Violations: 13A, Violation of Code: [.07(3)(d) ] Employee handwashing sign not posted in the mens restroom. The person in charge was advised to obtain a new handwashing sign for the restroom as soon as possible. New Violation. 14C, Violation of Code: [.05(10)(e) ] Single-service articles not stored 6 inches above the floor, in a clean/dry location. Several boxes and open sleeves of single use items such as plastic cups were found stored directly on the floor of the facility. The person in charge was advised to move these items to a clean dry location at least 6 inches off the floor to help prevent possible contamination from pests and other sources. New Violation. 16B, Violation of Code: [.06(2)(j) ] Backflow prevention device needed for inside fixture where the hose is attached. The person in charge was advised of the need to obtain a back flow prevention device for this fixture when a hose is attached to help prevent backflow into the system. New Violation. Remarks: Violation of Code: [.04(4)(q) ] Food not stored at least 6 inches above the floor. Three items of food were found stored directly on the floor of the walk in cooler and freezer. These items including pickles, liquid margarine, and frozen beef were removed for proper storage at least 6 inches off the floor to help prevent contamination from pests and other sources. Corrected On-Site. New Violation.
