Remarks: Employees drinking coffees in the food prep area: Coffee stored over the pizza prep station. The microwave plate and cavity were unclean with dried residue from cheese sauce. Pizza boxes were not inverted to protect the food-contact surfaces. The boxes had been removed from the original protective sleeves. Employee turned the boxes over the correct this. Single-use items on which food will be served must be either stored covered or inverted. Several food containers were cracked and broken. Lids on bulk flour and cornmeal were damaged. Soiled cardboard is being used as shelf liner in multiple locations in the restaurant. excessive mildew present on shelving inside the walk-in cooler. Dumpster is missing both lids. Damaged/open wall base near the compartment sink. Garlic sauce accumulated under the counter ledge near the front warmer. Food residue accumulated at the coving along the wall near the walk-in cooler and other spots in the kitchen. Pizza sauce residue on the wall behind the pizza cooler. Complete cleaning where needed and repair the wall.

Remarks: Large container of salsa at 69 deg. F cooling in walk in cooler (covered). Pans stacked wet. Plates, bowls contaminated with debris due to where they are being stored. Exterior surface of pans with old glue, and label residue. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
