the following TCS foods cold holding at improper temperatures: Prep cooler #1- Diced tomatoes ( 44 degrees F), shredded lettuce (44 degrees F), cut lettuce (45 degrees F), shredded cheese (48 degrees F), sliced tomatoes (50 degrees F) and feta cheese (51 degrees F) and self-service beverage cooler - single use container of milk (56 degrees F). The temperature was lowered in the prep cooler during the inspection, however, a temperature change was not observed. Observed water at the base of prep cooler #1 and icing on the top unit of the self-service beverage cooler. The person in charge reported the milk was placed in the unit this morning and the TCS foods in the prep cooler were pulled from the coolers in the back of the house at 10:00 a.m. Food temperatures were taken between 2:00 and 2:15 p.m.
      • Upon discussion with the person in charge, the food establishment is thawing pre-cooked steak in warm water prior to holding in refrigeration units to cool.
      • Observed stainless steel bowls and plastic food storage containers stacked while wet after chemical sanitizing.
      • Accumulation of debris was observed on the following non food contact surfaces: Exterior door release for the left side microwave, exterior doors of refrigeration equipment and kick place of the 2 door upright cooler.
  • Pizza Hut Powhatan, 1224 Powhatan Street Fredericksburg, VA 22401
    • Date inspected: July 1
    • Total violations and corrective actions: 3
    • Violations:
      • Observed no paper towels at the handsink next to the 3 compartment sink. During inspection, employee provided paper towels.
      • Observed 3 plastic containers and 3 plastic cups that were stacked while wet after cleaning and sanitizing. During inspection, the person in charge re-arranged containers and cups to allow to air-dry.
      • The light intensity at the mop sink is 1.5 foot candles.
  • Tila’s Cocina on Board (UUF-8211 VA), 12 Colyer Road Fredericksburg, VA 22406
    • Date inspected: July 1
    • Total violations and corrective actions: 8
    • Violations:
      • Observed one stainless steel plan, two cutting boards and one spatula soiled.
      • Absence of a consumer advisory noted on the menu posted on the exterior of the mobile unit for eggs which may be prepared undercooked upon a customer's request.
      • Observed two lighters stored in a drawer with packaged spices.
      • The small reach in cooler was observed at 45 degrees F and is not adequately functioning to cold hold TCS foods. The person in charge reported whole uncut vegetables and cheese are held in the unit.
      • Observed accumulation of debris on the following nonfood contact equipment in the mobile unit: Racks inside the oven, containers used to store food contact equipment, and wells of hot holding equipment.
      • Observed duct tape around the plumbing connection under the middle basin of the three compartment sink and the fixture was dripping when the water is running.
      • The person in charge could not provide a backflow preventer for the food grade hose to prevent backflow while filling fresh water tanks
      • The floors, walls, and sinks of the mobile unit are in need of cleaning.
  • Tea Tyme, 814 Caroline Street Fredericksburg, VA 22401
    • Date inspected: July 2
    • Total violations and corrective actions: 8
    • Violations:
      • Observed the 2 glass measuring cups in water in the handsink. During the inspection, the cups were relocated to the 3 compartment sink and handsink cleaned.
      • Observed several plastic containers of popping pearls and jam on the floor throughout the kitchen.
      • Observed 2 wet towels next to the paper towels and 1 at the prep unit.
      • Observed 4 stirring spoons in water (73 degrees F) in a metal container. During the inspection, the spoons and container washed and sanitized.
      • The following are in need of repair: 1. The reach in freezer door handle is loose. 2. Plastic containers are being used to keep the ice machine off the floor under the 3 compartment sink drainboard.
      • All nonfood contact surfaces in the establishment are in need of cleaning.
      • Observed an excess amount of unneccessary items throughout the kitchen (personal items, ceiling tile, vacuum, boxes, misc. items).
      • The kitchen floor is in need of cleaning.
  • Dunkin Donuts & Baskin Robins, 305 Garrisonville Road Suite #102 Stafford, VA 22554
    • Date inspected: July 2
    • Total violations and corrective actions: 8
    • Violations:
      • Absence of active managerial control observed during the inspection. The person in charge is not monitoring cold holding temperatures of TCS foods and monitoring hand washing.
      • Observed a food employee rinse their hands in the front area hand sink wearing single use disposable gloves.
      • Observed a food employee handle money and touch the point of service computer screen wearing single use disposable gloves, remove gloves and then proceed to handle blender equipment to prepare a dessert beverage in the ice cream section of the facility without washing their hands.
      • The following TCS foods were observed cold holding at improper temperatures: Prep cooler- sliced cheese (54, 56 degrees F), pork sausage patty (51 degrees F), pre-cooked egg patty (46, 50 degrees F), pre-cooked turkey sausage (50 degrees F), pre-cooked vegetable egg patty (50 degrees F), bagel cream cheese bites (46 degrees F), single use container of dairy creamer (59 degrees F) and 1 door upright cooler - milk (55 degrees F) and liquid whipped topping (52 degrees F). The person in charge reported the TCS foods in the prep unit were placed in the cooler at 9:00 a.m, and the milk and whipped topping have been in the 1 door upright cooler since this morning but could not specify a time.
      • Observed chemical spray bottle of disinfectant clean stored hanging over shelving at the three compartment sink over cleaned and sanitized food contact equipment.
      • Observed wiping cloths held in Quaternary Ammonium sanitizer at 0 ppm at located near the hot chocolate machine and coffee brewing station. Also observed a pile of wet cloths stored on top of a work table in the back of the house. A food employee reported the wiping cloths have been on the table for about 30 minutes.
      • Accumulation of debris was noted on the following non food contact surfaces: Base/shelving of the self service beverage cooler, up splash of the hot chocolate machine, exterior surfaces of the ice bin at the ice cream station, base of the Adamatic oven and stainless steel container lid stored on shelf under the Adamatic oven.
      • The following areas are in need of cleaning: wall behind the three compartment sink, ceiling vents in the back of the house, female toilet room and above the Xpress ovens.
  • Paradise Diner, 260 Warrenton Road Fredericksburg, VA 22405
    • Date inspected: July 6
    • Total violations and corrective actions: 10
    • Violations:
      • Observed food contact surfaces of the meat slicer soiled.
      • Observed the following TCS foods cold holding at improper temperatures, the person in charge reported the TCS foods have been in the refrigeration units overnight: 1- Prep cooler- Sliced tomatoes (47 degrees F), shredded lettuce (44 degrees F), Tzatziki sauce ( 47 degrees F), and grape leaves stuffed with cooked rice ( 48, 44 degrees F) and 2- 2 door reach in cooler- Whipping cream (51 degrees F). Temperatures were taken between 11:20 and 11:30 a.m.
      • Observed the front door to the food establishment are not tight fitting, observed a gap along the bottom of the door.
      • Observed the presence of multiple flying insects in the kitchen and dining room and two crawling insects in the kitchen. The person in charge provided a pest control invoice however, further monitoring and treatment are needed.
      • Observed the following equipment in poor repair- damaged gaskets in the prep cooler, water at the base of both the prep and 2 door reach in coolers.
      • The person in charge could not provide a Chlorine test kit.
      • Accumulation of debris was noted inside the fryer and exterior of refrigeration equipment.
      • Observed the following in poor repair: Blown out ceiling lights in the kitchen and the stainless steel flooring in the warewashing area has become rusted in sections.
      • Cleaning of the floors and walls is noted in the warewashing area.
  • Quality Inn, 543 Warrenton Rd Fredericksburg, VA 22406
    • Date inspected: July 6
    • Total violations and corrective actions: 8
    • Violations:
      • The person in charge reported the facility does not have a certified food manager.
      • The person in charge could not provide documentation of an employee health policy but was knowledgeable of symptoms and illnesses requiring restriction and exclusions.
      • Hand drying provision was not observed at the kitchen and employee toilet room handsinks.
      • Observed two containers covered with a lid were observed stored in the hand washing sink in the warewashing area hand sink.
      • Observed the interior surfaces of the kitchen microwave soiled.
      • Observed two large plastic containers used to store single use packages of cereal stored on the floor in the back of the house.
      • Observed the kitchen hand sink, 2 vat sink and counter top behind the 2 vat sink are not sealed to the countertop (for the sinks) or the adjacent wall
      • Accumulation of debris was observed on the following nonfood contact surfaces: Dish drain and utensil holder, up splash of the juice machine and base of the toaster oven.
