Restaurant Inspections: Roach Infestation, Sewage Backup
The Cherokee County Environmental Health Department has closed Mandarin House II in Canton until the problems are repaired.

- Evidence of no oversight of food management responsibilities; failure of person in charge (PIC) to perform duties as required by GA Food Code 511-6-1-.03(2)(a)-(o). Multiple risk factor violations observed.
- PIC had copy of ServSafe certificate of an employee of another permitted facility. This person is only present once a week and the responsibilities of the Certified Food Safety Manager (CFSM) are not being met. There must be a person with food management oversight present on a daily basis who is knowledgeable and responsible for all food operations.
- PIC has limited knowledge of employee symptoms requiring reporting and there was no knowledge of illnesses requiring reporting. Employee reporting information must be available and PIC must be knowledgeable. Handouts provided (decision guide and reporting agreement). Corrected On-Site.
- Kitchen hand-washing sink was turned off and had a box fan and a bag of rice stacked on top. PIC removed items and turned sink back on (this is the only hand sink in the kitchen). Corrected On-Site.
- PIC does not have procedures for cleanup of vomiting/diarrhea event. Handout will be provided.
- Observed cooked chicken placed in boxes in which raw chicken had been received. Do not reuse containers in this manner. Chicken (2 boxes) was discarded. Corrected On-Site.
- PIC was asked to discard out-of-temp items in walk-in cooler, including cooked chicken which was placed in boxes that raw chicken had been received in and was out of temperature. One employee carried chicken to personal vehicle rather than complying with request to discard. A second request was made for employee to remove chicken from vehicle and place in dumpster. Employee complied with this request. Corrected On-Site. New Violation.
- Walk-in cooler at 47°F; prep cooler across from grill at 62°F (ice packs had been placed on the few items in this unit). Cooked rice in walk-in unit at 44°F, 45°F; cooked chicken in walk in at 51°F, 52°F; chicken broth (made in house) at 45°F in walk-in. All items discarded. All items in prep top cooler also discarded. Corrected On-Site.
- Observed cooked rice in walk-in which had been cooked 3 days ago, according to PIC, without dates. Rice was also out of temperature and was discarded. Corrected On-Site
- Observed multiple food items stored in freezer units in Food Saver bags. These were cooked items such as rice with chicken/beef with noodles that PIC stated she cooked and placed in bags. These items were packaged in individual servings. Discontinue use of Food Saver system. This system may not be used without first consulting our office. PIC stated these were items for donation to charitable organization. PIC was instructed to remove these items from the facility immediately.
- The walk in cooler was not working with ambient temperature at 47°F. The prep cooler located across from the grill was not working with ambient temp at 62°F. Two other reach in refrigerators had stopped working. Refrigeration units need min 41°F temperature (36-38°F recommended) to allow for use of unit and to keep product temps at 41°F or below. Units that are not working need to be removed from facility if they cannot be repaired.
- Large rice warmer had broken/missing plastic cover on lid hinge. Foil was being used as cover. Replace hinge so that warmer can be properly cleaned and sanitized. Several of the wire racks in the walk-in unit had severely rusted shelves and were covered with foil. Replace this rusty equipment. Also, foil covered shelves restrict air-flow in the cooler
- Plumbing for vegetable sink hanging from wall in disrepair. This must be repaired so that vegetable sink is in working order.
- Sewage malfunction/blockage continues in the kitchen. After running the dish machine two times, waste water runs out of the grease trap top across floor to floor drain. Grease trap sitting on rock bed and overflow waste forced out from under unit. Construct solid floor under grease trap unit and seal unit to floor.
- Several residential type freezers have been installed in dining areas on top of carpet. These units must be relocated into kitchen area where approved flooring material is in place.
- The floor is in poor repair in kitchen and in women's restroom. These areas must be repaired.
- Facility is very cluttered inside and out. Eliminate unused equipment, broken equipment and employee items not necessary for restaurant. This will aid in facility cleanup and make room for needed items.
- Facility is infested with roaches. Roaches were seen running up walls, under equipment, and in food preparation areas. Must get this under control immediately. Roaches at this level indicates poor clean-up at this facility.
- Facility was closed on Sept. 24, 2016 due to sewage backup in kitchen. Facility must remained closed until sewage problems is fully repaired and all refrigeration issues are corrected.

- A follow-up inspection is planned for Oct. 6.
