• Facility does not have a dedicated Certified Food Safety Manager (CFSM) with routine food management responsibility. Must have at least one person with food management role on staff enrolled in course within 7 days.
• Facility does not have reporting system requiring employees to report symptoms and illnesses as required. Forms have been provided to establishment. Obtain and implement forms.
• Open employee beverage on prep table. Beverage was discarded. Open can of employee drink in prep cooler. Drink was discarded. Employee protein powders and gum stored in food equipment area with equipment and to-go bags. All items relocated on request. Corrected On-Site.
• Upon arrival, one hand sink was covered with tray of wontons and chef knives were stored in other hand sink. Both sinks were made accessible on request. Corrected On-Site.
• Facility does not have cleanup procedure for illness event. Forms have been provided. Obtain and implement procedure.
• Interior of ice machine heavily coated with mildew. Ice machine must be properly cleaned and sanitized at end of service today.
• Multiple items in walk-in at 45-46°F. Walk-in cooler is not maintaining proper food temperatures. Service was called for unit and freezer door was opened to help lower temperature.
• Small prep unit at fryer: multiple items at 47-48°F. All items relocated to freezer that were Time/Temp Control Foods (TCS). Unit temp must be adjusted before placing food in this unit.
• To-go containers soiled with grease and oil in cooking area. Containers were discarded. Store to-go containers in clean location.
• A follow-up inspection was scheduled, but no date was provided.

• Several seasonings in containers without proper label. Label bulk seasonings after removing from original container.
• Screen door in disrepair and gaps are visible around door. Repair screen and address gaps at door (both back doors). Also, repair gap at floor/wall juncture across from walk-in.
