• Observed build up on can opener and other food equipment. The manager must keep food and non food contact surfaces clean.
• Observed build up on salad dressing containers and must keep equipment clean.
• Observed accumulation of debris on warewashing machine drain board and associated equipment. The manager must keep dish machine and area clean.

• Croutons not covered in storage. PIC provided lid
• Grease and food residue coating on pizza pans, squeeze bottles and other equipment stored as clean. Grease film on interior of food prep sink also. Food contact surfaces must be washed, rinsed and sanitized properly to remove grease/oils.
• Observed 2 employees preparing food without hair restraints. Hair restraints must be worn when prepping food.
• Observed product build-up throughout kitchen on equipment exteriors. Establish cleaning schedule and maintain.
• Cold water knob at hand sink at pizza make line not working properly. This must be repaired.
• Floors and walls in dish pit and under equipment are not being regularly cleaned and show build-up. Clean and maintain.

• Observed large container of salsa in waitress area in ice water, but more than one half of container not submerged in water/ice--temped at 54°F. Portion was halved and placed in walk-in for rapid chill. Do not place product above ice water line of container.
• Times being used as product control measure for portioned salsas, but no written procedure available. Must post procedure.
• Drain at vegetable sink not attached to sink -- fell off when touched. This must be repaired. Drain at meat sink is also leaking. This must also be repaired.
• Walls and base tile in very poor condition in kitchen area. Ceiling tile in back is unapproved and is black from smoke damage. Floor tiles are damaged in areas and must be repaired. Products on floor in beer and dry storage room. Keep storage elevated to allow for cleaning.
