The Pfizer COVID-19 vaccine for ages 5-11 is becoming available across King County, Washington state and nationally. Availability varies by location. Use the links below to find a COVID-19 vaccine clinic near you:

All BSD Vaccine Clinic Slots for 11/05 and 11/06 Have Been Filled

In partnership with Safeway/Albertsons

We are working with our community vaccine partners to organize future clinics based on need.

The Pfizer vaccine has been approved for children aged 5 and older. Appointments are available to all BSD students aged 5 and up. To be eligible for the Pfizer vaccine, your student must be aged 5 or older. We are also offering the flu shot for students!

Clinic at Odle Middle School: All appointments have been filled.

• When: Friday, November 5, 2021, 5:00 p.m. to 8:00 p.m.
• Where: Odle Middle School, 502 143rd Ave NE, Bellevue, WA 98007

Clinic at Sammamish High School: All appointments have been filled.

• When: Saturday, November 6, 2021, 9:00 a.m. to 5:00 p.m.
• Where: Sammamish High School, 100 140th Ave SE, Bellevue, WA 98005

Sign Up for the COVID Vaccine

Sign up using this link to schedule a COVID-19 vaccine appointment for either Friday, November 5th or Saturday, November 6th. Select ‘Odle’ as the location for 11/5 and ‘Sammamish’ as the location for 11/6.

You can sign up for your child to receive the COVID vaccine, the flu shot, or both! If you want to get both shots, please let the pharmacist know when you arrive.

Consent and Release

If you have access to a printer, please complete and print the Informed Consent and Release of Information form (link is available after you sign up for an appointment through the link above), and bring it with you to the clinic. We will also have copies of the form available for you to complete at the clinic. ***Parent/guardian signature on this document is required if the person being vaccinated is under age 18. 

Parent/guardian must accompany all children receiving the vaccine who are aged 5-15 years old.

Things to Bring with You

At check-in please bring with you and have your health insurance card ready. If you do not have health insurance, you will not be turned away. You must have health insurance to receive a free flu shot. If you do not have insurance, you will have to pay out-of-pocket for the flu shot.
