Fairfax County reported violations at China Village, Amphora's Diner Deluxe, and others according to inspections from June 2-6.

Critical violation: Observation (CORRECTED DURING INSPECTION): Different types of raw animal foods stored in such a manner that may cause cross contamination as follows: OBSERVED RAW CHICKEN NEXT TO RAW SHRIMP, RAW BEEF AND RAW SHELLED EGGS IN WALK-IN-COOLER.
Non-critical violation: Observation (CORRECTED DURING INSPECTION): The following food items that are not easily identified by appearance were observed without a label: SALT AND SUGAR ALONG THE COOKLINE.

Critical violation: Observation (CORRECTED DURING INSPECTION): Raw animal food stored over ready-to-eat food in the refrigeration unit. Raw shrimp over salsa in the 3DR prep cooler.
Critical violation: Observation (REPEAT) (CORRECTED DURING INSPECTION): The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: milk at 70F in 1DR UR cooler. Upon discussion with person in charge, it was determined that the milk had been left out at room temperature in the morning for coffee.
Critical violation: Observation (REPEAT) (CORRECTED DURING INSPECTION): The following ready-to-eat food that is made using a combination of ready-to-eat, potentially hazardous ingredients is not properly dated for discard using the date of the first-prepared ingredient: cooked beans, tomato salsa in 2DR FT cooler.
Critical violation: Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: NY steak, parillada, lomo saltado.
Critical violation: Observation (REPEAT) (CORRECTED DURING INSPECTION): Observed dirty lids for containers stored with sanitized food-contact surfaces.
Non-critical violation: Observation: Scoops without handles are being used for salsa, drinks, and other food items.

Critical violation: Observation: OBSERVED THAT ASTERISKS WERE PROVIDED FOR UNDERCOOKED/COOKED TO ORDER FOOD ITEMS, BUT NO FOOTNOTE PRESENT THAT STATES THAT THE ITEMS ARE SERVED RAW OF UNDERCOOKED, OR CONTAIN RAW OR UNDERCOOKED INGREDIENTS. :
Critical violation: Observation (REPEAT) (CORRECTED DURING INSPECTION): The concentration of the Bleach solution in the 3-vat-sink and multiple sanitizer buckets was measured at >200 ppm.
