• Observed encrusted residue on slicer used for slicing meats. Slicer must be thoroughly cleaned and sanitized after use. Clean and sanitize before next use. Interior of ice machine behind deflector covered with film of mildew accumulation. Clean and sanitize machine at end of service today. Interior of salad prep top unit heavily coated with food spills. This unit must be properly cleaned.
• Large pizza prep unit missing thermometer; reach-in unit in pizza prep thermometer not visible. Keep thermometers in visible location in all unit.
• Salad prep unit in poor condition (top lid, door gaskets). Handle broken on prep unit @ grill. Damaged equipment makes cleaning difficult and may impair proper function. Repair and maintain equipment. Lids of pizza make table no longer fit unit properly-repair.
• Encrusted build-up on doors of reach-in units. Clean door handles regularly to prevent buildup and reduce cross contamination potential.
• Floor tiles missing in kitchen must be replaced; cabinets in pizza prep area must be cleaned and repainted with washable paint; floors under equipment in bar are very soiled and must be cleaned; walls in dish-area have worn and need to be resurfaced.

• Person-in-charge (PIC) is not a CFSM and does not demonstrate thorough knowledge of GA Food Code requirements, nor practices to ensure food safety guidelines are followed. PIC must receive further training.
• The Certified Food Safety Manager (CFSM) is responsible for training employees on GA Food Code requirements and for training employees on safe food handling practices. CFSM must train staff, including PIC, who manage operation when CFSM is not present.
• Observed multiple food items in facility which had been prepared in private home. All items identified and either discarded or removed from facility. Home prepared food are prohibited.
• Remarks: Copies of health policy and emergency cleaning procedures have been provided and must be available on request; Obtain chlorine test strips; Install closer on back door and on front door; Kitchen has been expanded and menu has changed without notification.
