Northside Hospital is happy to provide the COVID-19 vaccine to people who are age 65 or older by appointment only. Appointments are being scheduled for Friday, March 5, 2021. Below is the link to schedule an appointment and complete the online COVID-19 vaccine consent form. Please remember to bring the email confirmation or text confirmation of your appointment and your picture ID with you. You must wear a mask to your appointment. Northside Hospital - Scheduling (blockitnow.com) You will receive your vaccine at the Northside Hospital Conference Center, located at 1130 Bluffs Parkway, Canton, GA 30114. Please plan to arrive at your appointment on time and no more than 10 minutes early, as space in the waiting area is limited. You MUST receive both your first and second dose at the same location.
