Woodbridge Health Inspections: 16 Violations At 1 Facility

Several facilities in Woodbridge recorded violations during the most recent round of inspections from the county health department.

WOODBRIDGE, VA — Several facilities recorded violations during recent inspections from Prince William County's health department. The most recent inspections occurred between Dec. 6 and Dec. 12.

COVID-19 restrictions have disappeared around the state. Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

    • Date inspected: Dec. 8
    • Total violations: 1
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • Pizza Hut #37446, 12539 Dillingham Sq Woodbridge, VA 22192
    • Date inspected: Dec. 8
    • Total violations: 7
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • Boxes of foods stored in the walk-in freezer were observed to be open and foods uncovered.
      • Plastic wrap was being used to cover the upper shelving of the food prep station up front.
      • The shelving used for storing clean equipment was observed to be soiled/sticky to touch.
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses.
      • An extension has been placed on the floor drain so that the waste water pipes from the 3 compartment sink now extend down into the floor drain, eliminating the air gaps.
      • Light bulb in walk-in cooler is burnt out.

    • Date inspected: Dec. 11
    • Total violations: 4
      • Bottle of water was stored (on the food prep table) in a manner that may contaminate food, food contact surfaces or utensils.
      • Sliced tomatoes in covered lexan containers in the walk-in cooler found stored at 45 degrees F. Per staff, tomatoes were sliced 3 hours previously.
      • The wastewater pipe of the 3 compartment sink extends down into the floor drain, eliminating the air gap; the wastewater pipe from the ice machine is not air gapped either.
      • Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions such as phones, resulting in personal items being stored on top of prep table.

    • Date inspected: Dec. 11
    • Total violations: 16
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Raw shell eggs stored over ready to eat (RTE) juices in 3 door reach-in refrigerator; raw eggs and hamburger patties stored over or adjacent to RTE dressings, Red Bull drinks and beer in walk-in cooler (WIC).
      • The Jackson warewashing machine was not sanitizing properly based upon the final rinse gauge not registering the required 180 degrees F and the irreversible registering temperature indicator (thermolabel) not registering a surface temperature of 160 degrees F with a color change from white to black.
      • Cheese sauce @ 64°F being reheated in the hot holding unit.
      • Toxic chemical (grease cutter) stored near clean equipment (pizza pans) under flat top grill.
      • Foods (white granular) in bulk storage containers were not labeled properly with the common name of the food.
      • Food stored on the floor in the walk-in freezer (bags of fries) and in the dry storage area (bottles of fry oil).
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Jackson warewashing machine in disrepair and not sanitizing properly; Imperial range (stove top/oven) in disrepair and disconnected from gas supply.
      • Cardboard being used to line shelving in walk-in cooler and in dry storage area.
      • Grocery bags were used to store ready to eat foods (flat breads used for pizzas) in, in the Continental 2 door refrigerator.
      • An irreversible registering temperature indicator was not provided for the hot water mechanical ware washing operation.
      • A handwashing soap dispenser was observed to be installed over the sanitizer basin of 3 compartment sink.
      • The rinse temperature measuring device on the warewashing machine was in disrepair and not capable of indicating the rinse temperature as required.
      • Extensions (tape) have been placed on the original wastewater pipes of the 3 compartment sink so that they now extend down into the floor drain, eliminating the air gaps.
  • Pho Nu Cuoi Saigon, 2890 Dale Blvd Dale City, VA 22193
    • Date inspected: Dec. 12
    • Total violations: 6
      • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
      • Observed raw animal food such as raw eggs holding in a manner that may cause cross contamination of ready-to-eat (RTE) food in this case cooked pork.
      • The food-contact surface of the slicer is not cleaned at an appropriate frequency.
      • The date marking system for prepared ready-to-eat (RTE) Cooked pork in the refrigeration unit is not in proper use.
      • Observed the surfaces of the walk in cooler dirty and need cleaning.
      • The door of the walk in cooler is not maintained in good repair.

    • Total violations: 2
      • Hand sink opposite grill - Completely blocked with dishes and no longer accessible by employees.
      • Food bins with various powders/grains with scoops/cups submerged in food.
