Manassas Health Inspections: 20 Violations At 1 Facility
Several restaurants around the Manassas area recorded violations during recent inspections from the county health department.
MANASSAS, VA — Several facilities in the Manassas area recorded violations during recent health department inspections between Dec. 13 and Dec. 21.
COVID-19 restrictions have mostly disappeared around the state. Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- Employee was scrapping cooked pasta with bare hands into a container.
- The hand sink in the kitchen did not have a hand washing sign.
- There was a not in use pressure cooker that was not in clean to the sight and touch.
- There were several commercially prepared food items in the 2 door true reach in cooler that were out of date per manufactures packaging.
- An employee was observed working with food without a proper hair restraint on.
- The stationary under sink dishwasher was unable to verify if proper sanitization could be done.

- There were hand washing signs missing at a hand sink in the kitchen and from the bathrooms in the facility used by kitchen staff members.
- There were no irreversible test strip indicators for the high temp dish machine in the facility.
- There was no covered receptacle in the bathrooms for sanitary napkins.

- Various bulk food containers were not labeled properly with the common name of the foods stored within them.

- The person in charge (PIC) is not able to demonstrate that systems to control risks to food safety have been put into practice.
- Employees or applicants are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
- Employees are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food.
- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- The handwashing station at the rear of kitchen (by prep table1) is being used for purposes other than washing hands. Observed dishes and various rags in sink.
- Inadequate record keeping system of Shellstock Tags. No system at all.
- The identity of the source of shellstock are not maintained by retaining their tags or labels. Observed oysters in large unmarked bin with 0 record keeping/tags with oysters.
- Raw animal food stored over ready-to-eat food in the refrigeration unit. Raw eggs stored over cooked soup and raw RTE foods and vegetables including corn and cucumber.
- ICE in ice machine is in contact with mold/mildew. Food shall only contact surfaces of: 1. Equipment and utensils that are cleaned as specified under 12VAC5-421-1770 through 12VAC5-421-1860 and sanitized as specified under 12VAC5-421-1885, 12VAC5-421-1890 and 12VAC5-421-1900
- No original container for the stored Shellfish in Walk-in-Refrigerator.

- Shelving next to Prep-station 1 has un-treated/sealed wood wrapped in foil set as second shelf.
- Cutting boards observed discolored and in need of resurfacing/replacement.
- There is no chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration.
- Various shelving covered with foil. Foil is creased and covered in food contaminates/oil/grease/dust/dirt.
- Walk-in-Refrigerator door does not seal.
- Ceiling tiles are not kitchen tiles. They are absorbent and covered/coated with contamination/grease/oil/dirt/dust.

- Handwashing was blocked in the kitchen.
- Raw chicken was stored above pepper in one door upright freezer.

- A date marking system is not in proper use for ready-to-eat (RTE) pre cooked rice in the 3 door refrigeration unit.

- Duties of the PIC: The PIC shall demonstrate Active Managerial Control, through discussion by the EHS and observation that the systems or controls area actually being implemented. Failure of PIC to ensure employees are complying with the duties for employee health, hand hygiene, proper equipment sanitation, food safety, along with facility maintenance and repairs.
- Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy."
- The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- Observed that hand soap was not provided at each handwashing sink.
- No disposable towels were provided at the hand washing sink located in the food prep area.
- Raw animal food holding in a manner that may cause cross contamination of ready-to-eat foods (RTE). Observed raw beef stored on a shelf above ready to eat desserts.
- The food-contact equipment surface of the clean in place dough mixer is observed soiled with accumulations of grime and debris.
- The date marking system for prepared ready-to-eat (RTE) foods in the refrigeration unit is not in proper use. Observed no labeling system for all foods.
- Employees' medicines, first aid supplies, and other flammable liquid containers are not properly located to prevent contamination of food and food contact surfaces.
- No ambient (air/water) temperature measuring device was located in the walk-in cooler.
- Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: inside reach in cooling units, shelves inside walk in cooler, walls around food prep and ware washing areas, and floors.
- Observed a badly leaking faucet at the 3-compartment sink.
- The WALLS and CEILING SURFACEs are not smooth, durable, easily cleanable, and/or nonabsorbent. Observed multiple holes in both the ceiling and the walls throughout the establishment.
- Observed that mops are improperly stored between use.
- The floors are not maintained in good repair with several areas of broken tile.
- Observed several areas where bottled beverages, bulk food ingredients, and other materials are stacked directly on the floor for storage.
- The area designated for employees to eat, drink, and/or conduct business is not located so that food, equipment, and single-service and single-use articles are protected from contamination.
- The light intensity is below 10-foot candles in the dry storage and mop sink areas.
