• Several time/temp control items out of temperature in prep units (temps 47-50°F). Items discarded/relocated and service was called for units. Cold, TCS foods must be held at 41°F and below.
• Cooler gaskets and interior doors are damaged. Repair damaged areas.
• Excessive caked food buildup on cooler handles on prep line. These handles must be cleaned (sanitizing recommended) daily to prevent this type of buildup. Equipment on cook line has food/grease buildup and must be cleaned. Interior of glass door cooler in bar has food spills and must be cleaned
• Remarks: Consumer 'cooked to order' disclosure missing and applicable items not marked on menus; this must be corrected before offering items less than well done. Need signed agreement to report illness for each employee and cleanup procedure in case of illness event in facility.

• Observed several open top employee drinks stored in back food areas and at sushi bar. Employee beverages must be in disposable cup with lid and straw if stored in food areas.
• Observed employee use prep sink to wash hands rather than hand washing sink. Use dedicated sinks for handwashing. Person-in-charge (PIC) was educated on handwashing. Hand sink in dish area did not have working soap dispenser. Soap was replaced on request.
• Hand sink in back kitchen area soiled with food debris. Clean and maintain hand sinks in clean condition.
• Interiors of rice warmers have films of food residue and are not being cleaned thoroughly after use. Rice warmers must be cleaned and sanitized thoroughly at end of each day. Large plastic gloves used to handle raw meats (for mixing) observed with encrusted food residue. Gloves taken to dish area for proper cleaning/sanitizing.
• Plastic covering on bamboo sushi roller not being changed routinely. Change plastic every 4 hours.
• Employee working at hibachi grill without hair restraint. Hair restraints must be worn when preparing food.
• Inspection score of 91 from December 2015 posted at counter. This is not the most recent report. Current inspection report must be posted.
• Cooler exteriors and shelving in back kitchen have food grease buildup and must be cleaned. Ventilation systems above grill are accumulating grease/dust and must be cleaned
• Remarks: Cardboard dumpster area must be maintained; Add proper consumer advisory statement to paper menus for sushi items. Also need to add proper advisory statement at buffet when serving Greenshell mussels--these mussels are not fully cooked according to advisory on box.
