Manassas Health Inspections: 36 Violations At 1 Restaurant

The Prince William Health District inspected several Manassas restaurants, including one that recorded 36 violations.

MANASSAS, VA — A Manassas restaurant recorded 36 violations during a recent inspection from the Prince William Health District. The most recent round of inspections occurred between Oct. 12 and Oct. 18.

Most COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

  • McDonald's, 8203 Sudley Rd Manassas, VA 20109
    • Date inspected: Oct. 12
    • Total violations: 9
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed in the reach in cooler raw animal food (shell eggs) holding in a manner that may cause cross contamination of (pasteurized liquid eggs) ready-to-eat food (RTE) with an open container.
      • Chlorine sanitizing solution used was not at an acceptable concentration.
      • Sliced cheese cold holding at improper temperatures.
      • CPTM did not provide written procedures for the use of time as a public health control for TCS foods. Observed American cheese slices in prep area not being maintained in cold holding temperature.
      • Food stored in a location where it is subject to splash, dust or other contamination as follows: Observed large rolling container of trash placed against the rack of liquid soda syrups.
      • Observed y valve in closed position after air breaker/backflow devise with mop sink faucet water in on position. Potential failure of the vacuum backflow devise could occur under these conditions.
      • Observed that the walk in freezer/cooler is not maintained in good repair. Observed ice/frost build up at the door seal.
      • Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
  • Thai Tyme, 10167 Hastings Dr. Manassas, VA 20110
    • Date inspected: Oct. 13
    • Total violations: 36
      • Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: training has not occurred within the past year.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
      • Observed food employees using improper handwashing procedures: returning from the bathroom while still wearing food protection gloves.
      • The handwashing facility located at the rear of the kitchen is blocked by a dining table, preventing access by employees for easy handwashing.
      • The handwashing station at the front of the kitchen is being used for purposes other than washing hands.
      • Single-use gloves used for more than one task, are damaged, soiled, or saved for re-use.
      • Raw animal food holding (shell eggs) in a manner that may cause cross contamination of to ready-to-eat food (RTE uncovered noodles).
      • Food contaminated by food employees by dispensing ingredients via food storage bowls.
      • The food-contact equipment surface of the ice machine is observed soiled with accumulations of grime and debris.
      • Chicken on a stick noted not being adequately cooled to prevent the growth of harmful bacteria.
      • Rice on prep table hot holding at improper temperatures.
      • Pork dumplings observed cold holding at improper temperatures.
      • Employees' medicines are not properly located to prevent contamination of food and food contact surfaces.
      • Poisonous or toxic materials found stored or displayed with food, equipment, linens or single service items in such a way as to contaminate items.
      • The methods used for cooling garden salad with lettuce were not adequate.
      • The certified food manager could not provide a small diameter food temperature measuring device.
      • There was no ambient temperature measuring device located in the reach in prep cooler (right).
      • Insufficient information is provided on the label of the following food item(s) packaged in the food establishment: fried onions
      • Outer opening of the food establishment is not protected against entry of insects and rodents. Observed gap at the top of the screen door.
      • Dead or trapped insects observed in overhead light shields.
      • Food is subject to environmental sources of contamination during preparation. Observed a dirty table top fan on prep table.
      • Wiping cloths improperly stored between use.
      • In-use utensils improperly stored between use. Tongs used at the grill area observed on oven handle door where they could come in contact with employee aprons.
      • In-use utensils improperly stored between use. Scoops for rice cooker kept in a container of water at room temperature.
      • In-use utensils improperly stored between use. Observed knife wedged between prep tables.
      • Single-service items were observed reused for the storage of other foods.
      • The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: temporary shield attached to reach in freezer next to front hand sink.
      • There is no refuse container at the area immediately adjacent to the rear hand sink.
      • The refuse container located outside the establishment observed with open doors.
      • The walk in cooler is not maintained in good repair.
      • The walk in cooler and dry storage area are not cleaned as often as necessary to keep them clean due to foods and supplies being stored on the floor.
      • Wet mop is observed soiled and requires replacement.
      • Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials.
      • Observed that the area designated for employees to eat, drink, and use tobacco is not located to protect food, equipment, linens, and/or single-service items from contamination.
      • Lockers or other suitable facilities are not provided for employees clothing and other possessions. Observed cell phone on food prep table.
  • Logan's Roadhouse, 7731 Donegan Dr Manassas, VA 20110
    • Date inspected: Oct. 13
    • Total violations: 17
      • The person in charge failed to describe the relationship between the prevention of foodborne illness and the management and control of the following: (4) Hand washing, (7) Properly cooking raw animal foods, (8) Properly cooling foods , ((11) Preventing cross contamination during utensil use and (15) Employee health policy training undocumented.
      • Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: CFPM could not provide written documentation of employee health policy training.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • The dishwasher employee was observed with food and drink (on top of the ware washing machine) in an area other than designated break area while preparing/handling food or contacting clean food contact surfaces.
      • A food employee (Dishwasher) failed to wash her hands after handling soiled utensils. Observed putting away clean pans after loading dirty dishes without hand washing and changing gloves.
      • No hand drying provisions were provided at the handwashing sink in the food preparation area.
      • Observed (Fish) Raw animal food holding in a manner that may cause cross contamination of to (Ribs) ready-to-eat food (RTE) in walk-in cooler.
      • Observed operation of the mechanical warewashing machine, equipment using heat below 160*F, food-contact surfaces and utensils are not being sanitized after cleaning.
      • Observed (chicken breast poultry) item was not cooked to a sufficient temperature and/or time to eliminate pathogenic bacteria.
      • Observed tomatoes @ prep table, lettuce @ prep table, and sirloin @ prep drawer cold holding at improper temperatures.
      • In the warewashing area, observed a working container of a purple cleaning fluid not properly labeled with the common name.
      • Methods used for cooling brown gravy in walk in cooler were not adequate.
      • Vent cover over the deep fryer was observed in a state of disrepair and condition that prevents necessary maintenance and easy cleaning.
      • There is no data plate on the warewashing machine.
      • Observed a "Y" valve with the faucet water handle in the on position supplying water to the Ecolab cleaning product dispenser therefore applying pressure against the vacuum breaker risking water contamination within the facility.
      • Observed that the chemical storage area is in need of cleaning and repair.
      • Lockers or other suitable facilities are not provided for employees clothing and other possessions. Observed dishwasher employee rested their coat and back pack on top of ware washing machine.
