Is Your Loved One Special Needs? Police Would Like To Know, Too
It's voluntary and confidential: Waukesha police have a special needs form that people can fill out to help officers in a time of need.
WAUKESHA, WI — April is Autism Awareness Month and the Waukesha Police Department is making the public aware that they have a form that assists officers in providing the best service they can to those people with special needs.
Nearly five years ago, the Department began gathering information that they could have on hand to better serve those with special needs.
This information is gathered by completing a “Special Needs Form” so that officers know the likes and dislikes, the triggers or sensory issues, as well\ as de-escalation techniques that could be used when coming in contact with those citizens with special needs.

This is completely voluntary and people can choose which parts of the form they would like to complete and it is kept confidential. The information on this form, including the photo, will be entered into to the Waukesha Police Department database.
When a police officer has contact with the person listed on the form, the Emergency Communications Center can provide first responders with the information needed to successfully interact and communicate with your loved one, as well as your contact information.
