Leesburg Health Inspections: Bhai Sahab, Red Horse Tavern

The Loudoun County Health Department recorded violations at several facilities around Leesburg during recent restaurant inspections.

LEESBURG, VA — Several restaurants around Leesburg and western Loudoun County recorded violations during recent inspections from the health department. Inspectors conducted the most recent restaurant visits between Feb. 15 and Feb. 21.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • West End 1915, 36855 W Main St Purcellville, VA 20132
    • Date inspected: Feb. 15
    • Total violations: 4
      • Observed hand washing sink on the cook line blocked by a large container filled with water. Hand washing sinks shall be accessible at all times and used for hand washing only.
      • Observed container of mussels in prep unit without the shellstock tag. Facility dated the tag with 2/14; however, the last shellstock from the container has not been sold or served. The tag shall be dated once the last shellstock from the container is sold or served and maintained in the facility for 90 days in chronological order.
      • Observed pork (48F) and coleslaw (44F) cold holding at improper temperature in prep unit across from fryers.
      • Observed no irreversible registering temperature devise for the high temperature dish machine to measure if the machine is properly sanitizing dishes.
  • Red Horse Tavern, 112 W Washington St. Middleburg, VA 20118
    • Date inspected: Feb. 15
    • Total violations: 7
      • Observed: Not all food handling employees (at the time of inspection) had been trained in the signs and symptoms of foodborne illness. A verifiable means of training (employee health policy 1B form) needs to be available for Environmental Health Specialist to view.
      • Observed: employee hand sink in salad prep area, near walk-in freezer is not equipped with cleanser. Employee hand sinks need to be supplied with cleanser.
      • Observed: hand sink near walk-in freezer does not have paper towels. Employee hand sinks need to be supplied with paper towels.
      • Observed: Two Handwash sinks (1) at bar and 2) in kitchen next to exit) are blocked with trays and miscellaneous items. Employee handwash sinks are to be used only as handwash sinks.
      • Observed: Condiments, salad dressings in squeeze bottles in salad prep table that are used beyond 24 hours do not have date marks. Date mark TCS Time.temperature control for safety) items that are used in facility beyond 24 hours.
      • Observed: there is no thermometer to check the temperature of the wares being sanitized in the high temperature dish machine. Provide an irreversible thermometer or thermo test strips to check the temperature of the high temperature dish machine.
      • Observed: General accumulation of buildup (grease, residue) on walls, behind grill area), corners of floors, etc.. Maintain floors, walls, ceilings clean at times when foods are in protected storage and when food is not being prepared or served.

    • Date inspected: Feb. 15
    • Total violations: 3
      • Items in the 2nd line cooler are cold holding above 41F. Temperature control for safety foods (TCS) must be held below 41F.
      • Large quantity of chicken wings were cooked and placed in covered pan with lid to cool. Cooling methods must be adequate to cool foods with in the required 6 hours.
      • Vent covers above dishwasher, in front of grill line hood, and covering ice machine are collecting dust and debris. Filter baffle was removed above the grill line. Ventilation systems must be clean and maintained in proper working order.

      • Observed: Low temperature dish machine is not delivering chlorine = 0 ppm chlorine.

      • There is no Manager at this facility that is trained as a Certified Food Manager Certification (CFPM). Obtain this required credential.

    • Total violations: 3
      • Employee restroom did not have a hand washing signage instructing employees to wash their hands.
      • Employee was observed wearing single service gloves handing dirty dishes then proceeded to handle clean pans with the same pair of gloves thus contaminating the clean pans.
      • Single service cup was observed stored in the ice bin in use at the coffee station. Single service cups do not have handles to prevent hand contact with the ready to eat ice. Single service utensils do not hold up for extended use or service.

    • Total violations: 3
      • The low temperature dish machine at bar was checked by staff for the inspector and found the chloring sanitizer concentration was not reading high enough to sanitize bar glasses.
      • The caramelized onions that were cooked this morning was observed with an internal temperature of 52 degrees F after 5 hours while stored at prep cooler online, The onions were not cooling down fast enough to prevent bacterial growth.
      • Cooked beef short ribs were observed cold holding at upper compartment of prep cooler at 15 degrees F. Cold holding of foods needs to be 41 F or less.

    • Date inspected: Feb. 16
    • Total violations: 2
      • Many prepared items are not date labeled to indicate when they were prepared and date for disposition. All ready-to-eat TCS (temperature control for safety foods) need to be date labeled for use/discard within 7 days.
      • Spray bottle of bleach cleaner was placed on shelf in walkin cooler next to orange juice and above an open container of cut potatoes. Do not store cleaners in a manner that may contaminate food.

    • Date inspected: Feb. 16
    • Total violations: 2
      • Observed 3 compartment sink sanitizer (lactic acid) below minimum range of 704 ppm. Sanitizer (lactic acid) sanitizing range is 704-1875 ppm.
      • Observed improper cooling temperature of roasted turkey 52F, pepper steak 47F, smoke chicken 50F foods item according to label was prep more than 4 hours when temperatures were taken.

      • Observed employee drink container, without a lid, on top of the dish machine while dish machine was in use. An employee shall drink only in designated areas where the contamination of exposed food; clean equipment, utensils, and linens; unwrapped single-service and single-use articles; or other items needing protection cannot result.
      • Observed pico de gallo (57F) and shredded cheese (59F) cold holding at improper temperature next to the steam table on ice. Time/ temperature control for safety foods shall be cold held at 41F or below.
      • Observed chicken thawing in the 3 compartment sink in stagnant water. Foods shall be thawed under refrigeration that maintains the food temperature at 41°F or less, or completely submerged under running water at temperature of 70°F or below with sufficient water velocity to agitate and float off loose particles in an overflow, or as part of a cooking process.

      • Observed improper cold holding temperature half and half (at top) at milk station. Time/temperature control for safety held cold should held 41F or below.

    • Total violations: 5
      • Observed food employee use clean knife to open a box from delivery and then place the knife back in use without wash, rinsing, and sanitizing. Food contact surfaces should be wash, rinsing, and sanitizing before being place in use.
      • Observed dry chick peas being stored on the floor in a large plastic container uncovered in dry store area. Food should be protected from environmental contamination.
      • Observed improper cold holding temperatures of lamb in sauce 52F in walk in cooler and cheese 65F on counter of prep cooler. Foods held cold should be maintained at 41F or below.
      • Observed clean utensils in a plastic container store on the floor in the dishwashing area. Food in packages and working containers may not be stored less than 6 inches off the floor.
      • Observed ice machine plastic guard with a build up of dirt and debris. Non food contact surfaces should be cleaned at a frequency necessary to preclude accumulation of soil residues.

    • Date inspected: Feb. 21
    • Total violations: 2
      • Observed several containers of prepared foods stored past their expiration date: veggie salad, cherry compote, blueberry compote, boiled eggs. Foods shall not be held past their expiration date.
      • Insufficient lighting provided in dry storage area. At least 10ft candles of light is required where food is stored. Lighting requirement is at a minimum height of at least 30 inches off floor.

    • Total violations: 1
      • No soap was provided at the hand sink near the fryers. All hands sinks need to be stocked with hand soap at all times.
