Born May 14, 1916 in Chicago to Martin and Hedwig Jurczak, Anna M. Polansky passed away April 24, 2012, peacefully at her home.

Anna worked 28 years for as a mail order-entry clerk.Ā  She provided a loving home and enjoyed crocheting, cooking, baking, and sewing.

Anna is survived by her daughters Priscilla (the late Edward Joseph Jr.) Caputo, and Janice Polansky; grandchildren Robb (Mary Jean) Caputo, Michaleen (Douglas) Hundman, and Tony (Jackquelyn) Caputo; great-grandchildren, Taylor, Clay, Nicole, and Courtney Caputo, Camri, Chase, and Cory Hundman, and Jacob and Alexander Caputo.

She was preceded in death by her husband Otto, four brothers, two sisters and her parents.

Mass of Christian Burial was held at St. Thomas the Apostle Catholic Church and burial was held in Crystal Lake Memorial Park.
