Remarks: Chicken was covered in walk in cooler during the cooling process. Shrimp was thawed and immediately put on prep line after the thawing process to cool to 41. Items should be left uncovered during the cooling process. Adequate cooling methods shall be used. Beef in meat sink was laying in sink and not submerged. Beef should be in a container and fully submerged so there is an overflow out of container.

Remarks: Observed Certified food safety manager and employee touch ready to eat food with bare hands. Had food safety manager and employee discard green onions and fried tofu. Observed ice machine with mold build up. Had manager clean and sanitize ice machine. Observed rice noodles in water and cooked onion being kept at room temperature.
