Leesburg Health Inspections: Green Tree Tavern, Patma Thai Kitchen

The Loudoun County Health Department recorded violations at several facilities around Leesburg during recent restaurant inspections.

LEESBURG, VA — The Loudoun County Health Department recorded violations at several Leesburg area restaurants during inspections from Oct. 12 to Oct. 18.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

    • Date inspected: Oct. 12
    • Total violations: 1
      • Observed hot dogs 45F and coleslaw 50F in reach in cooler at an improper cold holding temperature.
  • Patama Thai Kitchen, 101 S. Maple Ave, Ste E Purcellville, VA 20132
    • Date inspected: Oct. 12
    • Total violations: 3
      • Observed 2 containers of household insect spray killer in the kitchen.
      • The following methods used for cooling were not adequate to facilitate proper cooling: placing warm, cooked meat in a bag and sealing the bag, placing warm sauce in a container and placing the tight fitting lid on the container.
      • Observed in-use utensils stored in room temperature, stagnant water next to the rice warmer.

    • Date inspected: Oct. 12
    • Total violations: 1
      • This violation has not been corrected at follow-up inspection. Employee health policy is not present at time of follow-up inspection.
  • Green Tree Tavern, 15 South King Street Leesburg, VA 20175
    • Date inspected: Oct. 13
    • Total violations: 6
      • Observed no Person In Charge (PIC) upon EHS arrival. Employee called PIC and PIC was on their way to the facility.
      • Observed no verifiable manner, that all employees are aware to report to the person in charge information about their health and activities as it relates to diseases that are transmissible through food.
      • Observed raw shell eggs stored over ready-to-eat bread in the walk in cooler.
      • Observed container of cooked onions with a prepared by date of 10/5.
      • Upon discussion with the Person In Charge (PIC), it was stated that the facility will offer burgers raw or undercooked. No consumer advisory was observed on the menu.
      • Observed food stored on the floor in the walk in freezer.

    • Date inspected: Oct. 17
    • Total violations: 1
      • Observed: There was no signage in the employee restroom instructing employees to wash hands.
  • MOD Pizza, 150-A Purcellville Gateway Dr Purcellville, VA 20132
    • Date inspected: Oct. 18
    • Total violations: 1
      • Observed: Siracha date marked October 3 has gone beyond 7seven days after being repackaged and held in cold holding.
