Nurse Managed Clinic

A Nurse Managed Wellness clinic is held at the North Stonington Wellness Center on the first Wednesday of the month, from 11:30 AM - 12:30 PM, unless the first Wednesday is a holiday. This clinic is sponsored by Visiting Nurse Association of Southeastern Connecticut.

The clinic will offer:

· Personal Health Record for each resident

· Health Assessments & Wellness Screenings

· Safety Assessments & Exercise – mobility activities

· Brown Bag medication checks & Education Programs

· Access to community resources – Basic case management

· A nurse that you can talk to
