Observations: Soap, for hand washing, was not supplied or available at the back hand wash sink. Bottle of oil was not labeled with the common name of the food. Several lids are cracked and spatulas in disrepair. Contact paper is being used to line the top of the salad prep cooler. Contact paper is not a washable/durable material. Nonfood-contact surfaces of equipment not kept free of an accumulation of dust, dirt, food residue, and other debris. Food containers, shakers, etc. unclean with old food debris, grease and dust. 3 compartment sink unclean with black mold-like substance. The men and women’s restroom doors do not self-latch. Floors throughout the facility need a really deep cleaning to remove old food debris and dust.
