• Pans of raw chicken stored on shelf above corn tortillas in walk-in. Items were rearranged on request. Follow storage chart that is posted in walk-in. Do not store raw meats/fish/chicken above tortillas or other foods that do not require further cooking.
• Excessive flies in kitchen area. Fly problem must be addressed using approved pest control methods only. Discussed acceptable methods with PIC.
• Back door propped open and fly fan at door is effectively preventing flies from entering kitchen. Keep door closed until screen door can be installed.

• Ice machine has heavy accumulation of mildew film on interior. Increase cleaning frequency of machine. Machine must be thoroughly cleaned and sanitized at end of business today. Mildew film also observed on ice dispenser/drink nozzles at self-serve station. This must be cleaned at close of business today.
• Food service gloves stored beside and below chemicals in chemical closet. Gloves relocated on request. Do not store gloves with chemicals.
• Faucet in women's restroom in disrepair and must be replaced. Rusted shelving in cooler units must be replaced.
• Floors under equipment at drive-thru, on cook line and in sliding rack area are very soiled with food spills and trash. Improve cleaning in these areas. Also need to clean interior cabinet under self-serve drink area. Address cleaning issues discussed in dining area.
• Remarks: Employee preparing food must wear hair cover.
