  • Person in charge lacks awareness in key areas of risk factor control or good retail practices for establishment / CA: Persons in charge, managers or CFSM must be properly trained or retrained.
  • Observed employee wash hands in prep sink. CA: Discussed using only hand sink for handwashing with owner and PIC.
  • Observed employees using bare hands to apply seasoning to fries and grabbing cut lemons. CA: Discussed proper procedures with employees and owner and had them throw out food. Corrected On-Site
  • Observed employee drinking from an open beverage container in a food preparation or other restricted area. CA: Discussed proper employee drinks with owner and PIC.
  • Observed food stored on floor (chicken). CA: Discussed with PIC how all food should be stored at least 6 inches off the floor.
  • Observed buildup of slime in the interior of ice machine. CA: Discussed with owner cleaning frequency, he cleaned on-site.
  • Proper consumer advisory with asterisks not present on menu. CA: Discussed proper advisory with owner.
  • No hand-washing sign provided at a hand-sink in men's bathroom. CA: PIC was notified of the issue.
  • Remarks: Discussed proper response procedures for vomiting & diarrhea events. Will check during re-inspection.
  • A follow-up inspection is set for Sept. 10.

  • As observed in the previous inspection, worker was seen handling ready to eat lettuce with bare hands when glove or utensil use is required. CA: The issue was discussed with PIC and food discarded, and worker was told to wear gloves.
  • Observed multiple dirty towels/wash clothes piled up in hand washing sink, making it inaccessible to employees. CA: Advised PIC to remove towels and to only use hand sink to wash hands.
  • Observed cooked ham on prep table in very close proximity to hand sink were no splash guard is present. CA: Advised PIC to discard food and to either install a splash guard or prep foods away from the hand washing sink.
  • Observed pink buildup inside corners of ice bin. CA: Discussed with PIC about cleaning the ice bin more frequently.
  • Observed quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Discussed with PIC about the issue and suggested that they change sanitizing buckets more frequently. The sanitizing bucket was replaced with a new solution at proper strength.
  • Observed in the main kitchen area next to the flat top, potentially hazardous foods (white gravy, sausage gravy and brown gravy) not held at 135 degrees Fahrenheit or above. CA: The issue was made known to the PIC and the items were discarded. It was then discovered that the hot holding unit was not turned to a high enough temperature to keep the items at 135 Fahrenheit.
  • Observed cooked potentially hazardous food (cabbage and pasta and shredded chicken, not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. CA: Discussed with PIC proper cooling techniques; food items were discarded.
  • Observed cell phone sitting on top of paper towel dispenser. CA: Discussed with PIC about ensuring that employees use designated areas for personal items.
  • Observed live flies in kitchen. CA: Discussed with PIC about increasing pest control efforts.
  • Remarks: This facility had a few issues that have been frequent violations in past inspections. One repeat violation, was bare hand contact when preparing ready-to-eat foods. After this was brought to the worker's attention, the worker seemed perplexed as to why she couldn't touch the items with her bare hands. This prompted the suggestion that staff be retrained on the safety issues concerning bare hand contact with ready-to-eat foods, and when it is and isn't appropriate to use gloves when preparing food. Will return to the facility within 10 days for follow-up inspection.
  • A follow-up inspection is scheduled for Sept. 16.

  • Observed tea containers open with no lids and subject to contamination. Discussed with PIC about the need to cover drink containers to prevent debris from falling in.
  • Observed a water container (cup) for cooking not labeled. CA: Discussed with manager how all food containers must be labeled as to their contents.
  • As Observed in previous inspections, the lights were not functioning in first two walk-in coolers. CA: Discussed with PIC about the need to have working lights with the walk in coolers. PIC voiced that the electrical wiring was in the process of being corrected.
  • Observed dead roaches on premises in the walk in the middle walk-in cooler. Observed live flies in kitchen. Discussed with PIC about Pest control devices being used, and to always remove dead insects from floors especially in kitchen and storage areas.

OKINAWA (Food Service Inspections)*

  • Observed two workers, in the back prep area seen chopping & handling ready to eat lettuce with bare hands when glove or utensil use is required. CA: Discussed with pic about when to use gloves, and why gloves are need during preparation of ready to eat foods. The food that was touched with bare hands was discarded.
  • Observed employee drop freshly cut lettuce in basin of middle section of the three compartment sink, and the contaminated food was not discarded, but returned to the bin with clean uncontaminated (ready to eat) lettuce. CA: After food was discarded, it discussed with manager and employees about why the food needed to be discarded and how and why the food got contaminated.
  • Observed pink build up on the interior of the ice bin. Also, observed food debris in containers where clean utensils were being stored. CA: PIC was notified of the issue who then voiced that the ice and dish bins would be clean.
  • Observed vegetables being washed in three compartment sink where meat also said to thawed (per previous report). CA: In previous inspection the PIC was asked about procedures for washing vegetables, some of which are ready to eat foods, and thawing raw animal foods as it pertains to sink designations. Also in the previous inspection, the PIC explained that employees used a prep sink and three compartment sink to both wash vegetables and thaw and wash raw animal foods. There was a prep sink designated for washing vegetables only, but it was being used for other unspecified purposes other than washing vegetable at the time of the inspection. As noted in the previous report, the PIC was then notified about the need to change these procedures immediately. Repeat Violation.
  • Observed grease accumulated under and behind cooking equipment. Observed grease accumulated under cooking equipment. PIC was notified of the issue. It was discussed the need for more frequent cleaning, and PIC voiced that the equipment would be clean.
  • A follow-up inspection is scheduled for Sept. 10.

RANCHO ALEGRE (Food Service Inspections)*

  • Observed expired Food Manager Certification.CA: Discussed with the PIC who voiced that she has taken the course to have it renewed but has yet to receive her copy. An informal visit to the facility will be conducted in 7 days to see if the CFSM certification is posted.
  • Observed fried pork found stored on the end of 3 compartment sink uncovered and subject to contamination. CA: Discussed with PIC who the moved the item to another location.
  • No time marking on containers of food( fried pork) when time as public health control is used. CA: Discussed with PIC risk factors involved with not time marking RTE foods and why it is important to be consistent with the procedure.
