CHSD170 Releases Information on Mandatory Health Care Records for Registration in Grades K and 6
Vital health records must be completed before the school registration process may begin
Chicago Heights School District 170 is releasing to the parents of students entering Kindergarten and Sixth grade in August, 2015, the following information regarding dental, health and immunization requirements.
Illinois mandates that all students entering Kindergarten must have a new physical exam, a current immunization record, a new dental exam, a new vision exam. Any child entering Kindergarten must show proof of having received two doses of varicella: the first dose given on or after the first birthday, and the second dose given no less than 28 days after the first dose. Parents also may present at the time of registration proof of prior varicella disease, or laboratory evidence of varicella immunity.
Before entering the sixth grade students must have new physical and dental examinations. Students also must receive one Tdap dose regardless of the interval since the last DTaP, DT, or Td dose, and shall show proof of having received two doses of varicella: the first dose given on or after the first birthday, and the second dose given no less than 28 days after the first dose. Parents also may present at the time of registration proof of prior varicella disease, or laboratory evidence of varicella immunity. Effective with the 2015-16 school year, the State of Illinois also is requiring that all sixth grade students show proof of having received one dose of the meningococcal conjugate vaccine (MCV) on or after their 11th birthday.

All required health forms, i.e. Certificate of Child Health Examination, dental examination, and vision examination forms, may be obtained at any District 170 school, or at your doctor’s office.
Please note that these forms MUST be completed, dated and signed by a doctor before a student will be permitted to begin the registration process for Kindergarten or sixth grade.
