Woodbridge Restaurant Inspections: Flies, 20 Citations At One Place

Health inspectors found as many as 20 violations during recent inspections in Woodbridge and eastern Prince William County.

WOODBRIDGE, VA — Some restaurants in the Woodbridge area recorded violations during recent health department inspections between April 11 and April 25.

Health inspectors conduct most restaurant and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection. Other violations are addressed later, and an inspector may conduct a follow-up inspection to ensure compliance.

Here are the most recent inspections from the Woodbridge area, according to the Prince William Health District.   • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • A food employee failed to wash his hands after putting away a food delivery and prior to making a pizza.
  • Corn meal in bulk container stored uncovered.
  • Insect control device is located over clean equipment storage in the back where dead insects may be impelled or fall.
  • Scoop for the corn meal was stored in the bulk food container with the handle down in the corn meal.
  • Adequate lockers or other suitable facilities are not provided for employees clothing and other possessions resulting in personal items being stored on the shelving used for food/single service/equipment storage (black bag stored with clean equipment and over marinara).

Violations:

  • Signage that notifies food employees to wash their hands was not provided and/or permanently affixed at all handwashing sinks used by food employees (signage at the handwashing sinks in the restrooms appears to have been removed recently).
  • The concentration of the quaternary ammonium solution was measured using the test kit at the 3 compartment sink and was found to be above the desired concentration at 500+ppm (toxic). This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
  • Cup dispensers up front are overstocked with cups that can be contaminated by handling them.
  • The hose attached to the mop sink faucet was observed to be fully extended into the basin thus creating a potential backflow issue.
  • The secondary door to the walk-in cooler (WIC) is in disrepair and taped shut with shiny metallic tape.

  • Bottles of water and oil used on food unlabeled.
  • Clean equipment was stored on the shelving with the food contact surfaces exposed to dust and other contamination.

  • Observed opened bags of biscuits on the top side of the cooking stove and on the prep cooler table.
  • The handwashing sink located next to the prep table is not maintained so that it is accessible at all times for employee use.
  • The following equipment food-contact surfaces were observed soiled to sight and touch: the can opener was observed soiled and needs cleaning.
  • The water temperature in the manifold servicing the high temperature warewashing machine was below the minimum allowable level of 180ºF.
  • Methods used for cooling Kim chi, glass noodles and cooked beef were not adequate. The food was measured at 58°F after 2 hours in the prep cooler.
  • Food stored on the floor or food stored less than 6" above the floor. Observed soy sauce stored on the floor in the kitchen and other foods stored on the shelves lees than 6 inches.
  • Wiping cloths improperly stored between use, not in chemical sanitizing solution.
  • In-use utensils improperly stored between use. Observed cutting knives store between the prep table and the cooking stove in the kitchen.
  • There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
  • Observed the wall behind the dishwasher, the cover of the ice machine and the floor in the kitchen dirty and in need of cleaning.

Red Robin Gourmet Burgers: 14090 Worth Ave Woodbridge, VA 22192

Violations:

  • There was no temperature measuring device located in the Hoshizaki 6 door prep reach in coolers in cook line in the kitchen.
  • Observed wiping cloths improperly stored between use on top of napkin dispenser at hand washing sink near cook line in the kitchen.
  • Observed knives stored in between prep tables near cook line in the kitchen.
  • The cutting board at the prep table are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
  • Observed door of the Curtis Walk in cooler in poor repair.
  • Observed faucet not sealed to hand washing sink near prep table in the kitchen.
  • There is no refuse container at the area immediately adjacent to the hand washing sinks in food preparation areas.

  • The person in charge is not able to provide a verbal or written policy for preventing the transmission of foodborne disease by an employee who has a disease or a medical condition.
  • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
  • Observed soiled knife stored on the shelf above prep cooler in sandwich production area.
  • Observed unlabeled seasoning shakers on counters in the sandwich production area.
  • Observed drain fly activity around drain underneath dishwasher across from the walk-in cooler and the walk-in freezer.
  • Employees wearing jewelry (watch) on their arms and hands while preparing food.
  • Observed scoop handle contacting cooked grilled chicken inside prep reach in cooler on sandwich production area.
  • The purple cutting board near the dry storage area was heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
  • The dry storage shelves near dishwasher area were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris.
  • An air gap between the water supply outlet and the flood rim level drain underneath the dishwasher is not observed.
  • Maintenance tools (brooms) were being stored near a hand washing sink and not stored in a way that prevents the contamination of food, equipment, utensils, linens, and single-service items.
  • Observed ladder stored in direct contact of ice machine near walk in cooler.

  • Wiping cloths improperly stored between use.
  • Observed in-use utensils and/or dispensing utensils observed improperly stored in water near hot holding unit in kitchen.
  • Observed scoop handle in direct contact with flour inside bulk bin in kitchen.
  • Single-service and/or single-use articles (portion cups) were being reused as scoops inside a bag of rice near storage shelves in kitchen.
  • There is no chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration.
  • Observed cardboard liners on storage shelves near the kitchen.
  • Observed floor with encrusted dirt build up inside walk in freezer.
  • Observed multiple overhead ceiling lights were out in food preparation areas in the kitchen.

Armetta's Italian Grill & Pizzeria: 4585 Daisy Reid Ave #109 Woodbridge, VA 22192

  • A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils, or after engaging in any activity that could contaminate the hands.
  • Observed ham, salami, mozzarella cheese, and wrapped ham slices stored at improper cold holding temperatures.
  • Observed multiple pans without dates of preparation/expiration.

  • Wet mop is not placed in a position that allows it to air-dry without soiling walls, equipment, or supplies.
  • Observed multiple burnt-out light bulbs over the grill.

Violations:

  • The Person In Charge (PIC) was not able to provide documentation that all food employees or conditional employees were trained and informed of their responsibilities to report symptoms or diagnosis of, or exposure to, diseases transmissible through food.
  • Knives were observed to be stored in between the prep table and the wall.
  • There were no ambient air temperature measuring devices (thermometers) in any of the refrigeration units in the facility.
  • Foods (mango coconut jelly, mango jam - coffee & mango flavors) stored in cabinets under wastewater lines from prep sinks.
  • Employees handling food were observed to not be wearing proper hair restraints.
  • The wet wiping cloths throughout the kitchen were not properly stored in sanitizer buckets in between use.

Violations:

  • The following handwashing sinks were found to have hot water at less than 100 degrees F: *handwashing sink, near the office, upon arrival, hot water, 60 degrees F; *handwashing sink, men's restroom, after the water heater was turned on, 91 degrees F.
  • The following food item(s) were observed hot holding at improper temperatures using a calibrated food temperature measuring device: mac n' cheese in the steam table at 124 degrees F.
  • The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cole slaw and shredded cheese on the cold holding prep unit.
  • Observed flies in the back near the water heater as well as in the dining area.

  • There is a large build up of ice in the walk-in freezer.
  • The temperature of the hot water at the 3 compartment sink was measured at 60 degrees F.
  • The hot water handle at the hand sink along the front line is in disrepair (missing) and the water has been turned off at the sink completely; there is a leak at the mop sink.
  • Shiny silver tape used to seal junction between soda dispenser and wall up front; shiny silver tape used to seal hole in wall at mop sink.
