Remarks: Physical adulteration of ice cubes: Dark colored matter was present on ice cubes inside the large automatic ice maker in the kitchen. Some pieces were gray-black and some were brown. The ice cubes were discarded. Accumulations of thin, whitish fungal growth resembling mildew are present on the food storage shelving inside the walk-in cooler.

Remarks: Multiple trays of portioned mozzarella cheese were not being maintained at 41 F or lower inside a reach-in cooler. Several food items were not marked with thaw dates after the products had been cooked/prepared, frozen, thawed, and placed into prep line coolers. Chafing fuels (Sterno brand) were stored on the top of the dry storage shelf above to-go cups and sandwich wrapping paper. The Fahrenheit thermometers inside multiple reach-in coolers were not providing temperature measurements within an approved range of accuracy.

Remarks: Multiple loads of dishes were being washed in the dishwashing machine which was not dispensing any chlorine sanitizer. Chicken wings held above the fryer were not discarded at the end of their holding time. Several portioned cups of creamy sauce (containing spinach, half/half, and cheese) were held in use past its refrigerated holding time. Mold present on the wall at the compartment sink.  Food debris and grease residue was present underneath equipment such as the main prep table, soda dispensing machine, etc. Floor tiles broken/missing in multiple spots. Clean the walls, sweep and mop the flooring. Replace the tiles. The facility must be maintained clean and in good repair. Adult roach was crawling up the wall at the vegetable sink. There were daylight gaps at the base of the kitchen exit door. Dead insects were present in some of the lighting shields in the prep area.

Remarks: No one in charge at beginning of inspection. Scallops, shrimp, and squid stored in ice and water. Stickers not being removed from dishes after washing. Several dishes and pans with old stickers.
