Observations: Multiple pots, pans, and other food containers that were stated as clean had old food debris, old stickers, and other sticky residue still on them. Cooked pork, bean sprouts, pepper sauce, cooked kim-chi, and other misc. items did not have a date mark on them. Multiple containers of oils, sauces, and spices not labeled with the common name of the food items.Stacking clean utensils wet. Storing clean utensils on the floor in the back storage area. Observed a few live roaches in the kitchen area. Pest control came out on 7-6-15 and is continuing to monitor and control the situation. Must continue to monitor and we will follow-up by 9-1-15 to ensure all pests have been eliminated.

Observations: hicken was not re-labeled after having been thawed and stored in the prep cooler. (2.) Cuts of deli meats were not labeled in the reach-in cooler. TCS foods held for more than seven days: macaroni prepared on June 13th and potatoes prepared on June 18th. The items were discarded.Breakfast toppings (brown sugar, cranberries, granola, etc) were on display for self-service without any shielding.
