Woodbridge Health Inspections: 18 Violations At 1 Facility

A restaurant and two schools around the Woodbridge area recorded violations during recent inspections from the county health department.

WOODBRIDGE, VA — Several facilities in the Woodbridge area recorded violations during recent health department inspections between Sept. 20 and Sept. 26.

COVID-19 restrictions have mostly disappeared around the state. Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

Here are the most recent inspections from the Woodbridge area, according to the Prince William Health District.

  • Leesylvania Elementary School, 15800 Neabsco Rd Woodbridge, VA 22191
    • Date inspected: Sept. 22
    • Total violations: 2
      • Tomatoes and lettuce that were just slice were found cooling in the Traulsen 2 door refrigerator on line A at 53 and 48 degrees, respectively (within 4 hour cooling period)
      • Box of cheese sticks found thawing on prep table at room temperature.
  • River Oaks Elementary School, 16950 McGuffeys Trail Woodbridge, VA 22191
    • Date inspected: Sept. 22
    • Total violations: 2
      • Tomatoes that were just sliced were found on serving lines #1 and #2 at 42 and 46 degrees F, respectively.
      • There is still a large build up of ice on the left side of the walk-in freezer beneath the fans.
  • Burger King# 9928, 16551 River Ridge Blvd Woodbridge, VA 22191
    • Date inspected: Sept. 22
    • Total violations: 18
      • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
      • This food establishment has not established written procedures for responding to vomiting or diarrheal events that happen in the facility.
      • Upon arrival in the kitchen a food employee at the sandwich assembly station was observed to be putting on gloves. When she and the Person In Charge (PIC) were asked what the proper procedures were in reference to glove wear and changing, it was determined that employees are not washing their hands prior to donning gloves.
      • No hand towels were available at hand sink near the 3 compartment sink.
      • The handwashing sink near the 3 compartment sink appears to being used as a dump sink and/or for cleaning equipment as there was food debris and the water was not draining from the sink basin (clogged).
      • Various containers of food in walk-in freezer stored open and exposed.
      • The 3 compartment sink was found to be unclean and not properly set up; no stoppers/plugs could be found to allow the compartments to hold water; and when asked, the PIC did not understand the proper set up of the 3 compartment sink. Equipment was not being washed/rinsed/sanitized properly.
      • Establishment is using TPHC for items such as cut tomatoes and lettuce. Per the chart on site, these items are discarded after 4 hours. Stickers were placed on the trays indicating when the sliced tomatoes and cut lettuce were to be discarded (2pm). However, when staff was asked at what time were the items placed on the counter, several answers were received indicating a total of time out of refrigeration of 8 hours, then 6 and finally 4 after the chart was consulted. There was no indication that the tomatoes had been properly cooled to 41 degrees and below after slicing as there were also sliced tomatoes, from this morning per staff, marked Friday, in the walk-in cooler in covered containers, at 45 degrees F.
      • Spray bottle of chemicals/toxics stored near single service plastic cups under counter in front service area.
      • Tomatoes, just sliced this morning, in covered container, in walk-in cooler at 45 degrees F.
      • The thermometer in the reach-in refrigerator in the front service area was in disrepair; there were no thermometers in either the chest freezer at the end of the grill or the one along the cook line; the integral thermometers on the walk-in cooler and walk-in freezer are in disrepair.
      • Employees handling food were observed to not be wearing proper hair restraints.
      • Paper cups in dispenser in front service area were stacked too high (spilling out of top of dispenser) to provide protection from hand contact.
      • The interior of the ice machine is in disrepair with parts hanging down into the ice machine.
      • There is a "Y" adapter on the faucet at the mop sink that allows 2 hoses to be attached. One hose was observed to be fully extended into the basin. Mop sink faucet vacuum breakers are not designed for the continuous pressure created by the "Y" adapter/hoses. There is a leak at the faucet as a result.
      • The wastewater pipes of the 3 compartment sink extend down into the floor drain, eliminating the air gaps.
      • Mop found in mop bucket of dirty water upon arrival.
      • Personal items such as purses, etc found stored near food and food contact equipment along cook line.
