Name and location: Waffle House #1700, 5970 Memorial Drive.

Score: 92/A

Previous scores: 96/A on Dec. 2, 2010 and 52/F on Dec. 1, 2010.

Violations cited: Black mildew was present on interior surfaces of the ice machine. Commercial sale food items were not properly disposed of past the sell-by expiration date.

AUG 2

Name and location: Dairy Queen, 5039 Memorial Drive.

Score: 96/A

Previous scores: 99/A on Oct. 7, 2010 and 100/A on Feb. 9, 2010.

Violations cited: Exterior panels of ice machine were beginning to collect dust and needed to be cleaned. Large and small roaches were observed crawling on the hot water heater inside the kitchen.

Name and location: Park Springs (Main Kitchen), 500 Spring House Circle.

Score: 82/B

Previous scores: 85/B on Jan. 26, 2011 and 90/A on Jul. 20, 2010.

Violations cited: Equipment food-contact surfaces were unclean; foods were not maintained at proper cold-holding temperature.

AUG 1

Name and location: Churchs Chicken, 5719 Rockbridge Rd.

Score: 94/A

Previous scores: 87/B on Mar. 8, 2011 and 100/A on June 25, 2010.

Violations cited: Lack of proper ventilation to keep kitchen from becoming overheated, equipment and floor tiles were not properly maintained and needed to be replaced. The back door of the kitchen was not effectively protecting the restaurant from weather or the entry of pests.

JULY 28

Name and location: Tony's Famous BBQ Wings, 567 Stephenson Rd.

Score: 83/B

Previous scores: 94/A on April 14, 2011.

Violations cited: Food did not have proper expiration dates marked. Fryer handles were unclean and had food buildup, fan in the main kitchen contained dust debris and food was not properly stored and covered.
