Observations: Employee drink being stored in an unapproved, uncovered container in the food prep or food storage area. At time of inspection, observed employee drink without lid and straw being stored on top shelf of clean dish rack. Drink was stored above clean dishes. At time of inspection, observed container of washed tomatoes in manual dish wash sink. Tomatoes were being washed in manual dish wash sink. Facility usually has pre-washed/pre-sliced tomatoes; however, person in charge stated facility ran out of pre-washed/pre-cut tomatoes and bought some unwashed, uncut tomatoes until next delivery. Tomatoes were discarded. Advised person in charge facility must only use pre-washed/pre-cut vegetables and fruits at all times. If facility does decide to wash vegetables, a code approved vegetable/fruit sink must be installed. Non-food contact surfaces not clean. Exterior of mechanical dish wash area not clean (buildup of debris).

Chicken cooling in deep, covered, plastic food containers, stacked 2 high, in the walkin cooler. Ineffective cooling methods used. Had PIC unstack and uncover food. Chicken temperatures measured at 10:35 and 11:21; food temperatures remained at 47F, 48F. Containers of coleslaw stored in undrained ice in the line cold top cooler; coleslaw containers were not waterproof. Employee wearing 2 bracelets while preparing food. Jewelry removed.

Observations: OBSERVED MOLD LIKE SUBSTANCE SUSPENDED FROM ICE MAKER IN ICE MACHINE. ICE IN ICE BIN HAD MOLD LIKE SUBSTANCE ON ICE IN THE BIN. ICE DISCARDED AND MACHINE WILL BE CLEANED OVERNIGHT. OBSERVED IN USE KNIFE WIPED ON CLEANING CLOTH HANGING FROM HOOK NEXT TO PREP COOLER. THIS CLOTH WAS NOT STORED CORRECTLY IN A SANITIZING SOLUTION. OBSERVED THRESHOLD AT BACK EXIT DOOR NOT SECURE TO ELIMINATE THE ENTRANCE OF VERMIN. A FOLLOW UP INSPECTION WILL BE CONDUCTED WITHIN 10 DAYS TO EVALUATE THE ICE MACHINE FOR CLEANLINESS

Observations: Microwave ovens in the kitchen had dried food residue inside and on their plates. Several foods, prepared and held for more than 24 hrs, were not marked/labeled with dates: alfredo sauce, black beans, andouille sausage, clam chowder, and gumbo soup. Several food containers had tape placed over open seams/cracks. Several lids were broken. Throughout the kitchen and in the restrooms, washable ceiling tiles have been replaced with acoustica tiles. The acoustical tiles had water damage and were buckling. Lighting was too dim at the beverage station, over the dishwashing compartment sink, and in the second dry storage closet.

Observations: Multiple potentially hazardous foods cold holding above 41 F in the True 2-dr reach-in cooler (no time control documentation). Food items (peeled hard cooked eggs, whole/2% milk) that were at a temperature of greater than 41 F were placed on time control True 2-dr reach-in cooler with an ambient air temperature of 64 F. Cooling units used to cold hold potentially hazardous foods must be capable of maintaining the foods at 41 F or below.
