• CFSM certificate expired Feb 2016. Must renew certificate within 30 days.
• Observed working performing cooking tasks with Band-Aid on finger without protective glove. Cuts on hands must be protected with waterproof bandage and glove when working with food. (Employee left kitchen during inspection. Concern addressed with both manager and owner).
• Observed employee using 3-comp sink for washing hands without soap, repeatedly. Employee was corrected each time and directed to hand sink. Employee complied with requests. Hands must be washed with soap in designated sink. Employees must be trained on proper hand washing.
• Observed several pans of chicken, which had been previously cooked (greater than 24 hours) without dates. These items must be date marked.
• Dumpster lid missing and must be replaced.
• Debris on dumpster pad must be cleaned.
• Add weather stripping oat top and bottom of back door.
• Employee items must be stored in employee area and not stored with consumer items.

• Baskets of wontons stored on tables in kitchen without cover. Cover these items in storage.
• Dish machine not sanitizing ("0" ppm cl). Dish machine service called and 3-compartment sink set up for sanitizing.
• Eggrolls and chicken prepared and held more than 24 hours not date marked. PIC placed dates on all items. Packages of tofu, once opened, must also be date marked. A container without date was correctly labeled.
• Observed several working bottles of chemicals not labeled. PIC labeled all items.
• Observed RAID bug spray in kitchen that was not approved for kitchen use. Item removed from establishment.
• Dumpster top lids are missing and must be replaced.
• Replace weather stripping around screen door.
• Lights in back storage room must be working. One light out.
• Lights in back storage room not shatterproof or covered. This must be corrected.
