Manassas Restaurant Inspections: McDonald's, Zam Zam Kabob House
The Prince William Health District recorded violations at several facilities around Manassas during recent restaurant inspections.
MANASSAS, VA — During recent inspections, the Prince William Health District recorded violations at daycares, schools, and restaurants around the Manassas area. The most recently published inspections occurred between June 8 and June 14.
Many COVID-19 restrictions have relaxed around the state, and health inspectors are returning to conduct many restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.
Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

- Total violations: 2
- Spicy chicken patties hot holding (128°F/130°F) at improper temperatures.
- Methods used for cooling sliced tomatoes, cut lettuces and cut watermelons were not adequate.
- El Vaquero West III, 14910 Washington St Haymarket, VA 20169
- Date inspected: June 10
- Total violations: 9
- No disposable towels were provided a the hand washing lavatory in the kitchen next to the prep table.
- Raw animal food stored over ready-to-eat food in the refrigeration unit.
- Unwrapped or uncovered food such as salsa and cooked pork meat in the walk in cooler.
- Food stored on the floor and/or food stored less than 6 inches off the floor.
- Employees observed working in the food service area without proper hair restraints ( Corrected during the inspection).
- Wiping cloths improperly stored between use, not in chemical sanitizing solution.
- Forks and knives were observed stored unprotected at the the bar desk in the front of the restaurant.
- Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
- Observed that the wall, the floor and doors of the walk in cooler in the kitchen are in need of cleaning.

- The soap dispenser for the handwashing sink is affixed to the wall above the clean equipment drainboard (right side) of the 3 compartment sink subjecting the clean equipment to contamination by splash.
- Waste lines (one with an extension) extend down into the floor drain, eliminating the air gaps.
- Enclosure for refuse, recyclables, or returnables not maintained in good repair.
- The wall, wastewater lines, floor drain and floor under the 3 compartment sink are unclean.

- Raw animal food ( ground beef) holding in a manner (next to hotdogs in the 1 door refrigerator) that may cause cross contamination of to ready-to-eat food (RTE).

- Unwrapped or uncovered food in the 2 door refrigerator such as raw chicken, pico de gallo, and raw beef slices.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Sliced tomatoes 48 F, sliced cheese 49 F, shredded cheese 49 F, Cooked Ham 45 F, Mozzarella Cheese 45 F. (Corrected during the inspection : The person in charge placed the food in the draw cooler for proper cold holding).
- The date marking system for prepared ready-to-eat (RTE : cooked chicken, pico de gallo, cooked sliced beef) in the refrigeration unit is not in proper use.
- The physical facilities such as the floor, mop sink are not cleaned as often as necessary to keep them clean.

- This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
- No disposable towels were provided a the hand washing lavatory next to the prep table.
- Different types of raw animal foods stored in such a manner that may cause cross contamination such as raw lamb stored on the top of raw chicken on the same shelve.
- Unwrapped or uncovered food such as cooked spicy chicken, chic peas, cooked basmati rice, chicken kabob, chicken karihi sauce, cooked butter chicken in the walk in cooler.
- TCS FOOD cold holding at improper temperatures : Cut melon, raw eggs, cooked rice and tortilla stored in a broken 2 door refrigerator and measured at 45 F each. (Corrected during the inspection :The person in charge removed the food and placed them in the walk in cooler for proper cold holding).
- Chic peas, cooked rice, chicken karahi, butter chicken prepared and held in the food establishment for more than 24 hours is not clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded.( Corrected during the inspection : the person in charge date make the food prepared 14 hours ago for proper disposition).
- Food such as barihani sauce, creamy tomato sauce and cooked potatoes were observed stored less than 6" above the floor. (Correct during the inspection : The person in charge removed the food from the floor and placed it on the shelves).
- Wiping cloths improperly stored between use.
- The cooking stove, the hood on top of the stove, the fryer, the 2 door refrigerator, the dish washing machine, the handle door of the walk in freezer and the freezer are not maintained in good repair
- Physical structure such the floor, the mop sink, the floor in the freezer noted in need of cleaning
- Observed an accumulation of litter on the premises.

- This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
- Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at the handwashing sinks in the restrooms.
- Previously used yellow lexan hot holding trays used to store foods such as eggs, sausages, steaks, etc were stacked along the cookline and observed to be reused for the folded eggs.
- Various pieces of equipment (plastic lids, holding trays) on clean equipment shelving unclean by site. 2. Hood filters were brought to the mop sink for cleaning. When stopped, the employee attempted to return them to the hood area for installation.
- The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cut lettuce on the prep reach-in.
- Chemicals (sanitizers) stored near single service coffee filters and coffee; spray bottle of sanitizer stored hanging (and aimed towards) bottles of syrups along front line.
- There was no ambient air temperature measuring device (thermometer) in the 1 door reach-in freezer.
- Food (boxes of hamburgers, chicken, ice) stored on the floor in the walk-in freezer.
- Clean equipment stored stacked and wet on the clean equipment storage shelf.
- Single service happy meal boxes were stored on the floor in the employee break room..
- Multiplex drink refrigerator is in disrepair with the handles broken..
- The maximum temperature of the hot available at the 3 compartment sink was measured at 100 degrees F, therefore the required wash temperature of 110 degrees F can not be obtained.
- The faucet on the righthand side of the 3 compartment sink doesn't allow the cold water to be shut off completely.
- There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. One hose was observed to be fully extended into the mop sink. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
- The wastewater pipes of the 3 compartment sink extend down into the floor drain, eliminating the air gaps.
- Litter was stored adjacent to the outside waste dumpster compactor; an abandoned fryer unit is also stored outside of the facility.
- The dumpster/compactor is in disrepair and the safely door can not be closed properly.
- The urinal in the men's restroom is in disrepair.
