Wallingford Nursing Home Fined for Numerous Violations

The state Department of Public Health also ordered the nursing home to hire a nurse consultant to monitor its care of residents.

The state Department of Public Health has fined Village Green of Wallingford Rehabilitation and Health Center $7,500 for numerous violations that were discovered during unannounced state inspections last October.

Kate Farrish of the Connecticut Health I-Team reports that Village Green has also been ordered to hire a nurse consultant to monitor its care of residents for at least three months.

The violations inspectors found included a failure to notify doctors of weight changes or when medication was not available, a failure to adequately investigate residents’ grievances, instances of abuse and neglect and a failure to maintain residents’ rooms in a clean, sanitary manner, according to the report.

Farrish reports that a licensed practical nurse said in a loud manner, “I’d like to slap her,’’ in front of one resident on Oct. 28, 2014.
