Leesburg Health Inspections: Pho Bar, Pupatella

The Loudoun County Health Department recorded violations at several facilities around Leesburg during recent restaurant inspections.

LEESBURG, VA — Several restaurants around Leesburg and western Loudoun County recorded violations during recent inspections from the health department. Inspectors conducted the most recent restaurant visits between Feb. 1 and Feb. 7.

Most COVID-19 restrictions around the state have relaxed, so health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

  • Kidtime Christian Academy, 71 N. Laycock St Hamilton, VA 20158
    • Date inspected: Feb. 1
    • Total violations: 1
      • Spray bottle chlorine sanitizing solution used was not at an acceptable concentration. Chlorine ppm was above 200 ppm chlorine. Maintain concentration between 50-100 ppm.
  • Pho Bar, 19385 Promenade Dr Leesburg, VA 20176
    • Date inspected: Feb. 1
    • Total violations: 7
      • Observed hand sink at bar blocked by a pot setting in front and metal container holding wet clothes. Hand sink should be available for employees use at all times.
      • Observed the blade of the slicer with food residue on the blade. Food contact surfaces should be clean to sight and touch and as need when in use.
      • Observed in prep cooler 1 cut lettuce 57F in strainer above capacity of prep cooler, noodles in strainer cut cabbage 52F, and sliced tomatoes 52F on shelf at an improper cold holding temperature. Food held cold should be maintained at 41F or below.
      • Observed broth and cooked chicken in the walk in cooler without date marking foods items should have at date of when prepped.
      • Observed boba 93F setting on the counter facility has written procedures for time as a public health control (TPHC) but boba was not label with a four hours time frame.
      • Observed cooked beef 126F in a container packed full in walk in cooler possible not allowing to cool properly.
      • Observed wet cloths stored in the hand sink in the bar with 0 ppm chlorine when storing wiping clothes in chlorine sanitizer should be 50-100ppm.
  • Coach's Corner Grill, 100 A Purcellville Gateway Ct Purcellville, VA 20132
    • Date inspected: Feb. 1
    • Total violations: 3
      • Observed: Employee touched ready-to-eat (RTE) celery with bare hands. Use of a barrier (tongs, gloves, deli paper) need to be used to minimize bare hand contact with RTE food.
      • Observed: There is no signage at employee handwash sink instructing employees to wash hands.. Each employee hand sink needs to have signage instructing employees to wash hands. Person in Charge (PIC) provided required signage. Corrected.
      • Observed: cardboard is being used to line wire racks holding dry storage food items in kitchen. A cleanable surface needs to be provided on wire racks.

    • Total violations: 1
      • Spray bottle of degreaser stored at dishwasher area was not labeled with the contents. Chemicals and cleaners must be labeled with common working name.

    • Total violations: 2
      • No supervisors or managers are certified food protection managers (CFPM). At least one employee with supervisory or management responsibility needs to have a CFPM certificate
      • Observed: Crabcake mixture, spanakopita in kitchen refrigerator do not have date marks identifying the date that they are to be discarded. Ready to eat TCS food prepared and held more than 24 hours need to be clearly marked to indicate the date or day the food shall be consumed or discarded when held at a temperature of 41F or less for a maximum of 7 days.

    • Total violations: 3
      • Observed bar hand washing sink used to dump beverages. Bar hand washing sink observed with beverage ice in the basin of the sink. Hand washing sinks shall be used for hand washing only.
      • Observed side exterior door not tight fitting. Door was observed to have daylight around door seams. Outer openings shall be tight fitting to prevent the entry of pests.
      • Observed outside dumpster overflowing with trash. Dumpster lid was unable to close. Refuse area shall be of sufficient capacity to hold refuse.

    • Total violations: 2
      • Facility does not have sanitizer test strips to test the sanitizer strength at the low temperature dish machine. Sanitizer test strips shall be available so employees can monitor the strength of the sanitizer to ensure it is within the correct range.
      • EHS observed an accumulation of food debris, dust and grime built up on top of the dish machine and also on the air vents next to the steam table. Maintain non-food contact surfaces clean to sight and touch.

    • Total violations: 3
      • Observed marinara (115F) and meatballs (118F) hot holding at improper temperatures in the steam well by the stove.
      • Observed several food items in the walk in refrigeration unit not properly dated for disposition. Per PIC, some items were prepared over 24 hours ago. If the facility holds the items for over 24 hours, the food shall be properly date marked for disposition.
      • Observed time as a public health control being used for pizza slices not marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food was removed from temperature control.
  • Doner Bistro Mobile, 37214 Bolyn Rd Purcellville, VA 20132
    • Date inspected: Feb. 2
    • Total violations: 1
      • Observed cut red cabbage kept past 7 days. Red cabbage must be served, sold or discarded within 7 days of preparation including the date of preparation of the first prepared ingredient. Person in charge voluntarily discarded.

      • Observed doner meat in walk in cooler without a date mark. Food items kept more than 24 hours should have a date mark.
      • Observed cut red cabbage and tzatziki sauce kept past 7 days. Red cabbage and tzatziki sauce must be served, sold or discarded within 7 days of preparation including the date of preparation of the first prepared ingredient.

      • Management doesn't have the signed form of the proper documentation of the new hired employee health policy showing kitchen staff are properly trained to report symptoms of illnesses transmittable through food. Manager must document staff training.
      • Employee observed touching the pot's handle with the gloves on and then touch the cooked bacon to place it on a plate. Employees must wash hands and change gloves in between tasks.
      • Rice, Chili, Cooked chili in the walk-in cooler noted as improperly cooled to prevent the growth of harmful bacteria. These were made yesterday and has been cooling for more than 6 hrs. Cool time/temperature control for safety (TCS) food(s) from 135F to 41F within a total of 6 hours and from 135F to 70F within 2 hours.
      • The menu doesn't have the consumer advisory on it. Any animal origin food that is being served not fully cooked needs to contain an asterisk to identify them as being undercooked.
      • Facility doesn't have a maximum irreversible registering thermometer on site for the high temperature dish machine in order to check the proper sanitation of the utensils.

      • Observed marinara cooling is 2 door reach in cooler in a plastic container tightly close with a lid this may not allow to cool properly.

      • Tangerines were peeled with bare hands. Certified Food Protection Manager (CFPM) indicated gloves were removed from kitchen and used bare hands and spoon to prepare. Use gloves; no bare hand contact permitted with ready to eat foods.

      • Observed employee washing dishes in the dishroom. The employee loaded the conveyor belt style dish machine with dirty wares then changed their gloves with a new pair of gloves they pulled out of their pants pocket before removing clean wares out of the dish machine. The employee did not wash their hands after removing the first pair of gloves. Clean wares shall only be in contact with clean utensils including single use gloves.

    • Date inspected: Feb. 3
    • Total violations: 3
      • Hot water at hand sinks were observed less than 100F, observed 75F and 90F. Hot water shall be adequate supply. Hand sinks are to provide 100F for proper handwashing.
      • Milk in use observed dated 2/1/2023; two days beyond use-by date. Do not use beyond the use-by date.
      • Written plan for responding to vomit or diarrheal accidents not identified by Person-in-Charge. Have plan and know how to implement as needed. Provide plan.
  • Heritage Hall Leesburg, 122 Morven Park Rd Nw Leesburg, VA 20176
    • Date inspected: Feb. 6
    • Total violations: 2
      • Observed cooling purees to not properly meet the cooling parameters. Chicken puree (93F), carrot puree (105F), and squash puree (114F) were observed not properly cooled from 135F to 70F within 2 hours. Foods were temperature checked by EHS at 12:00 PM. Said foods, according to the cooling log book, started the cooling process around 8:00 AM. Foods ranged from 139-140F according to cooling log book at 8:00 AM. Log book indicates puree reached 70F by 10:00 AM, however, when inspector temperature checked the foods on ice at 12:00 PM, the foods did not. Facility has 2 hours to cool food from 135F to 70F and a total of 6 hours to cool food from 135F to 41F or below.
      • Observed tomato puree and beef puree hot holding at improper temperatures. Tomato puree observed at 115F and beef puree observed at 105F. Hot hold time/ temperature control for safety foods at 135F or above.
  • Loudoun Nursing & Rehabilitation Center Inova Cornwall, 224 Cornwall St Leesburg, VA 20175
    • Date inspected: Feb. 6
    • Total violations: 1
      • Observed exterior door near dry storage area to not be tight fitting. Door was observed with daylight around the door seam at the bottom of the door. Ensure outer openings are tight fitting to prevent the entry of pests.

    • Total violations: 4
      • Employee health policy is present but not a signed agreement with employees. A signed (or verifiable) copy of the employee health reporting policy documenting training and adherence to reporting illness is required to be available.
      • Handwash signage reminding employees to wash hands is not available at 2 handsinks. Signage is required to be posted.
      • Facility serves burgers cooked to order, including undercooked burgers. A consumer advisory needs to be provided.
      • Several areas of the kitchen are noted in need of maintenance. Broken ceiling tiles need to be replaced, malfunctioning light bulbs need to be replaced, missing floor tile by grease trap needs to be replaced.

      • The first hand sink on the left when entering the kitchen is blocked with a chair, brooms, and buckets in sink all hand sink, hand sinks should be available for employees use at all times.
