Manassas Health Inspections: 23 Violations At 1 Restaurant

The Prince William Health District recorded violations at several facilities around Manassas and Gainesville during recent inspections.

MANASSAS, VA — Several facilities in the Manassas area recorded violations during recent health department inspections between Feb. 22 and Feb. 28.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Buffalo Wild Wings #345, 10061 Market Circle Dr Manassas, VA 20110
    • Date inspected: Feb. 22
    • Total violations: 5
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Water from the handwashing sinks located in the bar area was measured at a temperature less than 100°F.
      • No waste receptacle provided at handwashing sink lavatories in the kitchen, food preparation and cooking area.
      • Employees observed working in the food service area without proper hair restraints.
      • The handwashing sink faucets one across the ice machine and one beside 3 compartment sink are not in good repairs, the faucets are missing the hot water handle.

      • The loaded potato soup was at 45F in the walk in cooler.
  • Benjin Asian Bistro, 9110 Devlin Road 130 Bristow, VA 20136

      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • This food establishment is using a noncontinuous cooking process for raw animal foods (chicken) and has not submitted written procedures to or obtained approval from the health department.
      • No date marking system observed for prepared raw animal food in walk in cooler.
      • Containers of hand sanitizer are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.
      • Both walk in cooler and walk in freezer were overstocked and is not sufficient in number or capacity to meet the food storage demands of the establishment. Observed large volume of food stored on the floor.
      • Outer opening of the food establishment is not protected against entry of insects and rodents. Observed broken screen door with holes.
      • Wiping cloths improperly stored between use. Observed wet wiping cloths stored on clean prep surfaces.
      • In-use utensils (ice scoop) observed improperly stored.
      • Manufacturer containers (#10 cans) were observed reused for the storage of sauces at grill area.
      • Observed a slow draining hand sink.

    • Date inspected: Feb. 22
    • Total violations: 21
      • There were cups, personal food items, and others resting above the prep station in the kitchen.
      • There were no hand washing signs at any of the hand washing sinks in the kitchen.
      • The hand washing sink was used for dumping, there were onion peels in the drain.
      • The SSCS Dishwasher tested greater than 200ppm.
      • The cutting boards were heavily scratched and scored, no longer easily cleanable.
      • Chicken in the Cres Corr was reheated to 155.
      • There was no Consumer Advisory on the menu next to the burgers that could be served undercooked.
      • There was chemical substances observed above and on the same shelf as food contact surfaces.
      • There was a container of medication and medical supplies above ingredients and food items in the kitchen.
      • There was a container of diced tomatoes and potato salad in the sandwich prep station that were prepped this morning and placed in there without proper cooling the items.
      • There were several food containers that did not bare the name of the food item that was being kept inside.
      • There was a multilevel tray with food on it next to the hand sink.
      • An employee was observed with a bandage on her finger with no gloves on while handling food.
      • Several employees working in the kitchen with food were observed without hair restraints on.
      • There were several wet cloths around the kitchen that were not in sanitizer solutions.
      • Around the facility single use white containers with lids were being used to store food items.
      • There were cardboard boxes used to store ingredients (not original packaging, cardboard lining shelves.)
      • The 3 compartment sink spray nozzle was wrapped in string to keep it up, knob on the hot holding unit was no longer there, True 3 door sandwich prep unit handles were broken on the top and the top inside panel was hanging off.
      • There was no chemical sanitizer hooked up at the 3 compartment sink.
      • The wire shelving was covered in grease and debris.
      • The Fire suppression system and the hood system were significantly out of date and had a build up of grease.
  • Pho Nucuoi Saigon, 8130 Sudley Rd. Manassas, VA 20109
    • Date inspected: Feb. 22
    • Total violations: 6
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • The handwashing sink was not working in the kitchen across the prep unit.
      • There was no food temperature measuring devices.
      • Various bulk food containers were not labeled properly with the common name of the foods stored within them.
      • Employees observed working in the food service area without proper hair restraints.

    • Total violations: 2
      • Hot water was observed at 108*F which is an inadequate temperature observed at the warewashing sink.
      • Observed large ice buildup in reach in freezer. Not in good repair.
  • Pizza Hut #37456, 6896 Piedmont Center Plz Gainesville, VA 20155
    • Date inspected: Feb. 27
    • Total violations: 4
      • There was foil and cardboard lining shelves.
      • There was no irreversible temp indicators available.
      • The 3 compartment sink basin indicated by sign as sanitize, pipes are clogging and leaking.
      • The walls in the kitchen area are heavily covered in grease.
  • Domino's Pizza #4228, 7625 Linton Hall Rd Gainesville, VA 20155
    • Date inspected: Feb. 27
    • Total violations: 9
      • There was no verifiable employee health policy was not available at the time of the inspection.
      • The back hand sink did not have paper towels, papers towels in the bathroom were being held on the back of the toilet.
      • The back hand sink was missing a hand washing sign.
      • The cutting board was heavily scratched and scored and no longer easily cleanable.
      • There were chemicals above the prep table near the 3 compartment sink.
      • The 3 compartment sink was not able to hold water without adding plastic bags to the drain stoppers.
      • The spray nozzle at the 3 compartment sink was hanging below the top of the basin.
      • The 3 compartment sink was leaking from the faucet.
      • An employee was using her phone in the food prep area and laying it on the cutting board.

      • There were tops to pans resting in the basin of the hand sink.
      • The sanitizer line at the 3 compartment sink was not properly installed/clogged and not feeding sanitizer.
      • The Heavy Cream was temp at 50F in the drink prep station.
      • Wiping cloths; use limitation. B. Cloths in-use for wiping counters and other equipment surfaces shall be: 1. Held between uses in a chemical sanitizer solution at a concentration specified in 12VAC5-421-3380.
      • There were 2 cups of room temperature water holding in-use utensils in them.

    • Total violations: 6
      • Raw chicken stored over/adjacent to pre-cooked steak in walk-in cooler (WIC).
      • Spray bottle of Purnell surface cleaner hanging on shelving with foods (rice, onions).
      • Guacamole, just made this morning, was found stored in covered containers (and also with cling wrap covering it directly) in the walk-in cooler at 48-50 degrees F.
      • The thermometer in the Continental 2 door reach-in refrigerator was in disrepair.
      • Single service paper bowl being continuously used to scoop tortilla chips.
      • An extension (single service foil container) has been placed on the original wastewater pipe of the 1 compartment prep sink so that it now extends down into the floor drain, eliminating the air gap.

    • Date inspected: Feb. 28
    • Total violations: 23
      • Duties of the PIC: The PIC shall demonstrate Active Managerial Control, through discussion by the EHS and observation that the systems or controls area actually being implemented. Failure of PIC to ensure employees are complying with the following duties: Protecting food from contamination; Properly cold holding of RTE TCS foods; Proper Cooling methods & procedures: Proper hand washing: Proper food contact equipment and utensil sanitation.
      • Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: No written policy exists.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed that the sign that notifies food employees to wash their hands is not clearly visible at several hand sinks.
      • Observed that paper towels were not provided at each handwashing sink.
      • Soap was not provided at the hand washing sink in the mens bathroom.
      • The handwashing facility located at the prep area is blocked, preventing access by employees for easy handwashing.
      • Observed that paper towels were not provided at each handwashing sink.
      • Water from the handwashing sink at front prep table was measured at a temperature less than 100°F.
      • The food-contact equipment surface of the cooking pans is observed soiled with accumulations of grime and debris.
      • POTENTIALLY HAZARDOUS PREPARED FOODS (STEW, BEANS & LAMB) noted not being adequately cooled to prevent the growth of harmful bacteria. All prepared foods placed in walk in cooler after cooking with lids preventing heat escape.
      • Cut tomato and lettuce in the top of the prep cooler cold holding at improper temperatures.
      • No date marking system exists.
      • Food containers for cooling food are arranged so as not allow for maximum heat transfer.
      • Walk in cooler was overstocked and is not sufficient in number or capacity to meet the food storage demands of the establishment. Raw animal foods may not be stored on the floor of the walk-in freezer.
      • The certified food manager could not provide a small diameter food temperature measuring device.
      • There was no temperature measuring device located in the cold holding equipment.
      • Unlabeled large bulk food containers located on the floor.
      • Observed Rice cooker warmer in operation on the floor.
      • There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
      • There is no refuse container at the area immediately adjacent to the several hand sinks.
      • Absorbent materials (Cardboard) are used on the floors at the grill, walk in cooler and shelving areas.
      • Less than 10-foot candles of light were noted above the gyro broiler. Less than 20-foot candles of light were observed above the warewashing sink.
