TransitionalCareServices.com Introduces Mobile App to Reduce Re-Hospitalization

TransitionalCareServices.com is now offering its cutting-edge hospital-to-home transitional care services via a new mobile app.

TransitionalCareServices.com (TCS), which focuses on hospital-to-home transitions, recently made its services available via a mobile app. “The downloadable app allows you to request a certified transitional care manager (TCM) or home health nurse while you are in the hospital to assist with all issues regarding the transition to home,” said Richard Weatherman, of TransitionalCareServices.com. “It’s similar to Uber in that it’s on-call and you hire direct.”

Post-discharge is a vulnerable time for patients, and most re-hospitalizations within 30 days can be prevented by using TCS services. Utilizing the TCS app can help ease the burden and reduce the risk of re-hospitalization. Transitional Care Managers also go to the home prior to hospital discharge to ensure it is prepared for the care of the patient. Services are designed for the specific needs of each patient.

“This will be a national effort, and soon we will be able to service any hospital nationwide,” concluded Weatherman. “We are also in the future going to add several other levels of services, such as post-procedural care, and are always looking for certified home health nurses and TCM-certified RNs.”
