
The Berlin Town Clerk’s Office will be participating in Passport Day on Saturday, April 9, 2011, during the hours of 10am to 2pm.
Important Items to Remember:
As of April 1, 2011, each passport application must have the long- form birth certificate attached to it. The short form birth certificate will no longer be accepted.
If the long-form birth certificate has been issued by the Berlin Town Clerk’s office, the application cannot be processed by this office.
Application forms are available at www.travel.state.gov website or the Berlin Town Clerk’s office. The applicant, or a parent or guardian for a minor, must complete the application. Do not sign the application until you have been instructed to do so by the Town Clerk’s office. The applicant must take an oath before signing.
The application fee to the U.S. Department of State must be paid by check or money order. Each application must have its own check or money order. Fees for multiple applications cannot be combined.

Passport Book for Applicant 16 or Older, $110
Passport Book for Applicant Under 16, $80
Passport Card for Applicant 16 or Older, $30
Passport Card for Applicant Under 16, $15

Applicant must present:
Certified long-form birth certificate or nationalization papers
Proper photo ID (e.g., current driver’s license)
One passport photo
Check or money order for U.S. Department of State
Check or cash for the Town Clerk’s office
