Dirt In Ice Machine, Improper Food Dating: Manassas Restaurant Inspections
Health inspectors found foods using "prepared on" dates from the future and dozens of other violations at a recent inspection.
MANASSAS, VA — Some restaurants in the Manassas area recorded violations during recent health department inspections between July 19 and 31.
Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.
For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- A review of the menu with the foodservice operator indicates that there is no consumer advisory for the eggs (any style) that may be served raw and/or undercooked.
- There was no temperature measuring device located in the glass hot holding equipment.
- Unlabeled food containers with the following food items that are not easily identified by appearance: salt and oil.
- Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
- The ice machine is not in good repairs, is not working.
- Multiple light ceiling bulbs out at the back tortilla preparation area.
- Less than 50 foot candles of light was noted in the back tortilla preparation area (28 fc recorded).
- Less than 10-foot candles of light were noted in the walk-in cooler, (3.5 fc measured).

On the sushi side 3 compartment sink did not have a stoppers to be able to hold sanitizer in the 3rd basin.
- A container of cooked white rice was kept on the counter near the stove without properly being held in hot holding.
- In the sushi case there was raw salmon (48F) and raw tuna steaks (51F).
- In the 2 door glass coca cola reach in cooler there was a several big containers with the lid of cut cabbage, cooked noodles, cut tomatoes that were prepared about 45mins before the inspection that were cooling.
- There were packs of raw Reduced oxygen packaging fish in 2 door reach in coolers and prep units completely thawed.
- In several cold holding units they were missing ambient air thermometers.
- The faucet at the hand sink in the kitchen was leaking.

- The Person In Charge (PIC) was not able to provide documentation that all food employees (120) or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
- A food employee failed to wash his hands when changing tasks during the ROP process; a food employee failed to wash his hands prior to donning gloves to handle ready to eat (RTE) foods (tomatoes); several food employees failed to wash their hands prior to donning gloves due to wearing artificial nails; several food employees failed to remove their watches prior to washing their hands.
- The following handwashing sinks were found to have hot water at less than 100 degrees F: *handwashing sink in back, near dessert prep area.
- Raw shrimp, calimari, chicken Parmigiana and broccoli were found uncovered in the 3 door reach-in; cut lettuce and sliced tomatoes were found uncovered on top of the salad prep reach-in refrigerator (no cover at all).
- The food thermometer used in the ROP process was observed to be dropped on the floor, then rinsed off in the prep sink and sanitized in the wet wiping cloth bucket. The single thermometer was being used to evaluate foods being reheated in bags as well as in foods being cooked without proper washing/rinsing/sanitizing in between.
- Ground pork sausage, cooked from raw earlier this morning (<10:30am), found covered in walk-in cooler at 120 degrees F at 12:30pm.
- Containers of food, including marinara, 5 cheese marinara, Alfredo base, Pasta e Fagioli and Chicken & Gnocchi to be held for <7 days using ROP were not properly date marked, some were marked with "prepared on" dates in the future.

- The Certified Food Protection manager failed to complete and maintain the following records required by the HACCP Plan: 1. time/temperature parameters during cooling down of products 2. time/temperature parameters during reheating of products 3. time/temperature parameters during hot holding of products 4. walk-in refrigeration temperatures, taken manually, twice daily 5. corrective actions taken if there was a failure at any critical control point during cooking, cooling, cold holding (WIC), reheating or hot holding.
- Various foods such as cooked ground sausage in the WIC and ready to eat sliced tomatoes in the salad prep reach-in refrigerator found to be cooled improperly using deep and/or covered containers.
- The following cold/hot holding equipment had thermometers that were in disrepair: 1. 3 door reach-in refrigerator 2. hot holding cabinet in the ROP area.
- There were no temperature measuring devices (thermometers) in most of the reach-in refrigerators or in the hot holding equipment.
- Foods being prepared/stored in the desserts area and in the ROP area are subject to contamination from the closely adjacent handwashing sinks.
- Several employees were observed to be wearing artificial nails when working with exposed food.
- Employees observed wearing jewelry (watch) on their arms and hands while preparing food.
- Employees handling food were observed to not be wearing proper hair restraints.
- The wet wiping cloths at the salad prep reach-in refrigerator were not properly stored in the sanitizer bucket in between use. Sanitizer buckets were stored on the floor along the cookline.
- In use utensils along the cookline were stored in containers with running water that did not have sufficient water velocity to flush food particles to the drain.
- The cutting board being used as an additional splash guard along the handwashing sink in the ROP area is not properly designed for this use.
- The temperature dial on the hot holding cabinet in the ROP area is in disrepair.
- There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solution (quaternary ammonium) used at the 3 compartment sink.
- The sprayer arm at the scraping sink at the warewashing machine is in disrepair and leaking severely.
- The employee's toilet room doors and floors are not cleaned as often as necessary to keep them clean. The floor in the walk-in cooler was covered with water, but per the CFPM had not been cleaned recently.
- The doors to the employee toilet rooms are in disrepair and not easily cleanable.
