Remarks: No Certified Food Safety Manager (CFSM) certificate posted at the time of inspection. Violations for stocking a hand wash sink, cold holding, cooling and food separation (amongst other good retail practices) were observed today. Employee drinks in multi-use cups, with no straw, stored on the counter next to customer self-service food. No soap at two hand sinks. No paper towel at 2 of 4 operable hand sinks. One hand sink not working. Unwashed produce stored above washed produce, ready-to-eat foods in the upright coolers. Washed produce stored uncovered in upright coolers. Potentially hazardous foods stored >41F multiple coolers. Noodles, fried tofu not cooled to 41F within 6 hours. Ineffective cooling methods used to cool foods. Raw meats thawing at room temperature in the prep sink. Food containers stored on the floor in the walkin/upright cooler. Using gloved hands, employee handled dirty dishes and then clean dishes without washing her hands. No chlorine test kit available for measuring sanitizer concentration. No hot or cold running water provided at the hand sink next to the walkin cooler. Live cockroaches (multiple life cycles) observed in the kitchen, by the ice machine and by the upright freezer. Grease buildup on the cookline ventilation hood.
