Manassas Health Inspections: 29 Violations At 1 Restaurant

The Prince William Health District recorded violations at several facilities around Manassas during a recent run of restaurant inspections.

MANASSAS, VA — Several Manassas area restaurants recorded violations during inspections between Aug. 16 and Aug. 23, according to the Prince William Health District.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • Greater Mount Calvary Christian Church, 9514 Westmoreland Ave Manassas, VA 20110
    • Date inspected: Aug. 17
    • Total violations: 4
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed several unapproved ( not smooth and easily cleanable) stained kitchen ceiling tiles.
  • Lake Manassas Academy, 14930 Holly Knoll Ln. Gainesville, VA 20155
    • Date inspected: Aug. 17
    • Total violations: 3
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • No hand drying provisions were provided at the hand washing sink in the men's restroom(Corrected during the inspection : The person in charge placed the paper towel in the men's restroom next to the hand washing sink).
      • Hot water is not available in sufficient quantities to meet the use demands of the food establishment. Observed the insufficient hot water temperature at the hand washing sink located between the white refrigerator and the 3 compartment sink. The hot water at the hand washing sink was measured at at 88 F, 89 F and 90 F, by the EHS and the certified food protection manager using calibrated food thermometers.
  • Cactus Grill, 8099 Sudley Road Manassas, VA 20109
    • Date inspected: Aug. 18
    • Total violations: 2
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
  • 90 Degrees (90 Grados) Restaurant, 8509 Rixlew Ln Manassas, VA 20109
    • Date inspected: Aug. 18
    • Total violations: 29
      • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
      • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissible through food.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • A food employee was observed cleaning their hands in the 3VAT sink that is not designated as a handwashing sink.
      • There is no handwashing sink located beside 3VAT sink, or prep area - preventing routine handwashing by food workers. Previously approved hand-sink was "removed" and no hand-sink was replaced.
      • Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
      • Water from the handwashing sink at male's toilet room handwashing sink was measured at ( 72°F) a temperature less than 100°F.
      • The tags for the clamps and mussels are not available or are discarded immediately after the container is empty.
      • There is no written agreement or statement from the supplier or aquaculturist stipulating that the fish, raw-marinated, partially cooked, or marinated-partially cooked fish served or sold in ready-to-eat form, is aquacultured and/or the records are not retained in the food establishment for 90 calendar days beyond the time of service or sale of the fish.
      • The identity of the source of mussels are not maintained by retaining their tags or labels.
      • Raw animal food holding in a manner that may cause cross contamination of to ready-to-eat food (RTE). Uncovered container of raw beef stored above RTE foods (cabbage/other veg).
      • Raw animal food stored over ready-to-eat food in the refrigeration unit. Unpasteurized eggs being stored on shelving above ready to eat foods (greens & tomatoes).
      • The following equipment food-contact surfaces were observed soiled to sight and touch: cutting boards.
      • The raw and/or undercooked foods (raw tilapia, eggs) provided on the menu, menu board, table tent or brochure do not provide a disclosure statement.
      • Original containers of "bleach" does not have a legible manufacturer label.
      • Prep-cooler is not maintaining time/temperature control for safety (TCS) foods at 41°F or below.
      • Unlabeled food containers with the following food items that are not easily identified by appearance: Sugar/flour.
      • Linens (table cloths) found used in direct contact with the following food item(s): Pupusas making ingredients (dough).
      • Observed kitchen prep station/areas positioned on patio/outside seating area. According to the person in charge, food is been prepared and served preparation at this this location. Food is subject to environmental sources of contamination.
      • Large bag of onions and bags of portioned, frozen meat being stored on floor next to mop sink.
      • Clean equipment/utensils on shelves were found stored next to the mop sink.
      • Single-service items (5gallon bucket) were observed reused for the storage of "red sauce" and "slaw".
      • The nonfood contact surface of the wooden table and wood backing that is being used to hang pots/pans 3 compartment sink are moist absorbent and not designed or constructed to be easily cleanable.
      • Observed a hose attached to a faucet that extended below the flood rim level of the mop sink. The hose also has a hole in it.
      • The faucets in the mens bathroom are all broken. 2 faucets are missing entirely.
      • All the exhaust hood system light bulbs were not working.
      • The ceiling tiles next to the hood system are not cleaned as often as necessary to keep them clean.
      • The light intensity is below 10 foot candles in the grill and food preparation area.
      • Lockers or other suitable facilities are not provided for employees clothing and other possessions.
  • El Quetzal Grill, 8450 Centreville Rd Manassas Park, VA 20111
    • Date inspected: Aug. 23
    • Total violations: 10
      • Employees and conditional employees are not aware of the reporting procedures concerning information about their health as they relate to the symptoms caused by illness, infection, or other source that is associated with an acute gastrointestinal illness or a lesion.
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Stored food is not protected from cross contamination from food that is damaged, spoiled, or recalled. Observed multiple containers of food in cold holding not protected with a lid.
      • Observed unsuitable small containers for dispensing foods from the reach in cooler. Us of a handled utensil prevents hands and fingers from accidently coming in contact with foods.
      • The food-contact surface of the cutting boards is not cleaned at an appropriate frequency.
      • TCS FOOD resting in a cold holding ice bath at improper temperatures. Observed Pico Degalo, Lettuce, and corn salad at improper temperatures.
      • The date marking system for prepared ready-to-eat foods in the refrigeration unit is not in proper use.
      • The ambient air and water temperature measuring device that is scaled in Celsius or dually scaled in Celsius and Fahrenheit located in the prep cooler is not accurate and/or easily readable.
      • Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses are not stored off the floor and/or in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles.
      • The kitchen FLOOR is not smooth, durable, easily cleanable, and/or nonabsorbent. Observed multiple broken floor tiles.
